Background:Pediatric cardiology patients often experience decreased quality of life(QoL)and higher rates of mental illness,particularly with severe disease,but the relationship between them and comparisons across diagnostic groups are *** mixed-methods cross-sectional study assessed the association between QoL anxiety and behavior problems among children with structural heart disease,arrhythmia,or other cardiac ***:Children(6–14 years,n=76,50%female)and their parents completed measures of QoL(PedsQL),behavior(BASC-2,subset of 19 children)and anxiety(MASC-2,children 8+years).Pearson correlations/regression models examined associations between QoL,behavior and anxiety,controlling for age,sex,diagnosis and condition *** t-tests compared results to published normative ***-structured interviews(15 children,13 parents)examined self-and parent-reported perceptions of the patient’s emotional ***:Child reported social QoL(95%CI difference:−11.0,−2.1),emotional well-being(95%CI difference:−12.5,−3.8)and adjustment skills(95%CI difference:−13.9,−3.0)were significantly below published norms(peither age,sex,nor diagnosis were significantly related to questionnaire *** to children with mild conditions,child and parent-reported QoL was significantly lower in children with complex conditions(np2≥0.14,per QoL was strongly associated with more internalizing problems based on both child(r=−0.78,pent(r=−0.67,pesponses suggested emotional distress was attributed to“normal”child problems rather than the child’s cardiac ***:Children with complex cardiac conditions,regardless of diagnosis,had diminished *** the PedsQL in clinical settings may help identify pediatric cardiology patients experiencing emotional distress(i.e.,anxiety and depression).

In this paper a geomorphic-centered system was proposed for classifying the wetlands on the Qinghai-Tibet Plateau in western China, where the flora comprises primarily grasses. Although the geomorphic properties (e.g., elevation and morphology) of wetlands form the primary criteria of classification, this system also takes hydrological processes into implicit consideration. It represents an improvement over the hydrogeomorphic perspective as the relative importance of the two components (wetness and landform) of wetlands is clearly differentiated. This geomorphic-centered perspective yields insights into the hydrogeomorphic dynamics of plateau wetlands while indicates their vulnerability to change and degradation indirectly. According to this geomorphic-centered perspective, all plateau wetlands fall into one of the seven types of alpine, piedmont, valley, terrace, floodplain, lacustrine, and riverine in three elevational categories of upland, midland, and lowland. Upland (alpine and piedmont) wetlands with the steepest topography are the most sensitive to change whereas midland (floodplain, terrace and valley) wetlands are less vulnerable to degradation owing to a high water reserve except terrace wetlands. They have a dry surface caused by infrequent hydrological replenishment owing to their higher elevation than the channel. Low lying (lacustrine and riverine) wetlands are the most resilient. The geomorphic-centered perspective developed in this paper provides a framework for improving recognition and management of wetlands on the Plateau. Resilient wetlands can be grazed more intensively without the risk of degradation. Fragile and vulnerable wetlands require careful managementto avoid degradation.

Background: The effect of reducing ritonavir boosting doses on the efficacy and safety of fosamprenavir-based regimens has not been well studied. Methods: In a 52-week, phase 4, open-label, single-center pilot study, 26 antiretroviral-naive, HIV-infected patients with viral loads >1000 copies/mL received induction with fosamprenavir/ritonavir 1400 mg/200mg plus abacavir/lamivudine 600 mg/300mg once daily for 28 weeks. Patients achieving a viral load 10 copies/mL and CD4+ count 110/mm3. Of 12 induction/maintenance completers, 10 (83%) achieved viral loads 3 at baseline to 292/mm3 at induction-week 28 and to 296/mm3 at maintenance-week 24. The incidence of adverse events at maintenance-week 24 did not differ from that at induction-week 28 (P > 0.05). Median fasting total-cholesterol, LDL-cholesterol, and triglycerides remained below NCeP cut-off levels. Baseline/induction-week 28/maintenance-week 24 median total-cholesterol was 130/177/183 mg/dL, LDL-cholesterol 78/107/114 mg/dL, HDL-cholesterol 33/41/43 mg/dL, total-cholesterol: HDL-cholesterol ratio 3.9/4.3/4.3, and triglycerides 93/145/119 mg/dL. During induction, total VLDL/chylomicron, LDL, and HDL particles increased;during maintenance, VLDL/chylomicron particles decreased, but LDL and HDL particle concentrations did not notably change. Conclusions: Reducing ritonavir boosting from 200 mg to 100 mg once daily in HIV-infected patients stabilized on once-daily fosamprenavir/abacavir/lamivudine resulted in maintenance of virologic suppression, enhanced CD4+ count, and improved triglycerides.

Tumor necrosis factor-α inhibitors (anti-TNFs) are effective in the treatment of inflammatory bowel disease (IBD) recalcitrant to conventional medical therapy. As the peak incidence of IBD overlaps with the prime reproductive years, it is crucial to establish pharmacologic regimens for women of childbearing age that achieve effective disease control without posing significant fetal harm. A systematic literature review was performed to identify all human studies with birth outcomes data after maternal exposure to infliximab, adalimumab, or certolizumab pegol within 3 mo of conception or during any trimester of pregnancy. Live births, spontaneous abortions or stillbirths, preterm or premature births, low birth weight or small for gestational age infants, and congenital abnormalities were recorded. Fifty selected references identified 472 pregnancy exposures. The subsequent review includes general information regarding anti-TNF therapy in pregnancy followed by a summary of our findings. The benefits of biologic modalities in optimizing disease control during pregnancy must be weighed against the potential toxicity of drug exposure on the developing fetus. Although promising overall, there is insufficient evidence to prove absolute safety for use of anti-TNFs during pregnancy given the limitations of available data and lack of controlled trials.
