Clinical Implications and Management of Sub Clinical Hyperthyroidism: A Review

Sub clinical hyperthyroidism (SCH) is characterized by normal free thyroid hormone concentrations along with a low or undetectable serum TSH (thyrotropin) level. The increased use of TSH as a screening measure and improved assay sensitivity is contributing to the diagnosis of sub clinical hyperthyroidism more frequently than ever in our clinical practise leading to the increased prevalence of the disease. The significance of SCH remains uncertain for most patients as some will revert to normal thyroid status over time whereas others will either remain static or progress to overt thyroid disease in the future. The detrimental effects of a persistently suppressed TSH has now been extensively studied and its effect on the cardiovascular system, the skeleton, mood disturbance, quality of life is quite significant leading to considerable morbidity and mortality. Majority of the patients are asymptomatic and lack overt features but the relevance of treatment is more focussed in elderly patients where the risk of developing cardiac arrhythmia and loss of bone mineral density is much more than young people in whom a conservative approach is usually preferred. The issue is contentious, the situation is challenging and the benefits of treatment are debatable. The consensus for who, when and how to treat is growing but still hasn’t been universally accepted. We attempt to review the recent literature available for sub clinical hyperthyroidism and suggest an analytical approach to its investigations and management.

A Review of Endocrine Disorders in Thalassaemia

Endocrine dysfunction in thalassaemia is amongst the most common complication and is principally attributed to excessive iron overload and suboptimal chelation. The prevalence is quite high particularly in multiethnic populations but determining the prevalence is often difficult due to the widespread heterogeneity of the population and timing of exposure to chelation therapy. Disturbances in growth, pubertal development, abnormal gonadal functions, impaired thyroid, parathyroid and adrenal functions, diabetes and disorderly bone growth are commonly encountered. Early detection and institution of appropriate transfusion regimen and chelation therapy and treatment of complications are the keys to managing this population including regular follow. In this article, we review the literature in relation to the various endocrine complications encountered in thalassaemia.

Ecology and management of the black inch worm, Hyposidra talaca Walker (Geometridae： Lepidoptera) infesting Camellia sinensis (Theaceae)： A review

The black inch worm, Hyposidra talaca （Walker）（Geometridae： Lepidoptera） has earned considerable importance as a defoliator during last decade in north east India. Characteristic features of H. talaca, Such as switching from wild plants to Camellia sinensis, and completion of 6-8 generations in a year without winter diapause appear to be the major reasons for persistence of the defoliator on C. sinensis. There are reports on the aggressive infestation of the pest on C. sinensis than on alternate host plants and development of high tolerance^resistance to certain insecticides. Till date, more than one hundred alternate host plants have been recorded for H. talaca, but with very few natural enemies. To manage this pest chemically, synthetic pesticides are widely used. Integrated management is recently being encouraged wherein cultural, mechanical, physical, biological and chemical methods have been contemplated. This review collates the work and literature on the ecology of H. talaca and offers an analysis of the problems encountered in managing the defoliator in tea plantations. Further, future directions for better management of H. talaca are suggested.

Tectonic juxtaposition of crust and continental growth during orogenesis: Example from the Rengali Province, eastern India

The southern houndary of the Singhbhum Craton witnessed multiple orogenies that juxtaposed thin slice of granulite suite of the Rengali Province against the low-grade granite-greenstone belt of the craton along the E-W trending Sukinda Thrust. The strong southerly dipping mylonitic foliation within the granulites along with the pronlinent down-dip mineral lineation, suggest a northerly-verging thrusting. Mylonitized charnockite at the contact zone contains enclaves of mafic and ultranlafic granulite, whereas granitoid gneiss contains enclaves of pelitic granulite. Mafic granulite enclaves preserve an early （SiM） foliation that formed during DiM deformation. This rock, along with the host charnockite, were intensely deformed by the D2M thrusting event and resulting S2M foliation development in both rock suites. Geothermobaronietric and pseudosection analyses show that the garnet-clinopyroxene-plagioclase- orthopyroxene-ihllenite-quartz assemblage in mafic granulite was stabilized at high-pressure and temperature conditions （10 12 kbar, 860 C） and was overprinted by a fine-grained assemblage of cli- nopyroxene-plagioclase J hornblende that developed during deconlpression （down to 5.5-7.5 I〈bar）. Matrix hornblende shows incipient breakdown to garnet-clinopyroxene-quartz intergrowth due to a granulite facies reworking. A contrasting P-T history is preserved in the pelitic granulite. The peak assemblage garnet-orthopyroxene-cordierite-quartz-rutile was stabilized at -6.0 kbar, 730 C which resulted flom heating of the mid crust magma during the D2M thrusting. The contrasting P-T histories could result flonl the tectonic juxtaposition of lowerand mid-crustal section during the D2M event. Evidences of an early orogenic imprint within the mafic granulite imply involvement of deep continental crust during southward growth of the Singhbhum Craton.

The identification of jets originating from quarks and gluons,often referred to as quark/gluon tagging,plays an important role in various analyses performed at the Large Hadron Collider,as Standard Model measurements and searches for new particles decaying to quarks often rely on suppressing a large gluon-induced *** paper describes the measurement of the efficiencies of quark/gluon taggers developed within the ATLAS Collaboration,using √s=13 TeV proton–proton collision data with an integrated luminosity of 140 fb^(-1) collected by the ATLAS *** taggers with high performances in rejecting jets from gluon over jets from quarks are studied:one tagger is based on requirements on the number of inner-detector tracks associated with the jet,and the other combines several jet substructure observables using a boosted decision tree.A method is established to determine the quark/gluon fraction in data,by using quark/gluon-enriched subsamples defined by the jet *** in tagging efficiency between data and simulation are provided for jets with transverse momentum between 500 GeV and 2 TeV and for multiple tagger working points.
