Detection of Early Noise-Induced Hearing Impairment in Pilots

Objective To detect early signs of noise-induced hearing loss(NIHL) in military pilots without hearing complaints. Methods Pure tone audiometry and acoustic reflex thresholds were tested in 36 military pilots (72 ears) with noise exposure history but no complaints of hearing loss. Conventional test frequencies (0.25-8 kHz) and extended high frequencies (EHF, 10 and 12.5 kHz) were included in audiometry. White noise and pure tones at 0.5, 1, 2, and 4 kHz were used for acoustic reflex tests. Twenty normal hearing subjects(40 ears) with no exposure to occupational noise were used as controls. Results Pure tone thresholds at all conventional frequencies and at EHFs were elevated in the pilots, with the maximum shift at 4 kHz, compared with controls (p < 0.01). The pilots also showed elevated ART to white noise and decreased differentials between white noise and pure tone ARTs (p< 0.01). Conclusion Early signs of NIHL are present in some symptom-free military pilots. High frequency hearing threshold shift, elevated white noise ART and decreased differential between white noise and pure tone ARTs may be objective indicators of early NIHL.

Purpose : Effects of Tryptergium Wilfordii Polyglycosidium (TWP) on the Tumour Necrosis Factor (TNF) levels in aqueous humor after intraocular lens (IOL) implantation were studied in ***: Twenty-seven pigmented rabbits were divided into three groups. In the first group, the IOL were placed in the capsular bag after lens extraction. In the second group, rabbits received TWP therapy after IOL implantation. And in the third group, rabbit received prednisone therapy after IOL implantation. Aqueous humor samples were aspirated on the 1st, 3rd, 7th, and 14th postoperative day. A modified double antibodies indirect sandwich EL ISA was used to detect for the presence of TNF. The data were closely studied by means of analysis of variance with SAS ***; It was found that TNF level in aqueous humor reached its maximum on the 7th postoperative day in the group with IOL implantation. Background The purpose of otomicrosurgery in modern times is to achieve complete removal of disease, which is the basis for hearing rehabilitation. Artificial ossicles are important for hearing rehabilitation. One of the materials is hydroxyapatite that acts as ossicular replacement prosthesis (HA prosthesis, PORP & TORP) and is commonly used as substitute bone in clinics. Although HA prostheses feature good biocompatibility, stability and certain resistance to infection, the structure of the material can be decomposed and absorbed. Based upon animal experiments and clinical application, this article explores the infectivity of HA prostheses and influencing factors. Materials and Methods HA prostheses were implanted in the bulla in guinea pigs. Infected HA prostheses were examined under a scanning electron microscope. Ninety patients implanted with HA prostheses were followed for more than 3 months. Five cases that had undergone revision operations were reported. Results Scanning electronic microscopy showed that the normal compact structures had disappeared and become loose in infected HA prostheses, with their micropores infiltrated with effusion, fibrous tissue and inflammatory cells. As a result, the normal ceramic structure was decomposed and the micropores structure disappeared. The infected mesotympanum in the reported cases also led to HA infection. The shape and structure of bone were destroyed, which was visible by visual examination and light microscopy. The process of cholesteatoma recurrence was always associated with infection, although the HA prosthesis was sometimes spared. Conclusion While HA prosthesis is anti-infective to some extent, long-term infection and cholesteatoma recurrence can lead to destruction and decomposition of HA prostheses. Long-term infected middle ear cavity, dysfunctional Eustachian tube, residual and recurrent cholesteatoma and other factors can cause decomposition of HA prosthesis and result in hearing loss.

