To scope or not-the challenges of managing patients with positive fecal occult blood test after recent colonoscopy

BACKGROUND Colorectal cancer(CRC) is a major health problem. There is minimal consensus of the appropriate approach to manage patients with positive immunochemical fecal occult blood test(iFOBT), following a recent *** To determine the prevalence of advanced neoplasia in patients with a positive iFOBT after a recent colonoscopy, and clinical and endoscopic predictors for advanced *** The study recruited i FOBT positive patients who underwent colonoscopy between July 2015 to March 2020. Data collected included demographics, clinical characteristics, previous and current colonoscopy findings. Primary outcome was the prevalence of CRC and advanced neoplasia in a patient with positive iFOBT and previous colonoscopy. Secondary outcomes included identifying any clinical and endoscopic predictors for advanced *** The study included 1051 patients(male 53.6%;median age 63). Forty-two(4.0%) patients were diagnosed with CRC, 513(48.8%) with adenoma/sessile serrated lesion(A-SSL) and 257(24.5%) with advanced A-SSL(AA-SSL). A previous colonoscopy had been performed in 319(30.3%). In this cohort, four(1.3%) were diagnosed with CRC, 146(45.8%) with A-SSL and 56(17.6%) with AA-SSL. Among those who had a colonoscopy within 4 years, none had CRC and 7 had AA-SSL. Of the 732 patients with no prior colonoscopy, there were 38 CRCs(5.2%). Independent predictors for advanced neoplasia were male [odds ratio(OR) = 1.80;95% confidence interval(CI): 1.35-2.40;P < 0.001), age(OR = 1.04;95%CI: 1.02-1.06;P < 0.001) and no previous colonoscopy(OR = 2.07;95%CI: 1.49-2.87;P < 0.001).CONCLUSION A previous colonoscopy, irrespective of its result, was associated with low prevalence of advanced neoplasia, and if performed within four years of a positive iFOBT result, was protective against CRC.

The effect of the thickness of the dielectric boundary layer that connects amaterial of refractive index n1 to another of index n2 is considered for the propagation of an electromagnetic pulse.A qubit lattice algorithm(QLA),which consists of a specially chosen non-commuting sequence of collision and streaming operators acting on a basis set of qubits,is theoretically determined that recovers theMaxwell equations to second-order in a small parameterǫ.For very thin but continuous boundary layer the scattering properties of the pulse mimics that found from the Fresnel discontinuous jump conditions for a plane wave-except that the transmission to incident amplitudes are augmented by a factor of√n2/*** the boundary layer becomes thicker one finds deviations away from the discontinuous Fresnel conditions and eventually one approaches the expectedWKB *** there is found a small but unusual dip in part of the transmitted pulse that persists in ***,the QLA simulations still recover the solutions to Maxwell equations even when this parameterǫ→*** examining the pulse propagation in medium n1,ǫcorresponds to the dimensionless speed of the pulse(in lattice units).

Plastic waste management has emerged as a critical environmental issue due to the exponential increase in plastic consumption *** terephthalate(PET)is extensively used in the production of water bottles,which constitutes a significant fraction of the plastic *** recycling is a challenging task due to the lack of efficient and cost-effective depolymerization *** this study,we developed a microwave(MW)catalytic depolymerization method for PET recycling using modified zinc oxide loaded with manganese oxide as a *** modified Mn_(3)O_(4)/ZnO catalyst presents high efficiency in depolymerizing PET into its monomers with only 0.4 wt% ratio of the catalyst to PET at 175℃ for 5 min,resulting in 100% conversion of PET and 88% selectivity toward bis-hydroxyethyl terephthalate *** is believed that Mn_(3)O_(4)provides additional Lewis acid sites,promoting the dissociation of glycol from PET,and the MW irradiation plays a crucial role in rapidly heating the ethylene glycol and the catalyst,thereby accelerating the PET depolymerization *** addition,the heterogeneous nature of the catalyst facilitates its easy separation from the reaction mixture for reuse,simplifying the catalyst recovery process and enabling costeffective and sustainable PET ***,this study provides an innovative and sustainable solution for PET recycling,contributing toward the circular economy and mitigating the environmental impact of plastic waste.

Tricube Weighted Linear Regression and Interquartile for Cloud Infrastructural Resource Optimization

Cloud infrastructural resource optimization is the process of precisely selecting the allocating the correct resources either to a workload or *** workload execution,accuracy,and cost are accurately stabilized in opposition to the best possible framework in real-time,efficiency is *** addition,every workload or application required for the framework is characteristic and these essentials change over ***,the existing method was failed to ensure the high Quality of Service(QoS).In order to address this issue,a Tricube Weighted Linear Regression-based Inter Quartile(TWLR-IQ)for Cloud Infrastructural Resource Optimization is introduced.A Tricube Weighted Linear Regression is presented in the proposed method to estimate the resources(i.e.,CPU,RAM,and network bandwidth utilization)based on the usage history in each cloud ***,Inter Quartile Range is applied to efficiently predict the overload hosts for ensuring a smooth *** results show that our proposed method is better than the approach in Cloudsim under various performance *** results clearly showed that the proposed method can reduce the energy consumption and provide a high level of commitment with ensuring the minimum number of Virtual Machine(VM)Migrations as compared to the state-of-the-art methods.

Archean-Ediacaran evolution of the Campos Gerais Domain-A reworked margin of the São Francisco paleocontinent(SE Brazil):Constraints from metamafic-ultramafic rocks

The Campos Gerais Domain(CGD)in southeastern Brazil is an approximately 180 km×35 km area of Archean–Proterozoic rocks located southwest of the São Francisco Craton(SFC).The Archean–Paleopro terozoic evolution of the CGD—alongside its potential correlation with the SFC or other cratonic blocks in the region-is currently *** present the results of systematic petrography,bulkrock geochemistry,mineral chemistry and geochronology for a suite of scarcely studied mafic–ultramafic rocks from the *** also provide a compilation of previously reported bulk-rock geochemical and spinel group mineral chemical data for mafic–ultramafic rocks throughout the CGD,and geochronological information for various lithotypes in the *** CGD records a protracted Mesoarchean to Statherian(3.1–1.7 Ga)crustal evolution,which we interpret to share a common history with the southern SFC and their related reworked segments,suggesting that it is a westward extension of this cratonic *** metavolcano-sedimentary rocks of the Fortaleza de Minas and Alpinópolis segments represent a Mesoarchean greenstone belt that is stratigraphically and chemically comparable to Archean greenstone belts worldwide,and that is broadly coeval with a local suite of tonalite-trondhjemite-grano diorite(TTG)gneisses and migmatites.U-Pb SHRIMP zircon data from a subalkaline metagabbro yielded a concordia age of ca.2.96 Ga,revealing a previously unrecognized phase of Archean magmatism in the CGD that can be chrono-correlated with metakomatiite and TTG generation elsewhere in the São Francisco *** data contradict a hypothesis whereby the metavolcano-sedimentary rocks of the Jacuí-Bom Jesus da Penha and Petúnia segments represent an ophiolite,as previously suggested,instead presenting features that point to formation in association with a continental *** with a U-Pb(SHRIMP)crystallization age of ca.2.13 Ga recorded by zircon grains from a metaultramafic rock,these data h
