“Medioglycaemia”: A new concept in glycaemic control in intensive care (ICU) units?

Introduction: Critically ill patients can experience stress-induced hyperglycaemia. Glycaemic control therapy (GCT) is administered to control patients’ blood glycaemic levels and reduce the incidence of infection, myocardial infarctions and organ failure. However, there are many factors influencing the effectiveness of glycaemic control for patients. This investigation aimed to review the method of Glycaemic Control Therapy (GCT) used in two hospital settings, to assess the effectiveness of glycaemic control on patients’ blood glycaemic levels and examine any barriers that may be in place. Method: A retnrospective audit was carried out on patients’ case notes in Intensive Care Units (ICU) within the East Midlands, UK. This method prevents the study outcomes being swayed because GCT has already taken place. To reduce selection bias the most recent available case notes were selected. All the patients who were admitted to these adult ICU’s between March and April 2010 had their case notes examined, those who were administered GCT were included in the study, this involved 79 from Hospital A and 50 from Hospital b. The patients’ notes were retrospectively audited. Results: Different glycaemic control protocols were being implemented in each hospital, despite both belonging to the same ICU network. In most incidences, regardless of age, diabetes status or diagnosis, patients were administered the same sliding scale insulin (SSI). It was also found that GCT commenced for 41.9% (n = 52) of ICU patients (across both Hospitals) when glycaemic levels were below the established threshold of 10mmol/L. Additionally, a new glycaemic range has been discovered, where 88.3% (n = 113) of patients (across both Hospitals) receiving GCT were not controlled in hypoglycaemia, normoglycaemia or hyperglycaemia. To assess the prevalence of a panel of serologic markers that reflect gut barrier dysfunction in a mixed cohort of pediatric and adult primary sclerosing cholangitis (PSC) patients. METHODSSera of 67 PSC patients [median age (range): 32 (5-79) years, concomitant IbD: 67% and cirrhosis: 20%] were assayed for the presence of antibodies against to F-actin (AAA IgA/IgG) and gliadin (AGA IgA/IgG)] and for serum level of intestinal fatty acid-binding protein (I-FAbP) by ELISA. Markers of lipopolysaccharide (LPS) exposure [LPS binding protein (LbP)] and various anti-microbial antibodies [anti-OMP Plus IgA and endotoxin core IgA antibody (EndoCAb)] were also determined. Poor disease outcome was defined as orthotopic liver transplantation and/or liver-related death during the follow-up [median: 99 (14-106) mo]. One hundred and fifty-three healthy subjects (HCONT) and 172 ulcerative colitis (UC) patients were the controls. RESULTSA total of 28.4%, 28.0%, 9% and 20.9% of PSC patients were positive for AAA IgA, AAA IgG, AGA IgA and AGA IgG, respectively. Frequencies of AAA IgA and AAA IgG (P both) and AGA IgG (P = 0.01, for both) but not AGA IgA were significantly higher compared to both of the HCONT and the UC groups. In survival analysis, AAA IgA-positivity was revealed as an independent predictor of poor disease outcome after adjusting either for the presence of cirrhosis [HR = 5.15 (1.27-20.86), P = 0.022 or for the Mayo risk score (HR = 4.24 (0.99-18.21), P = 0.052]. AAA IgA-positivity was significantly associated with higher frequency of anti-microbial antibodies (P b IgA and P = 0.012 for anti-OMP Plus IgA) and higher level of the enterocyte damage marker (median I-FAbPb>AAA IgA posb>b>vsb>b>negb>: 365 vs 166 pg/mL, P = 0.011), but not with serum LbP level. 从尚未涉及的昆虫种类中建立新的细胞系能为基础研究和生物技术应用提供重要资源。本实验通过细胞培养技术,建立了3株来源于鳞翅目昆虫烟草天蛾Manduca sexta卵组织的新细胞系,分别命名为Qb-Ms1-8,Qb-Ms2-2和Qb-Ms2-7。这3株细胞已经培养在TNM-FH培养基中,28℃条件下传代培养了约50代,大部分细胞呈梭形,细胞群体倍增时间分别为51,31和49h。虽然这3株细胞系对苜蓿银纹夜蛾核型多角体病毒(Autographa californica multiple nuclear polyhedrosis virus,AcMNPV)不够敏感,侵染后96h感染率在33%～40%之间,但是Qb-Ms2-2细胞与bTI-Tn5b1-4细胞比较,分泌型碱性磷酸酶(SEAP)活性表达更高。本研究从建立的3株烟草天蛾新细胞系中筛选出SEAP高表达的细胞系Qb-Ms2-2,为进一步细胞克隆和筛选提供了新资源。

Introduction. Pathogenesis of sarcoidosis remains partially unknown. Cutaneous lesions are frequent (20 to 35% of cases). Their clinical features and follow- up data are highly variable. Numerous treatments have been proposed. The clinical features and followup data of four patients with chronic cutaneous sarcoidosis treated with methotrexate are reported. Case report. Mean age of patients (3 female, 1 male) was 40 years old (34- 49 years). One patient presented with a lupus pernio, two patients with papules and nodules, and the last with an annular lesion of the face. All patients had been previously treated with topical corticosteroids and/or hydroxychloroquine without any success. Patients were treated with methotrexate at doses ranging from 12.5 mg to 30 mg per week for at least 6 months. Complete remission of cutaneous lesions was observed in 3 of 4 patients after a mean treatment duration of 29 months (16 to 36). Methotrexate side effects were observed in one patient (elevated liver enzymes) leading to methotrexate discontinuation. Discussion. Methotrexate seems to be an effective treatment of cutaneous sarcoidosis. It should be used namely in patients who failed to respond to previous treatments with topical corticosteroids or antimalarial drugs.

使用PCR方法,在肉仔鸡胸肌和腿肌中,未检出来源于抗草甘膦的转基因豆粕外源cp4 epsps基因;在盲肠食糜和粪便中却检出外源基因,但出现率很低。这表明,在现有的检测条件下,外源DNA的完整性在肉鸡消化道很大程度被破坏,没有进入鸡肉中。
