Anti-hepatotoxic and antioxidant influence of Ipomoea carnea against anti-tubercular drugs induced acute hepatopathy in experimental rodents

Objective:To assess the hepatoprotective effct of Ipomoea carnea(***)extract against antitubercular drug-induced liver toxicity in experimental ***:*** extracts(125,250 and 500 mg/kg,*** weight)were administered daily for 35 d in experimental *** toxicity was induced by combination of three antitubercular drugs(isoniazid 7.5 mg/kg,rifampicin 10 mg/kg and pyrazinamide 35 mg/kg)given orally as suspension for 35 d in *** groups received *** extracts along with antitubercular *** hepatoprotective activity was assessed using various biochemical parameters like aspartate aminotransferase,alanine aminotransferase,alkaline phosphatise and total ***,in-vivo antioxidant activities as lipid peroxidation,reduced glutathione,superoxide dismutase and catalase were measured in rat liver homogenate along with ATPase and *** biochemical observations were supplemented by histopathological ***:Obtained results demonstrated that treatment with *** extracts significantly(P<0.05-P<0.001)and dose-dependently prevented drug induced increase in serum levels of hepatic ***,*** extracts significantly(up to P<0.001)reduced the lipid peroxidation in the liver tissue and restored activities of defence antioxidant enzymes,reduced glutathione,superoxide dismutase and catalase towards normal *** of the liver tissue showed that *** extracts attenuated the hepatocellular necrosis,massive fatty changes and led to reduction in inflammatory cells ***:The results of this study strongly indicate the protective effect of *** extracts against liver injury,which may be attributed to its hepatoprotective activity,and there by scientifically support its traditional use.

Surgical treatment of proximal humerus fractures using PHILOS plate

Objective：To evaluate functional outcome and complications of open reduction and internal fixation with proximal humeral internal locking system （PHILOS） plate for proximal humerus ***：We reviewed 51 patients who underwent open reduction and internal fixation with PHILOS plate between the years 2007 to *** were 35 men and 16 women with a mean age of 38 years （range 24-68）.There were 41 patients in the age group of ＜60 years and 10 patients in the age group of ＞60 *** to Neer classification system,8,15 and 23 patients had 2-part,3-part,and 4-part fractures,respectively and 5 patients had 4-part fracture *** surgeries were carried out at our tertiary care trauma *** evaluation of the shoulder at final follow-up was done using Constant-Murley ***：The mean follow-up period was 30 months （range 12-44 months）.Two patients were lost to *** the remaining 49 patients,all fractures were united clinically and *** mean time for radiological union was 12 weeks （range 8-20 weeks）.At the final follow-up the mean Constant-Murley score was 79 （range 50-100）.The results were excellent in 25 patients,good in 13 patients,fair in 6 patients and poor in 5 *** the follow-up,four cases of varus malunion,one case of subacromial impingement,one case of deep infection,one case of intraarticular screw penetration and one case of failure of fixation were *** cases of avascular necrosis,hardware failure,locking screw loosening or nonunion were ***：PHILOS provides stable fixation in proximal humerus *** prevent potential complications like avascular necrosis,meticulous surgical dissection to preserve vascularity of humeral head is necessary.

Chronic kidney disease(CKD)and hypertension(HTN)are closely associated with an overlapping and intermingled cause and effect *** in renal functions are usually associated with a rise in blood pressure(BP),and prolonged elevations in BP hasten the progression of kidney function *** of HTN by normalizing the BP in an individual,thereby slowing the progression of kidney disease and reducing the risk of cardiovascular disease,can be effectively achieved by the anti-hypertensive use of calcium channel blockers(CCBs).Use of dihydropyridine CCBs such as amlodipine(ALM)in patients with CKD is an attractive option not only for controlling BP but also for safely improving patient *** clinical experiences with its use as monotherapy and/or in combination with other anti-hypertensives in varied conditions have demonstrated its superior qualities in effectively managing HTN in patients with CKD with minimal adverse *** comparison to other counterparts,ALM displays robust reduction in risk of cardiovascular endpoints,particularly stroke,and in patients with renal *** with its longer half-life displays effective BP control over 24-h,thereby reducing the progression of endstage-renal *** conclusion,compared to other classes of CCBs,ALM is an attractive choice for effectively managing HTN in CKD patients and improving the overall quality of life.

Unusual presentation of glomus tympanicum tumour: New bone formation in the middle ear

The objective of this study is to increase awareness of the rare presentation, diagnostic difficulties and management of glomus tympanicum of the middle ear. A 49 years old male, with a background of hypertension and epilepsy, presented with a two month history of left sided conductive hearing loss, pulsatile tinnitus and headache. Clinically and radiologically a diagnosis of glomus tympanicum was made. Intraoperatively, extensive osteogenesis of the middle ear resulting in ossicular fixation and erosion was found. This patient required a two stage operation for full clearance of disease. A stapedectomy drill was used to drill off the bony overgrowth surrounding the ossicles resulting in improved hearing thresholds and full clearance of the disease at two year follow up. Glomus tympanicum can result in new bone formation in the middle ear with resultant ossicular fixation and conductive hearing loss. This can be effectively treated surgically with restoration of hearing.

