Cartilage oligomeric matrix protein: A novel non-invasive marker for assessing cirrhosis and risk of hepatocellular carcinoma

AIM: To assess serum cartilage oligomeric matrix protein(COMP) as a marker of cirrhosis and risk of progression to hepatocellular carcinoma(HCC). METHODS: A COMP enzyme-linked immunosorbentassay was used to test 187 patients with chronic liver diseases at the time point of first evaluation. The selected patients included 72 with chronic hepatitis B infection, 75 with chronic hepatitis C infection, 22 with primary biliary cirrhosis, 7 with autoimmune hepatitis type 1, and 11 with alcoholic liver disease. Demographic, biochemical, histological and clinical characteristics of the patients were recorded at the first evaluation. One hundred and forty-seven patients were followed for a median [interquartile range(IQR)] duration of 96.5(102) mo. The clinical, biochemical and histological data, as well as the development of cirrhosis, HCC according to internationally accepted criteria and in case of death, a liver-related cause during the follow-up period, were recorded at the electronic database of our clinic. COMP determination was also performed in 43 healthy individuals who served as the control study ***: COMP positivity(> 15 U/l) was detected in 22%-36% among chronic liver disease groups. Strikingly, almost 83% of COMP-positive patients were cirrhotic at baseline, independently of cause of liver disease. Among the patients who developed HCC during follow-up, 73.7%(14/19) were COMP positive at baseline. COMP positivity was significantly associated with older age(P lammatory activity(P = 0.001), higher aspartate aminotransferase(P lanine aminotransferase(P lutamyl transpeptidase(P = 0.003), alkaline phosphatase(P = 0.001), bilirubin(P l normalized ratio(P = 0.002) and alpha-fetoprotein levels(P lower albumin(P latelet count(P = 0.008). Metabolism of fibrates by cytochrome P450s and UDP-glycosyltransferases in rat and human liver microsomes

Fibrates are widely used for the treatment of ***,the contributions of the phase I and phase II metabolic pathways to the clearance of fibrates are *** this study,we investigated the metabolism of gemfibrozil(Gem) ,clofibric acid(CA) ,fenofibric acid(FA) and bezafibrate(Beza) by cytochrome P450s(P450s) and UDP-glycosyltransferases(UGTs) using a substrate depletion *** also compared the metabolic characteristics of rat liver microsomes(RlM) and human liver microsomes(HlM) .The intrinsic clearance rates mediated by P450s,UGTs and both were 172 ± 22,643 ± 26,798 ± 103 μl min-1 mg-1,respectively,for Gem and 43 ± 11,88 ± 12,119 ± 15 μl min-1 mg-1,respectively,for CA in *** fractions metabolized by P450s and UGTs in RlM were 22% and 81% for Gem,36% and 74% for *** P450-and UGT-mediated depletion rates for Gem were 303 and 1607 nmol min-1 mg-1 in RlM versus 86 and 243 nmol min-1 mg-1 in *** corre-sponding rates for CA were 1.1 and 1.7 nmol min-1 mg-1 in RlM versus 0.025 and 0.038 nmol min-1 mg-1 in ***,both P450s and UGTs substantially contribute to the clearance of Gem and CA,with UGTs playing a greater *** avoid un-der-estimating the impact of these pathways,it is necessary to measure NADPH-and UDPGA-dependent *** the fractions of these two pathways in RlM and HlM were similar,the depletion rate of Gem and CA in RlM was higher than that in *** metabolism of FA and Beza by P450s and UGTs was too low to calculate intrinsic clearance in both RlM and *** results indicate that fibrates are metabolized via similar pathways in rats and humans,and it is applicable to use RlM to predict the clearance of fibrates in human.

Case study approach to modeling historical disinfection by-product exposure in Iowa drinking waters

In the 1980 s, a case–control epidemiologic study was conducted in Iowa（USA） to analyze the association between exposure to disinfection by-products（DBPs） and bladder cancer risk. Trihalomethanes（THMs）, the most commonly measured and dominant class of DBPs in drinking water, served as a primary metric and surrogate for the full DBP *** THM exposure was calculated, based on rough estimates of past levels in Iowa. To reduce misclassification, a follow-up study was undertaken to improve estimates of past THM levels and to re-evaluate their association with cancer risk. In addition, the risk associated with haloacetic acids, another class of DBPs, was examined. In the original analysis, surface water treatment plants were assigned one of two possible THM levels depending on the point of chlorination. The re-assessment considered each utility treating surface or groundwater on a case-by-case basis. Multiple treatment/disinfection scenarios and water quality parameters were considered with actual DBP measurements to develop estimates of past levels. The highest annual average THM level in the re-analysis was156 μg/l compared to 74 μg/l for the original analysis. This allowed the analysis of subjects exposed at higher levels（〉 96 μg/l）. The re-analysis established a new approach, based on case studies and an understanding of the water quality and operational parameters that impact DBP formation, for determining historical exposure.

Morinda citrifolia l (Noni) has been used in folk remedies by Polynesians for over 2000 years, and is reported to have a broad range of therapeutic effects, including antibacterial, antiviral, antifungal, antitumor, antihelmin, analgesic, hypotensive, anti-inflammatory, and immune enhancing effects. In order to reveal the nutritional and medicinal value of the Noni plant, and to summarize scientific evidence that supports the Polynesians' claim, a literature review and recent advances in Noni research is given below.

Burn recidivism:a 10-year retrospective study characterizing patients with repeated burn injuries at a large tertiary referral burn center in the United States
