Recent Aims: new diagnostic methods are needed to detect peripheral arterial disease easier than using the ankle-brachial index measured by Doppler devices. We investigated whether the use of pocket pulse oximeters could meet sensitivity and specificity criteria as screening method to detect significant peripheral arterial perfusion deficits. Methods: We measured oxygen saturation (SaO2) at index fingers and great toes (on horizontal and elevated 30°) by a pocket pulse oximeter in 250 subjects with diabetes mellitus attending the outpatient clinic. A finger-to-toe SaO2 gradient greater than 2% was considered abnormal. Ankle-brachial index was measured by a hand held Doppler device. Peripheral arterial disease was defined as an ankle-brachial index less than 0.9. Results: A total of 1392 (93%) valid SaO2 readings were obtained. Twenty-seven (11%) patients were excluded due to not having measurable SaO2 finger-to-toe gradients. A total of 223 patients were analyzed. Peripheral arterial disease was detected in 47 (21%) patients. A finger-to-toe SaO2 gradient greater than 2% had sensitivity 42.6% (95% CI 30.0% - 55.3%), specificity 79.1% (95% CI 75.7% - 82.6%), positive predictive value 35.7% (95% CI 25.2% - 46.4%), negative predictive value 83.4% (95% CI 79.8 - 87.1), positive likelihood ratio 2.03 (95% CI 1.23 - 3.17) and negative likelihood ratio 0.73 (95% CI 0.54 - 0.93) to detect peripheral arterial disease. The area under the receiving operating characteristic curve was 0.69 (95% CI 0.62 - 0.77). Conclusion: Pocket pulse oximeters showed insufficient sensitivity as screening method for detecting peripheral arterial disease in patients with diabetes mellitus.

Caue铁矿位于Sao Francisco地盾南部Itabira地区。在该区已确定出其变质程度达到角闪岩相三个地质单元:①太古代火山沉积层序;②元古代含铁建造单元;③元古代石英岩单元。含铁建造单元被角闪岩脉切割,在构造上被火山沉积层序内的滑石片岩和角闪岩叠覆。所有岩石均受到三期褶皱作用及相应的逆冲作用和石香肠化的影响。在含铁建造中渐进的简剪切由东向西持续进行从而形成D_1和D_2构造,D_3构造为具褶劈的开阔褶皱。 Caue含钯金矿床由5个矿体组成,其围岩为含铁建造单元内的多色赤铁岩。多色赤铁岩为强烈剪切、变质并已发生热液蚀变的苏必利尔湖型含铁建造,其组成为赤铁矿、石英、滑石、金云母,含少量的电气石、磷灰石和独居石。在Corpo Y矿体中,最高的金聚集出现在石英脉中,而在Corpo Y矿体、Central矿体、Aba Leste矿体和Abanorte矿体中最高的金品位位于赤铁矿中局部石英聚集处。除Aba norte矿体外,其余矿体中的矿体均平行于S_1面理。Aba nortz矿体内,富金的香肠化赤铁矿脉平行于S_2。矿石矿物为:钯金、金、钯的氧化物和硒钯矿。主要成矿事件与热变质作用的峰值(T=600℃)一致,与D_1剪切作用和逆冲作用同期。与该事件有关的热液蚀变以形成滑石、金云母、电气石、赤铁矿和石英脉为特征,构成多色赤铁岩岩石类型。多色赤铁岩风化使硅酸盐蚀变成高岭土,氧化物蚀变为针铁矿。在高温和高氧逸度环境下,Pb和Au可能已经转换为氧化物络合物,它们的沉淀主要与pH值的增加相对应。这种现象可以作为矿化流体与多色赤铁岩再作用的结果出现,也可能代表蚀变的白云岩化氧化物相含铁建造(白云岩化铁英岩)。这些贵金属最可能的来源是太古代火山沉积岩层。根据Caue矿床的特征,有理由认为这是一种新的金矿类型,本文将其定义为“以强烈剪切和变质的苏必利尔湖型含铁建造为围岩的含钯金矿床”。沿Sao Francisco地盾南部的东部边界,其它的含钯金矿床也是以多色赤铁岩为围岩的,由此认为这种地质环境特别宜于该类型的矿床的产生。

To clarify the species status of sturgeon from rivers of the Iberian Peninsula, eight molecular markers (4 nuclear and 4 mitochondrial) have been analysed in different specimens from historical museum samples and prehistoric samples from archaeological sites. These analyses indicate that one of these specimens (UGP captured in the Guadalquivir River in the 19th century) is A. sturio, based on all the eight molecular markers, four of them used from the first time in this study. In previous analyses based on 5 genetic markers, our group assigned two specimens captured in this river in the 1970-80s (EBD8173 and EBD8401) to the species A. naccarii, suggesting the presence of this species in the Iberian Peninsula. In this work, this conclusion is drawn after successfully obtaining a mitochondrial marker in a very old scute from a prehistoric site (Acinipo, about 1500 BC, from the Guadalquivir River basin). On the other hand, in the specimen EBD8174 captured in the Guadalquivir in 1975, we have obtained two new mitochondrial markers confirming that it can be considered A. sturio for all the mitochondrial markers, but nuclear ones identify it as A. naccarii. Finally, two very old samples (nerja E-VI and nerja n/62-63) were not successfully characterized by any molecular markers. Some aspects and consequences of our results are discussed, such as the origin of the “mosaic” specimen EBD8174 and, above all, the native status of A. naccarii in historic and prehistoric times in the southern Iberian Peninsula.