Understanding the molecular mechanisms of cancer prevention by dietary phytochemicals:From experimental models to clinical trials

Chemoprevention is one of the cancer prevention approaches wherein natural/synthetic agent(s) are prescribed with the aim to delay or disrupt multiple pathways and processes involved at multiple steps, i.e., initiation, promotion, and progression of cancer. Amongst environmental chemopreventive compounds, diet/beverage-derived components are under evaluation, because of their long history of exposure to humans, high tolerability, low toxicity, and reported biological activities. This compilation briefly covers and compares the available evidence on chemopreventive efficacy and probable mechanism of chemoprevention by selected dietary phytochemicals(capsaicin, curcumin, diallyl sulphide, genistein, green/black tea polyphenols, indoles, lycopene, phenethyl isocyanate, resveratrol, retinoids and tocopherols) in experimental systems and clinical trials. All the dietary phytochemicals covered in this review have demonstrated chemopreventive efficacy against spontaneous or carcinogen-induced experimental tumors and/or associated biomarkers and processes in rodents at several organ sites. The observed anti-initiating, anti-promoting and anti-progression activity of dietary phytochemicals in carcinogen-induced experimental models involve phytochemical-mediated redox changes, modulation of enzymes and signaling kinases resulting to effects on multiple genes and cell signaling pathways. Results from clinical trials using these compounds have not shown them to be chemopreventive. This may be due to our:(1) inability to reproduce the exposure conditions, i.e., levels, complexity, other host and lifestyle factors; and(2) lack of understanding about the mechanisms of action and agent-mediated toxicity in several organs and physiological processes in the host. Current research efforts in addressing the issues of exposure conditions, bioavailability, toxicity and the mode of action of dietary phytochemicals may help address the reason for observed mismatch that may ultimately lead

Minimally invasive osteosynthesis of distal tibial fractures using anterolateral locking plate： Evaluation of results and complications

Purpose： Soft tissue healing is of paramount importance in distal tibial fractures fora successful outcome. There is an increasing trend of using anterolateral plate due to an adequate soft tissue cover on ante- rolateral distal tibia. The aim of this study was to evaluate the results and complications of minimally invasive anterolateral locking plate in distal tibial fractures. Methods： This is a retrospective study of 42 patients with distal tibial fractures treated with minimally invasive anterolateral tibial plating. This study evaluates the hone and soft tissue healing along with emphasis on complications related to bone and soft tissue healing. Results： Full weight bearing was allowed in mean time period of 4.95 months （3-12 months）. A major local complication of a wound which required revision surgery was seen in one case. Minor complica- tions were identified in 9 cases which comprised 4 cases of marginal necrosis of the surgical wound, 1 case of superficial infection, I case of sensory disturbance over the anterolateral foot, 1 case of muscle hernia and 2 cases of delayed union. Mean distance between the posterolateral and anterolateral incision was 5.7 cm （4.5-8 cm）. Conclusion： The minimally invasive distal tibial fixation with anterolateral plating is a safe method of stabilization. Distance between anterolateral and posterolateral incision can be placed less than 7 cm apart deoending on fracture oattern with proper surgical timing and technique.

Immuno-contraceptive potential of sperm specific LDHC<sub>4</sub>and SPAM-1 (PH-20) sub units in dog

PH-20 (Spam-1) antigen appears to be a bi-functional sperm plasma membrane protein with hyaluronidase activity and its role is related to cumulus penetration as well as zona binding. (LDH-C4) is a key enzyme, distributed specifically in testis and is highly immunogenic, function of which relates to energy metabolism and capacitation of the sperm. Therefore, in this study, we observed the effect of purified native PH-20 and LDH-C4 antigens on sperm function and morphology in dog. Native PH-20 (46, 32 kDa) and LDH-C4 (36, 30, 28) sub units, purified from dog sperm SDS- extracts were used to test as immunocontraceptive candidates in stray dog. Dogs 1, 2, 3, 4, 5/6 and 7/8 were immunized with 46, 32, 36, 30/28, 46/32, 36/30/28 kDa sub units respectively by i/m route and semen was analysed at weekly intervals. High titre of 3200 - 6400 was achieved in all dogs after three boosters. Immune response in treated dogs was confirmed by increase in Ig level in blood serum and reaction of partially purified serum Ig of immunized dogs with respective antigens in double immune diffusion and immunoblots. Immunization of dogs either with individual PH-20/ LDHC sub units or pooled PH-20/LDHC4 sub units showed a significant effect on sperm count, percent motility, viability, HOST and sperm morphology. All the sperm parameters declined to a significant level (p < 0.05) between 45 - 115 DPI and remained low than that of the recommended values for fertile dog semen till 175 DPI. The incubation of spermatozoa in HOS solution in the presence of anti-PH-20 and anti-LDHC Ig significantly reduced the percent HOS positive spermatozoa. A significant decline (p ≤ 0.05) in acrylamide penetration assay was also observed in the presence of anti 46 and 30/28 kDa Ig. Since percent motility, sperm count, HOST and percent normal spermatozoa in immunized dogs were significantly less than that of recommended fertile dog semen, it can be concluded that PH-20 (46/32 kDa) and LDHC4 (36/30/28 kDa) sub units
