Aim： To investigate the existence of TmPRSS2：ERG fusion gene in circulating tumor cells （CTC） from prostate cancer patients and its potential in monitoring tumor metastasis. methods- We analyzed the frequency of TmPRSS2： ERG and TmPRSS2：ETV1 transcripts in 27 prostate cancer biopsies from prostatectomies, and TmPRSS2：ERG transcripts in CTC isolated from 15 patients with advanced androgen independent disease using reverse transcription polymerase chain reaction （RT-PCR）. Fluorescence in situ hybridization （FISH） was applied to analyze the genomic truncation of ERG, which is the result of TmPRSS2：ERG fusion in 10 of the 15 CTC samples. Results： TmPRSS2： ERG transcripts were found in 44% of our samples, but we did not detect expression of TmPRSS2：ETV1. Using FISH analysis we detected chromosomal rearrangements affecting the ERG gene in 6 of 10 CTC samples, including 1 case with associated TmPRSS2：ERG fusion at the primary site. However, TmPRSS2：ERG transcripts were not detected in any of the 15 CTC samples, including the 10 cases analyzed by FISH. Conclusion： Although further study is required to address the association between TmPRSS2：ERG fusion and prostate cancer metastasis, detection of genomic truncation of the ERG gene by FISH analysis could be useful for monitoring the appearance of CTC and the potential for prostate cancer metastasis.

Fluid therapy is perhaps the most common intervention received by acutely ill hospitalized patients; however, a number of critical questions on the efficacy and safety of the type and dose remain. In this review, recent insights derived from randomized trials in terms of fluid type, dose and toxicity are discussed. We contend that the prescription of fluid therapy is context-specific and that any fluid can be harmful if administered inappropriately. When contrasting ‘‘crystalloid vs colloid'', differences in efficacy are modest but differences in safety are significant. Differences in chloride load and strong ion difference across solutions appear to be clinically important. Phases of fluid therapy in acutely ill patients are recognized, including acute resuscitation, maintaining homeostasis, and recovery phases. Quantitative toxicity(fluid overload) is associated with adverse outcomes and can be mitigated when fluid therapy basedon functional hemodynamic parameters that predict volume responsiveness and minimization of non-essential fluid. Qualitative toxicity(fluid type), in particular for iatrogenic acute kidney injury and metabolic acidosis, remain a concern for synthetic colloids and isotonic saline, respectively. Physiologically balanced crystalloids may be the ‘‘default'' fluid for acutely ill patients and the role for colloids, in particular hydroxyethyl starch, is increasingly unclear. We contend the prescription of fluid therapy is analogous to the prescription of any drug used in critically ill patients.

Background: Inhaled anesthetics, including halothane, iso- and sevoflurane induce proinflammatory cytokine release. Halothane is an inhaled anesthetic agent that is metabolized by the liver into a highly reactive product, trifluoroacetyl chloride, which can react endogenously to form a trifluoroacetyl-adduct (TFA-adduct). The mAA-adduct is formed by acetaldehyde and malondialdehyde reacting with endogenousproteins and is found in both patients and animals post-consumption of alcohol. These TFA and mAA-adducts have been shown to cause the release of proinflammatorycytokines by endogenous inflammatory cells. If both adducts share a similar mechanism of cell activation, receiving general anesthesia following alcohol ingestion could exacerbate the inflammatory response caused by the inhaled general anesthetic halothane and lead to solid organ (including liver and brain) injury. methods: Control diet and alcohol-fed rats were randomized to receive halothane pretreatments by intraperitoneal injection mixed in sesame oil. Following the intraperitoneal injections, the intact heart was removed, HECs were isolated and stimulated with unmodified bovine serum albumin (Alb), mAA-modified Alb (mAA-Alb), Hexyl-mAA, or lipopolysaccharide (LPS), and supernatant concentrations of TNF-α were determined. Results: Halothane pre-treated rat HECs demonstrated significantly greater TNF-α concentration following mAA-adduct and LPS stimulation than the non-halothane pre-treated in both pair and alcohol-fed rats, but was significantly greater in the alcohol-fed groups. Conclusion: These results demonstrate that halothane and mAA-adduct pre-treatment will increase the inflammatory response (TNF-α release) in rat HECs following LPS and mAA stimulation in vitro. Also, these results suggest that halothane exposure may increase the risk of alcohol-induced solid organ injury secondary to TNF-induced inflammation. The distribution of ultraviolet(UV)radiation field provides critical constraints on the physical environments of molecular *** 1 kpc of our solar system and fostering protostars of different masses,the giant molecular clouds in the Gould Belt present an excellent opportunity to resolve the UV field structure in star-forming *** performed spectral energy distribution(SED)fitting of the archival data from the Herschel Gould Belt Survey(HGBS).Dust radiative transfer analysis with the DUSTY code was applied to 23 regions in 14 molecular complexes of the Gould Belt,resulting in the spatial distribution of the radiation field in these *** 10 of 15 regions with independent measurements of star formation rate,their star formation rate and UV radiation intensity largely conform to a linear correlation found in previous studies.

Hyper-immunoglobulin m syndrome is an X-linked primary immunodeficiency disease caused by mutations in the CD40 ligand *** CD40 ligand has been recently highlighted as playing a key role in the pathogenesis of primary biliary *** the present study,we assessed an extensive set of serum autoantibodies in a series of well-defined patients with hyper-immunoglobulin m ***,liver-related and liver-not-related autoantibodies IgG,Igm and IgA were tested by ELISA and standard indirect immunofluorescence in HEp-2 cells in 13 Tunisian patients(8 males and 5 females,aged 1–12 years)with hyper-immunoglobulin m syndrome during 1995–2012 and,as controls,21 age-and gender-matched blood *** level of Igm antibody against mIT3 was significantly higher in patients than in controls(35.8 vs 10.7,P=0.002).Half of the hyperimmunoglobulin m syndrome patients were found to be anti-mIT3 Igm positive vs none of the controls(Po0.0001).Twenty-three percent of patients were found to be anti-sp100 antibody positive vs only 0.05%of *** immunofluorescence,92.3%of patients were mIT3 Igm positive vs none of the *** conclusion,the Igm class of anti-mIT3 antibodies was shown to be present by both ELISA and immunofluorescence in most of the patients with hyper-immunoglobulin m *** presence of the hallmark of primary biliary cholangitis,a disease where the CD40 ligand is a key player,in an immunodeficiency disease caused by mutations in the CD40 ligand gene is very intriguing and opens new scenarios in understanding the immune pathogenesis of primary biliary cholangitis.
