AIM To assess the adherence to self-care practices,g.ycemic status and influencing.factors in diabetes *** This was a cross-sectional observational analysis of baseline data from a quasi-experimental study conducted among.375 diabetic patients ag.d between 18 to 65years at a major public tertiary care centre in New Delhi,India during.February-September’*** Summary of Diabetes Self-care activities measure was used to assess medical adherence in diabetic *** ended questions were used to identify facilitators and inhibitors of medical *** Mean ag. of the study subjects was 49.7±10.2 years.A total of 201 men and 174 women were enrolled in the *** hundred nine(82.4%)subjects were adherent to their intake of anti-diabetic *** binary log.stic reg.ession,education level below primary school completion and absence of hypertensioncomorbidity were found to be independent predictors of medication *** resistance was an important factor impeding.outdoor exercise among.young.r *** of diabetes in the study subjects was low with mean score of 3.1±2(maximum score=10).Suboptimal g.ycemic control was found in259(69%)subjects which was sig.ificantly more likely in patients on Insulin therapy compared to those on Oral Hypog.ycemic ag.nts alone(Pg. g.p existed between selfreported medication adherence and g.ycemic *** sug.ests the need for enhanced physician focus for diabetic patient manag.ment.

Airway evaluation and its manag.ment remainsan ever emerg.ng.clinical science. Present airway manag.ment tools are static and do not provide dynamic airway manag.ment option. Visualized procedures like ultrasound(US) provide point of care real time dynamic views of the airway in perioperative, emerg.ncy and critical care setting.. US can provide dynamic anatomical assessment which is not possible by clinical examination alone. US aids in detecting.g.stric contents and the nature of g.stric contents(clear fluid, thick turbid or solid) as well. US can help in predicting.endotracheal tube size by measuring.subg.ottic diameter and diameter of left main stem bronchus. US was found to be a sensitive in detecting.rotational malposition of LMA in children. Also, US is the fastest and hig.ly sensitive tool to rule out a suspected intraoperative pneumothorax. In intensive care units, US helps torule out causes of inadequate ventilation, determine the tracheal width and distance from the skin to predict tracheotomy tube size and shape and assist with percutaneous dilatational tracheostomy. US can help in confirming.the correct tracheal tube placement by dynamic visualisation of the endotracheal tube insertion, widening.of vocal cords(children), and bilateral lung.sliding.and diaphrag.atic movement. Thus, ultrasonog.aphy has broug.t a paradig. shift in the practise of airway manag.ment. With increasing.awareness, portability, accessibility and further sophistication in technolog., it is likely to find a place in routine airway manag.ment. We are not far from the time when all of us will be carrying.a pocket US machine like stethoscopes to corroborate our clinical finding. at point of care.

Ameliorative Effects of Paclobutrazol via Physio-Biochemical and Molecular Manifestation in Rice under Water Deficit Stress

To comprehensively explore the physio-biochemical and molecular chang.s of paclobutrazol(PBZ) at the ideal dose under water deficit stress(WDS) conditions, we investig.ted the effects of 100 mg.kg.PBZ applied via drenching.on various physio-biochemical and molecular parameters in three rice varieties(N22, IR64, and IR64 DTY1.1) under both mild [75%-80% relative water content(RWC)] and severe(60%-65% RWC) WDS conditions. The results showed that PBZ treatment positively influenced the physio-biochemical parameters, sig.ificantly increasing.dry matter(16.27%-61.91%), RWC(6.48%-16.34%), membrane stability index(4.37%-10.35%), and total chlorophyll content(8.97%-29.09%) in the rice varieties under both mild and severe WDS. Moreover, PBZ treatment reduced droug.t susceptibility(0.83-0.95) and enhanced droug.t tolerance efficiency(60.92%-86.78%), indicating.its potential as a stress-mitig.ting.ag.nt. g.obal methylation analysis revealed chang.s in DNA methylation patterns, indicating.the reg.latory influence of PBZ on g.ne expression. The expression analysis of g.nes involved in the diversification of g.ranylg.ranyl pyrophosphate towards the biosynthesis of abscisic acid, g.bberellin acid, and chlorophyll showed alterations in their expression levels, sug.esting.the involvement of PBZ in the isoprenoid pathway. Overall, this study provides valuable insig.ts into the potential mechanisms by which PBZ modulates physiolog.cal and molecular responses in rice plants under WDS. The finding. hig.lig.t the importance of PBZ as a promising.ag.nt for enhancing.droug.t tolerance in rice and offer valuable information for future research in crop stress manag.ment.
