Objectives:To detail the US multi-institutional experience with the Occlutech©(Occlutech International AB,Helsingborg,Sweden)atrial flow regulator(AFR)in children and adults with acquired or congenital heart disease.Background:The creation of a long-term atrial communication is desirable in several cardiovascular disease phenotypes,most notably pulmonary arterial hypertension,disorders of increased left ventricular filling and increased cavopulmonary pressures in patients with a Fontan type circulation.Methods:Patients were identified for inclusion from the AFR device manufacturer database.Data was collected using a RedCap database following IRB approval.8 weeks of follow up data was sought for each patient based on available data.Data was analyzed and summarized using SPSS.Results:We report the experience of 6 US centers in the implantation of AFR devices in 15 patients,across a wide age range,with different disease phenotypes and a variety of indications.Implantation was technically successful in all patients and improvement was noted in both clinical and hemodynamic parameters.There were no immediate or intermediate term complications reported.3 patients died remote from implantation.Their deaths were not felt to be related to the AFR device or related procedural complications.Conclusion:Compassionate use of the AFR device in children and adults with congenital&acquired heart disease is technically feasible and produces beneficial short term hemodynamic and symptomatic improvement.Widespread uptake of this technique and treatment at specialist centers has the potential to provide significant benefits to a variety of complex patients with currently limited treatment options and indeterminate prognosis.

This study investigates how financial literacy and behavioral traits affect the adoption of electronic payment(ePayment)services in japan.We construct a financial literacy index using a representative sample of 25,000 individuals from the Bank of japan’s 2019 Financial Literacy Survey.We then analyze the relationship between this index and the extensive and intensive usage of two types of payment services:electronic money(e-money)and mobile payment apps.Using an instrumental variable approach,we find that higher financial literacy is positively associated with a higher likelihood of adopting ePayment services.The empirical results suggest that individuals with higher financial literacy use payment services more frequently.We also find that risk-averse people are less likely to adopt and use ePayment services,whereas people with herd behavior tend to adopt and use ePayment services more.Our empirical results also suggest that the effects of financial literacy on the adoption and use of ePayment differ among people with different behavioral traits.

Interannual dynamics of stemwood nonstructural carbohydrates in temperate forest trees surrounding drought

Interactions between water and carbon dynamics underlie drought-related tree mortality.While whole-tree water relations have been shown to play a key role in the response to and recovery from drought,the role of nonstructural carbohydrates(NSC) and how their storage and allocation changes surrounding drought events deserves further attention and is critical for understanding tree survival.Here,we quantified in situ NSC responses of temperate forest trees to the 2016 drought in the northeastern United States.Sugar and starch concentrations were measured in the stemwood of five tree species from 2014 to 2019,which allowed us to monitor NSCs in relation to climatic conditions before,during,and after the natural drought.We found that immediately following the drought,measured stemwood NSC concentrations decreased.However,NSC concentrations rebounded quickly within three years.Notably,trees allocated proportionally more to starch than to sugars following the 2016 drought.In winter 2017,starch comprised 45% of total stemwood stores,whereas starch made up only 1-2% in other years.Further,we modeled and assessed the climatic drivers of total NSC concentrations in the stem.Variation in total NSC concentrations was significantly predicted by the previous year’s temperature,precipitation,and standardized precipitation-evapotranspiration index(SPEI),with stemwood concentrations decreasing following hotter,drier periods and increasing following cooler,wetter periods.Overall,our work provides insight into the climatic drivers of NSC storage and highlights the important role that a tree’s carbon economy may play in its response and recovery to environmental stress.

In insects and other ectotherms,cold temperatures cause a coma resulting from loss of neuromuscular function,during which ionic and metabolic homeostasis are progressively lost.Cold adaptation improves homeostasis during cold exposure,but the ultimate targets of selection are still an open question.Cold acclimation and adaptation remodels mitochondrial metabolism in insects,suggesting that aerobic energy production during cold exposure could be a target of selection.Here,we test the hypothesis that cold adaptation improves the ability to maintain rates of aerobic energy production during cold exposure by using^(31)P NMR on live flies.Using lines of Drosophila melanogaster artificially selected for fast and slow recovery from a cold coma,we show that cold exposure does not lower ATP levels and that cold adaptation does not alter aerobic ATP production during cold exposure.Cold-hardy and cold-susceptible lines both experienced a brief transition to anaerobic metabolism during cooling,but this was rapidly reversed during cold exposure,suggesting that oxidative phosphorylation was sufficient to meet energy demands below the critical thermal minimum,even in cold-susceptible flies.We thus reject the hypothesis that performance under mild low temperatures is set by aerobic ATP supply limitations in D.melanogaster,excluding oxygen and capacity limitation as a weak link in energy supply.This work suggests that the modulations to mitochondrial metabolism resulting from cold acclimation or adaptation may arise from selection on a biosynthetic product(s)of those pathways rather than selection on ATP supply during cold exposure.

