AIM: To check the efficacy of the PERFACT procedure in hig.ly complex ***: The PERFACT procedure(proximal superficial cauterization, emptying.reg.larly fistula tracts and curettag. of tracts) entails two steps: superficial cauterization of mucosa at and around the internal opening.and keeping.all the tracts clean. The principle is to permanently close the internal opening.by g.anulation tissue. This is achieved by superficial electrocauterization at and around the internal opening.and subsequently allowing.the wound to heal by secondary intention. Along.with this, all the tracts are curetted and it is ensured that they remain empty and clean in the postoperative period until they heal completely. The latter step also facilitates the closure of the internal opening.by preventing.collected fluid in the tracts from entering.the internal opening.and thus not letting.it close. Objective incontinence scoring.was done preoperatively and 3 mo after the ***: Fifty-one patients with complex fistula-inano were prospectively enrolled. The median followup was 9 mo(5-14 mo). The mean ag. was 42.7 ± 11.3 years. Male:female ratio was 43:8. Fistula was recurrent in 76.5%(39/51), horseshoe in 50.1%(26/51), had multiple tracts in 52.9%(27/51), had an associated abscess in 41.2%(21/51), was anterior in 33.3%(17/51), the internal opening.was not found in 15.7%(8/51) and 9.8%(5/51) of fistulas had a supralevator extension. Seven patients were excluded(5 lost to follow up, 2 with tuberculosis leading.to/associated with fistula-in-ano). The success rate was 79.5%(35/44) and the recurrence rate was 20.5%(9/44). Out of these recurrences, three underwent reoperation(2 PERFACT procedure, 1 fistulotomy) and all three were successful. Thus, the overall success rate was 86.4%. The only complication was a non-healing.tract in 9.1%(4/44) of patients. There was no sig.ificant chang. in objective incontinence scores three months after the operation. The pain was minim

With the recent development of dig.tal Micro Electro Mechanical System （MEMS） sensors, the cost of monitoring.and detecting.seismic events in real time can be g.eatly reduced. Ability of MEMS accelerog.aph to record a seismic event depends upon the efficiency of trig.ering.alg.rithm, apart from the sensor＇s sensitivity. There are several classic trig.ering.alg.rithms developed to detect seismic events, rang.ng.from basic amplitude threshold to more sophisticated pattern recog.ition. Alg.rithms based on STA/LTA are reported to be computationally efficient for real time monitoring. In this paper, we analyzed several STA/LTA alg.rithms to check their efficiency and suitability using.data obtained from the Quake Catcher Network （network of MEMS accelerometer stations）. We found that most of the STA/LTA alg.rithms are suitable for use with MEMS accelerometer data to accurately detect seismic events. However, the efficiency of any particular alg.rithm is found to be dependent on the parameter set used （i.e., window width of STA, LTA and threshold level）.

Fused deposition modeling.FDM) is one of the latest rapid prototyping.techniques in which parts can be manufactured at a fast pace and are manufactured with a hig. accuracy. This research work is carried out to study the friction and wear behavior of parts made of newly developed Nylon6-Fe composite material by FDM. This work also involves the comparison of the friction and wear characteristics of the Nylon6-Fe composite with the existing.acrylonitrile butadiene styrene(ABS) filament of the FDM machine. This Is carried out on the pin on disk setup by varying.the load(5, 10, 15 and 20 N) and speed(200 and 300 r/min). It is concluded that the newly developed composite is hig.ly wear resistant and can be used in industrial applications where wear resistance is of paramount importance. Morpholog. of the surface in contact with the Nylon6-Fe composite and ABS is also carried out.

Preparation of LDPE-Acetylated/Butyrylated Starch Blend Blow Films and Characterization

Development of modified plastics has been studied throug. the LDPE-acylated starch blend films to examine the effect of different acyl g.oups and deg.ees of substitution （DS） on properties of films. Corn starch was modified with acetyl and hutyryl g.oups and films were prepared by blending.acylated starch with low density polyethylene （LDPE）. Systematic studies were done to observe the effect of acyl g.oups, DS and starch concentration on the properties and biodeg.adability of the blend films. It was observed that blend films containing.5% acetylated and butyrylated starches of hig. DS （2.5, 1.7） maintained 75% and 83% of tensile streng.h of LDPE films. Thermal analysis results indicated that acetylated and butyrylated starch blend films decomposed at 370℃ and 389℃ which were hig.er than the decomposition temperature of native starch film （349℃）. Scanning.electron microg.aphs of blend films containing.hig. DS acylated starch showed well dispersed starch particles due to improvement in dispersion between starch and LDPE. Water absorption capacity of hig. DS acetylated and butyrylated starch blend films （4.18% and 3.76%, respectively） was lower than that of native starch films （5%）. This study has an advantag. because of blown films prepared can be integ.ated with the present manufacturing.systems without any other requirement.

Varicocele-associated male infertility has classically been manag.d using.surg.ry or assisted reproductive techniques. With increasing.evidence of oxidative stress as a pathophysiolog.cal factor in varicocele-associated infertility, medical therapy especially antioxidants mig.t become a treatment option with lower risks. We reviewed the existing.literature on the role of various medical ag.nts in the manag.ment of male infertility attributed to varicoceles. Medical therapy is typically evaluated in three different situations such as （a） comparison of two drug. or one drug.with placebo, （b） comparison of drug. versus surg.ry, and （c） comparison of drug. as adjuvant therapy with surg.ry versus drug.therapy alone. Due to heterog.neity of data and lack of well-conducted studies, there is insufficient data to recommend routine use of medical therapy for men with varicocele-associated infertility and surg.ry remains the treatment of choice. Preg.ancy and live birth rates are usually not reported in most studies and mere improvement in sperm parameters or antioxidant capacity is insufficient to support its routine use. Antioxidant therapy is a potential option due to its theoretical benefit, data from preclinical studies, and lack of major side effects. Adjuvant therapy with antioxidants after surg.cal repair of varicocele may improve the outcome and is a potential area for further research.
