The role of serial physical examinations in the management of angioedema involving the head and neck:A prospective observational study

Objective:To elucidate the progression of angioedema of the head and neck with routine management and to assess the utility of serial physical exams and fiberoptic laryngos-copy in its management. methods:This study was a prospective observational research. From 2013 to 2014, a prospec-tive observational study was conducted at a tertiary referral center. Forty patient were ap-proached, 7 refused, 33 (18e90 years old) were enrolled. Patients presented with angioedema involving the head and neck over a 12 month period were asked to participate in the study. Physical examination and fiberoptic laryngoscopy were performed at presentation and then repeated at least 1 h later. Results:Thirty-three patients with head and neck angioedema from any cause were enrolled (mean age 58, range 23e89 years). The upper lip was the most commonly involved site (58%). On reevaluation, 82% of patients reported subjective improvement in symptoms. Theassociation between subjective improvement and the physical exam, including fiberoptic laryngoscopy findings, was statistically significant (P Conclusion:In stable patients with angioedema of any head and neck subsite, self-reported symptoms are associated with clinical stability or improvement as assessed by physical signs and fiberoptic laryngoscopy. Patients’ symptoms may be an appropriate surrogate to monitor clinical status without the need for routine serial physical examinations or fiberoptic laryngos-copy, though further study is needed. Elevated mercury in Ambient Air and Soils Impacts of Historical Air Emissions (1897-1991) from a Chlor-Alkali Plant (CAP)

mercury contamination was found to be widespread in soils at a property in Upstate New York. Historical site use suggested that the mercury did not result from prior industrial use of the property. Soil contamination may have resulted from atmospheric deposition of mercury released from properties in close proximity to the contaminated property. The purpose of this forensics investigation was to examine to what extent atmospheric deposition of elemental mercury may have influenced mercury levels found in surficial soils on the contaminated property and further to identify the source(s) of the mercury. Work efforts included an examination of historical records available for a chlor-alkali plant (CAP) upwind of the contaminated property to establish historical use and disposal practices for elemental mercury. mercury emissions test data from the Upstate New York chlor-alkali facility were modeled (USEPA ISC3) as a means of estimating impacts on ambient air and soils vicinal to the facility. mercury emissions from the facility were modeled as both a point source and volume source. For example, at a location 305 meters to the east and 30 meters to the north of the modeled source centerline elemental mercury concentrations in ambient air were estimated at 270 ng/m3 (average results based upon 5 years of meteorological data). This value is contrasted to a background concentration of 1.6 ng/m3 (USEPA Report to Congress 1997). As a result of the modeling data in combination with findings related to mercury use and disposal practices at the NY CAP documented from the records review, it was concluded that emissions from the CAP facility during the period of operation (1897-1991) most likely accounted for the concentrations of elemental mercury found in surficial soils at properties situated downwind of the CAP. These findings were further corroborated by information available in the open literature for CAPs world-wide.

Boosting solar steam generation by structure enhanced energy management

Interfacial solar-steam generation is a promising and cost-effective technology for both desalination and wastewater *** process uses a photothermal evaporator to absorb sunlight and convert it into heat for water *** solar-steam generation can be somewhat inefficient due to energy losses via conduction,convection and ***,efficient energy management is crucial for optimizing the performance of solar-steam ***,via elaborate design of the configuration of photothermal materials,as well as warm and cold evaporation surfaces,performance in solar evaporation was significantly *** was achieved via a simultaneous reduction in energy loss with a net increase in energy gain from the environment,and recycling of the latent heat released from vapor condensation,diffusive reflectance,thermal radiation and convection from the evaporation ***,by using the new strategy,an evaporation rate of 2.94 kg m^-2 h^-1,with a corresponding energy efficiency of solar-steam generation beyond theoretical limit was achieved.

Using minimal photothermal material to achieve maximum evaporation rate is extremely important for practical applications of interfacial solar evaporation *** this work,we found that with the increase in the size of evaporation surfaces,the evaporation rate *** experimental and numerical simulation results confirmed that when the evaporation surface size increased,the middle portion of the evaporation surface acted as a‘‘dead evaporation zone”with little contribution to water *** on this,the middle portion of the evaporation surface was selectively removed,and counterintuitively,both the evaporation rate and vapor output were increased due to the reconfigured and enhanced convection above the entire evaporation *** such,this work developed an important strategy to achieve a higher evaporation rate and increased vapour output while using less material.

Arthritis has a worldwide increasing prevalence with increased demands on healthcare systems to provide arthroplasty surgery. There is growing evidence that early discharge and rehabilitation at home following hip and knee replacement provide similar or better results in function, complication rates and patient satisfaction. We suggest that this method of rehabilitation is a cost-effective viable option in fitter patients whose medical demands will be low postoperatively.

A new spallation mechanism of thermal barrier coatings on aero-engine turbine blades

Laboratory experiments were conducted to study the spallation behaviour of thermal barrier coatings(TBCs) on aero-engine turbine blades manufactured by the electron-beam physical vapour deposition technique(EB-PVD). Intact blades were heated at temperature 1135℃ in a furnace for certain time and then cooled to the room temperature in the laboratory condition. It was found that no spallation occurred during cooling, but spallation happened at constant room temperature after cooling. The spallation mechanism is studied by using the mechanical model developed(Harvey 2017 and Wang 2017), which are based on the hypothesis of pockets of energy concentration(PECs). Some observations of the spallation behaviour are well predicted by the model.

Efficacy and safety of tenofovir in chronic hepatitis B: Australian real world experience

AImTo evaluate the long-term treatment outcomes of tenofovir therapy in patients in a real world Australian tertiary care setting. mETHODSWe performed a retrospective analysis of treatment outcomes among treatment-naïve and treatment-experienced patients receiving a minimum 3 mo tenofovir therapy through St Vincent’s Hospital melbourne, Australia. We included patients receiving tenofovir [tenofovir disoproxil fumarate (TDF)] monotherapy, as well as patients treated with TDF in combination with a second antiviral agent. Patients were excluded if they demonstrated human immune-deficiency virus/hepatitis C virus/hepatitis delta virus coinfection or were less than 18 years of age. We considered virological and biochemical response, as well as safety outcomes. Virological response was determined by measurement of hepatitis B virus (HBV) DNA using sensitive assays; biochemical response was determined via serum liver function tests; histological response was determined from liver biopsy and fibroscan; safety analysis focused on glomerular renal function and bone mineral density. The primary efficacy endpoint was complete virological suppression over time, defined by HBV DNA RESULTSNinety-two patients were identified who fulfilled the enrolment criteria. median follow-up was 26 mo (range 3-114). mean age was 46 (24-78) years, 64 (70%) were male and 77 (84%) were of Asian origin. 55 (60%) patients were treatment-naïve and 62 patients (67%) were HBeAg-negative. Complete virological suppression was achieved by 45/65 (71%) patients at 12 mo, 37/46 (80%) at 24 mo and 25/28 (89%) at 36 mo. Partial virological response (HBV DNA 20-2000 IU/mL) was achieved by 89/92 (96.7%) of patients. multivariate analysis showed a significant relationship between virological suppression at end of follow-up and baseline HBV DNA level (OR = 0.897, 95%CI: 0.833-0.967, P = 0.0046) and HBeAg positive status (OR = 0.373, 95%CI: 0.183-0.762, P = 0.0069).