Gliomas are the most common form of central nervous system tumor. The most prevalent form, glioblastoma multiforme, is also the most deadly with mean survival times that are less than 15 months. Therapies are severely limited by the ability of these tumors to develop resistance to both radiation and chemotherapy. Thus, new tools are needed to identify and monitor chemoresistance before and after the initiation of therapy and to maximize the initial treatment plan by identifying patterns of chemoresistance prior to the start of therapy. Here we show how magnetic resonance imaging, particularly sodium imaging, metabolomics, and genomics have all emerged as potential approaches toward the identification of biomarkers of chemoresistance. This work also illustrates how use of these tools together represents a particularly promising approach to understanding mechanisms of chemoresistance and the development individualized treatment strategies for patients.

Low-intensity extracorporeal shock wave therapy for Peyronie’s disease:a single-center experience

The aim of this article is to assess the outcomes of a low-intensity extracorporeal shock wave therapy(LiESWT)protocol for the treatment of Peyronie’s disease(PD).Patients treated for PD were prospectively recorded,and data were retrospectively reviewed.Age,characteristics of fibrous plaques,concomitant treatments,International Index of Erectile Function(IIEF-5),Lue score,and pain score on Likert scale were collected.Patients in acute phase of PD and an angulation of<40°were included.The protocol consisted of 6 weekly sessions of 4000 pulses each,applied from different directions,with a maximal power of 20 W and 8 Hz frequency.We included 39 patients(median age:56.8 years,interquartile range[IQR]:35.8–62.2 years).The median number of sessions received per patient was 7.2.After treatment,the median Lue score decreased from 6.8 initially to 3.3(P=0.003),the median Likert pain score dropped from 1.8 to 0.7(P=0.004),the median plaque size was reduced from 2 cm to 1.2 cm(P=0.08),and the median penile curvature diminished from 31°to 17°(P=0.07).On univariate and multivariate analysis,the only predictors of success were younger age(odds ratio[OR]=0.95,P=0.03 and OR=0.91,P=0.04,respectively)and concomitant use of phosphodiesterase-5 inhibitors(PDE5i;OR=0.92,P=0.02 and OR=0.93,P=0.01,respectively).LiESWT had a favorable impact on Lue score and notably penile pain,curvature,plaque size,and erectile function in patients treated for PD during the early inflammatory phase,with no side effects.Younger age and concomitant use of PDE5i were the only success predictors.

The hydrogenation of levulinic acid to γ-valerolactone over Cu–ZrO2 catalysts prepared by a pH-gradient methodology

A novel pH gradient methodology was used to synthesise a series of Cu–ZrO2 catalysts containing different quantities of Cu and Zr.All of the catalysts were highly selective to the desired product,γ-valerolactone, and are considerably more stable than Cu–ZrO2 catalysts prepared by other co-precipitation methods for this reaction.Characterisation and further investigation of these catalysts by XRD, BET, SEM and XPS provided insight into the nature of the catalytic active site and the physicochemical properties that lead to catalyst stability.We consider the active site to be the interface between Cu/CuOxand ZrOx and that lattice Cu species assist with the dispersion of surface Cu through the promotion of a strong metal support interaction.This enhanced understanding of the active site and roles of lattice and surface Cu will assist with future catalyst design.As such, we conclude that the activity of Cu–ZrO2 catalysts in this reaction is dictated by the quantity of Cu–Zr interface sites.