Acute gastrointestinal bleeding(AGIB) is a prevalent condition with significant influence on healthcare costs. Endoscopy is essential for the management of AGIB with a pivotal role in diagnosis, risk stratification and management. Recently, hemostatic powders have been added to our endoscopic armamentarium to treat gastrointestinal(GI) bleeding. These substances are intended to control active bleeding by delivering a powdered product over the bleeding site that forms a solid matrix with a tamponade function. Local activation of platelet aggregation and coagulation cascade may be also boosted. There are currently three powders commercially available: hemostatic agent TC-325(Hemospray), EndoClotTM polysaccharide hemostatic system, and Ankaferd Bloodstopper. Although the available evidence is based on short series of cases and there is no randomized controlled trial yet, these powders seem to be effective in controlling GI bleeding from a variety of origins with a very favorable side effects profile. They can be used either as a primary therapy or a secondline treatment, and they seem to be especially indi-cated in cases of cancer-related bleeding and lesions with difficult access. In this review, we will comment on the mechanism of action, efficacy, safety and technical challenges of the use of powders in several clinical scenarios and we will try to define the main current indications of use and propose new lines of research in this area.

AIM: To evaluate the surface expression of triggering receptor on myeloid cell 1 (TREM-1), class Ⅱ major histocompatibility complex molecules (HLA-DR), andthe expression of the splicing variant (svTREM-1) ofTREM-1 in septic patients and those subjected to major abdominal ***: Using flow cytometry, we examined the surface expression of TREM-1 and HLA-DR in peripheral blood monocytes from 11 septic patients, 7 elective gastrointestinal surgical patients, and 10 healthy volunteers. svTREM-1 levels were analyzed by RT-PCR. RESULTS: Basal expression of TREM-1 and HLA-DR in healthy volunteers was 35.91±14.75 MFI and75.8±18.3%, respectively. In septic patients, TREM-1 expression was 59.9±23.9 MFI and HLA-DR expression was 44.39±20.25%, with a significant differencebetween healthy and septic groups (P＜0.05) for bothmolecules. In the surgical patients, TREM-1 and HLA-DR expressions were 56.8±20.85 MFI and 71±13.8% before surgery and 72.65±29.92 MlFI and 72.82±22.55% after surgery. TREM-1 expression was significantly different(P = 0.0087) between the samples before and aftersurgery and svTREM-1 expression was 0.8590±0.1451 MF1, 0.8820±0.1460 MF1, and 2.210±0.7873MF1 in the healthy, surgical (after surgery) and septic groups, respectively. There was a significant difference (P = 0.048) in svTREM-1 expression between the healthy and surgical groups and the septic ***: TREM-1 expression is increased during systemic inflammatory conditions such as sepsis and the postoperative phase. Simultaneous low expression of HLA-DR molecules correlates with the severity of illness and increases susceptibility to infection. Additionally, TREM-1 expression is distinctly different in surgical patients at different stages of the inflammatory response before and after surgery. Thus, surface TREM-1 appears to be an endogenous signal during the course of the inflammatory response. svTREM-1 expression is significantly increased during sepsis, appearing to be an indicator of

A forest plantation, product of the reforestation of pine trees, represented a pool of biological resources for the implementation of a solid state fermentation process. The trees were identified as Pinus pseudostrobus Lindl from which lignocellulosic material in the form of pine needles was collected. Soil fungi, responsible for plant litter decomposition, were cultured at laboratory conditions and tested for their ability to grow on cellulose and hemicellulose as the sole carbon sources. A fungal strain, belonging to the genus Penicillium, was selected for growing it on pine needles as the substrate in a solid state culture. After following the culture for six days, the newly isolated strain exhibited a much higher capacity for spore production and holocellulose degradation, compared to a purchased strain of Penicillium chrysogenum and two control conditions. This work marks the beginning of future studies focused on commercial applications and represents the first report of a biotechnological process based on pine needles and their degradation by an ascomycetes species belonging to the genus Penicillium.

A 17-year-old nulligravida woman presented to the emergency department complaining of a six months’ history of severe dysmenorrhea. The patient underwent a laparoscopic excision of a right ovarian endometrioma in another hospital one year ago, 24 months after menarche at 14 years of age. Transvaginal ultrasonography, magnetic resonance imaging, hysteroscopy as well as laparo- scopy revealed a unicornuate uterus with a noncommunicating right rudimentary horn, a dilated right fallopian tube and a novel 40 mm right ovarian endometrioma. An excision of the right fallopian tube, the rudimentary horn and the endometrioma was performed. Since endometriosis may be originated by retrograde menstruation, an obstructive müllerian malformation should be ruled out at the diagnosis of endometriosis, particularly in adolescents. It may lead clinicians to provide an initial definitive treatment for avoidingrapid recurrent endometriosis.