AIM: Primary neoplasmas of the jejunum and ileum are infrequent and lack specific manifestations and inaccessibility of conventional endoscopy, so the diagnosis of these tumors are usually delayed. So far the data of primary jejunoileal neoplasmas is still scarce, especially in Chinese medical literature in English. There may be some differences among the Chinese and the westerners in jejunoileal ***: A retrospective analysis was made on clinical findings and pathological ***: Of the 60 patients with jejunal or ileal neoplasmas, the most frequent symptom was abdominal pain (57%), followed by tarry stool (43%) and hematochezia (10%). Abdominal mass (40%) was the most common finding on physical examination, followed by anemia and weight loss (35%). 67% of the jejunoileal neoplasms were located in the jejunum. Among the malignant neoplasmas (68%), malignant stroma (47%) was most common, while the benign stromoma (20%) was the most common benign neoplasmas. Preoperatively, 40 patients (67%) were diagnosed as small bowel neoplasmas, of which 34 were found by enteroclysis. Abdominal mass was shown by CT in 18 cases and by ultrasonography in 13. The mean duration of symptoms before diagnosis was 7 months. In 41 patients with malignant tumors, the duration of symptoms before diagnosis exceeded 12 months in 21 cases, lymphatic or distant metastases were found in 26 (63%)cases during operation. An emergency laparotomy was performed in 4patients (7%) owing to intestinal obstruction or ***: Primary jejunoileal neoplasmas in Chinese present some difference from Westerners on clinical features and histopathologic types. Enteroclysis remains the major relevant diagnostic procedure in this study, the misdiagnotic rate is high preoperatively due to failure of detection by conventional imaging procedures such as CT and inaccessibility of routine endoscopy. Aim:To assess the anti-hepatitis B virus（HBV）effect of hyperoside extractedfrom Abelmoschus manihot（L）***:The human bepatoma HepG2.2.15 cell culture system and duck hepatitis B virus（DHBV）infection modelwere used as in vivo and in vitro models to evaluate the anti-HBV ***:In the cell model,the 50% toxic concentration of hyperoside was 0.115 g/L;themaximum nontoxic concentration was 0.05 g/L,On the maximum nontoxicconcentrations,the inhibition rates of hyperoside on HBeAg and HBsAg in-the2.2.15 cells were 86.41% and 82.27% on d 8,*** the DHBV infectionmodel,the DHBV-DNA levels decreased significantly in the treatment of ***-1·d-1and 0.10 g·kg-1dosage groups of hyperoside（P<0.01）.The inhibitionof the peak of viremia was at the maximum at the dose of 0.10 g·kg-1·d-1andreached 60.79% on d 10 and 69.78% on d 13,***:Theseresults suggested that hyperoside is a strong inhibitor of HBsAg and HBeAgsecretion in 2.2.15 cells and DHBV-DNA levels in the HBV-infected duck model.

Purpose: The tumour necrosis factor (TNF) level in aqueous humor after transscleral fixation of intraocular lens (IOL) implantation in rabbits and discuss the effect of TNF on postoperative anterior ocular ***: Twenty - seven pigmented rabbits were divided into three groups. Group 1: transscleral fixation of posterior chamber (PC) IOL implantation;Group 2; Lens of rabbits was removed without IOL implantation; Group 3; the comtrol group, without surgical intervention. On the 1st, 3rd, 7th and 14th postoperative days,aqueous humor samples were obtained. An modified double antibodies indirect sandwich ELISA was used to detected for the presence of TNF. The data were analyzed by using analysis of variance of SAS software. Results: It was found that TNF level in aqueous humor was increased after transscleral fixation of IOL implantation. To explore the biological roles of human Pescadillo and investigate its potential effect on tumorigene- sis, the cDNA of Pescadillo was fused with that of GST. After purification and elution, the purified GST-Pescadillo fusion protein was obtained, and the antibody against the fusion protein was generated. Endogenous Pescadillo protein was observed to be remarkably induced by estrogen. It was mainly distributed in the tissues such as breast, ovary and intestine, all of which contain proliferating cells, and was also detected in many cell lines of human cancer: renal carcinoma, hepatoma, ovarian cancer, colon carcinoma, and breast cancer. The expression level of Pescadillo was increased significantly in breast cancer tissues compared with their paired margin tissues. Taken together, these data suggest that Pescadillo may play important roles in the initiation and development of cancer and may be a po- tential target in cancer diagnosis and therapy.