Association between high deductible health plan coverage and age at pediatric umbilical hernia repair

Background High deductible health plans(HDHPs)are associated with the avoidance of both necessary and unnecessary healthcare.Umbilical hernia repair(UHR)is a procedure that is frequently unnecessarily performed in young children,contrary to best practice guidelines.We hypothesized that children with HDHPs,as compared with other types of commercial health plans,are less likely to undergo UHR before 4 years of age but are also more likely to have UHR delayed beyond 5 years of age.Methods Children aged 0-18 years old residing in metropolitan statistical areas(MSAs)who underwent UHR in 2012-2019 were identified in the IBM Marketscan Commercial Claims and Encounters Database.A quasi-experimental study design using MSA/year-level HDHP prevalence among children as an instrumental variable was employed to account for selection bias in HDHP enrollment.Two-stage least squares regression modeling was used to evaluate the association between HDHP coverage and age at UHR.Results A total of 8601 children were included(median age 5 years,IQR 3-7).Univariable analysis revealed no differences between the HDHP and non-HDHP groups in the likelihood of UHR being performed before 4 years of age(27.7%vs 28.7%,p=0.37)or after 5 years of age(39.8%vs 38.9%,p=0.52).Geographical region,metropolitan area size,and year were associated with HDHP enrollment.Instrumental variable analysis demonstrated no association between HDHP coverage and undergoing UHR at5 years of age(p=0.87).Conclusions HDHP coverage is not associated with age at pediatric UHR.Future studies should investigate other means by which UHRs in young children can be avoided.

Protease supplementation reduced the heat increment of feed and improved energy and nitrogen partitioning in broilers fed maize-based diets with supplemental phytase and xylanase

An experiment was conducted to explore the effects of digestible amino acid(dAA)concentrations and supplemental protease on live performance and energy partitioning in broilers.Ross 308 male broilers(n?288)were distributed into 24 floor pens and offered 1 of 4 dietary treatments with 6 replicates from 1 to 35 d of age.Dietary treatments consisted of a 2×2 factorial arrangement with dAA concentrations(standard and reduced[34 g/kg below standard])and supplemental protease(without or with)as the main factors.At 1,15,28,and 35 d of age,feed and broilerswereweighed to determine live performance.From20 to 23 d of age,a total of 32 birds(2 birds/chamber,4 replicates)were placed in closed-calorimeter chambers to determine respiratory exchange(heat production,HP),apparent metabolisable energy(AME),retained energy(RE),and net energy(NE).From 29 to 35 d of age,supplemental protease in the reduced-dAA diet decreased broiler feed conversion ratio(FCR)by 5.6 points,whereas protease supplementation in the standard-dAA diet increased FCR by 5.8 points.The indirect calorimetry assay revealed that supplemental protease decreased(Pj/kg.Also,from 20 to 23 d of age,broilers offered the reduced-dAA diet with supplemental protease had a higher daily body weight gain(BWG)(t10.4%),N intake(t7.1%),and N retention(t8.2%)than those offered the standard-dAA with supplemental protease.Broilers offered the reduced-dAA without supplemental protease exhibited a 3.6%higher AME-to-crude protein(CP)ratio than those offered other treatments.Protease supplementation in the standard-and reduced-dAA diets resulted in 2.7%and 5.6%lower AME intake-to-N retention ratios,respectively,compared with the unsupplemented controls.Reduced-dAA increased(P<0.05)AME intake(t4.8%),RE(t9.8%),NE intake(t5.8%),NE intake-to-CP ratio(t3.0%),and RE fat-to-RE ratio(t8.6%).Protease supplementation increased(P<0.05)respiratory quotient(t1.2%)and N retention-to-N intake ratio(t2.2%),NE-to-AME ratio(t1.9%),and reduced HP(3.6%),hea

Linking science and decision making to promote an ecology for the city:practices and opportunities

To promote urban sustainability and resilience,there is an increasing demand for actionable science that links science and decision making based on social-ecological knowledge.Approaches,frameworks,and practices for such actionable science are needed and have only begun to emerge.We propose that approaches based on the co-design and co-production of knowledge can play an essential role to meet this demand.Although the antecedents for approaches to the co-design and co-production of knowledge are decades old,the integration of science and practice to advance urban sustainability and resilience that we present is different in several ways.These differences include the disciplines needed,diversity and number of actors involved,and the technological infrastructures that facilitate local-to global connections.In this article,we discuss how the new requirements and possibilities for co-design,co-production,and practical use of social-ecological research can be used as an ecology for the city to promote urban sustainability and resilience.While new technologies are part of the solution,traditional approaches also remain important.Using our urban experiences with long-term,place-based research from several U.S.Long-Term Ecological Research sites and U.S.Department of Agriculture,Forest Service Urban Field Stations,we describe a dynamic framework for linking research with decisions.We posit that this framework,coupled with a user-defined,theory-based approach to science,is instrumental to advance both practice and science.Ultimately,cities are ideal places for integrating basic science and decision making,facilitating flows of information through networks,and developing sustainable and resilient solutions and futures.
