Introduction: In our practice, renal measurements in patients with normal renal function usually appear to be lower compared to standards reported in reference literature. The standards are probably different in our context. Given the importance of renal biometrics in nephrological practice, we felt it necessary to carry out this preliminary study in order to identify an order of reference measurements appropriate to our context. Methods: This was a cross-sectional study that took place from 18 August to 04 November 2018 at the Yalgado Ouedrarogo University Hospital Center. The first 100 subjects aged between 18 and 30 years who met the following inclusion criteria were selected: to be black african Burkinabe and to have normal renal function. The height, width and thickness of each kidney were measured using ultra-sound scanners. For statistical tests, a value of p Results: The average age was 23.9 ± 6.1 years and the sex ratio was 2.2. Mean heights, widths and renal thicknesses were 94.5 ± 14.5, 38.7 ± 10.7 and 36.3 ± 10.3 mm, and 96.7 ± 16.7, 42.7 ± 16.7 and 36.8 ± 10.8 mm respectively for right and left kidneys. The distribution of the different renal dimensions in our sample followed a normal distribution. In the particular case of the right kidney, the probability that its height was between 79.2 and 109.7 mm was 0.95 and the probability that it was less than 79.2 mm or greater than 109.7 mm was 0.05. Renal height was significantly higher in subjects with height greater than 1.70 m (p ≤ 0.02). The left kidneys were on average significantly larger than the right kidneys (p = 0.0001). Conclusions: Our study is not extrapolable to the general population of Burkina Faso. It suggests, however, that the kidneys of the apparently healthy Burkinabe subject are smaller than what is reported in anatomy reference books. Epidemiological Profile of Chronic Hemodialysis Patients in Ouagadougou

Goal: By describing the epidemiology of chronic hemodialysis patients in the single hemodialysis unit of Burkina Faso, we want to contribute to the prevention and improvement of chronic renal failure management (CRF) in this country. Patients and Methods: A descriptive cross-sectional study was conducted in the hemodialysis unit of the University Hospital Center Yalgado Ouedraogo (UHC-YO) during the period from February 12 to May 15, 2015. The patients who started hemodialysis in this unit and were treated for at least three months were included in this study. The sociodemographic and clinical data were collected. The statistical significance was defined for a probability (p ≤ 0.05). Results: One hundred and seventy-two patients (71.2% of the 240 patients of the unit) have been included. The sex ratio was 1.6. The average age was 45.2 ± 12.4 years old. The presumed causes of CRF have been identified in 134 cases (77.9%). The most frequent were hypertensive nephropathy (65 cases;48.5%), chronic glomerulonephritis (41 cases;30.6%;including 11 viral origin and 16 with history of recurrent otorhinolaryngologic infections and/or urinary schistosomiasis). Hemodialysis began in an emergency context in 118 cases (68.6%). The average duration of hemodialysis was 29.4 ± 28.4 months. Conclusion: The main suspected causes of CRF were hypertension and chronic glomerulonephritis. The origin of the latter seemed more often infectious. Prevention of CRF in Burkina Faso should be focused on that of hypertension and infectious diseases.

Lesions resulting from duodenal involvement are rare. Their diagnosis and management remain difficult because of the topographic location of most of the duodenum in the retroperitoneum. We report a case of isolated duodenal rupture. At the time of the first operation, the duodenal lesion had gone unnoticed. During our management, we discovered a rupture of the lower knee of the second duodenum and we performed a triple anastomosis after the closure of the ends (proximal and distal). The immediate postoperative course was complicated by the death of the patient on the second day of surgery. Duodenal lesions have much mortality which is increased by the delay in diagnosis and management.

The issue of wound care has always played an important role in the practice of medicine. This is evidenced by Ambroise Paré’s decision to publish his first work on this subject, “La manière de traiter les Plaies” (The Way of Treating Wounds) in 1545. Objective: To evaluate the therapeutic methods we practice and to describe the impact of the pathology on socio-economic and professional development. Patients and Method: This was a retrospective study carried out at the Fousseyni Daou Hospital in Kayes from January 1, 2018, to December 31, 2020. We included all patients with an ulcero-necrotic wound on immunocompetent terrain hospitalized in the department. Patients with ulcerative necrotic wounds of diabetes, HIV, or cancerous origin were not included. The parameters studied were: etiologies, local care, sequelae, and socio-economic and professional aspects. Results: We collected 57 patients of whom 43 were men and 14 were women, i.e. a sex ratio of 3. The mean age was 40.7 years with a standard deviation of 8.4 with extremes (7 years and 80 years). The average consultation time was 25.1 days. The most represented socio-professional stratum was agropastoralism in 37 cases (65%). The predominant etiological factor was neglected traumatic wounds in 34 cases (59.6%). The site was the lower limb in 39 cases (68.5%). The germ found was Staphylococcus aureus in 21 cases (36.8%). The particularity during local care was the use of table sugar in 9 cases (15.7%) and maggot therapy in 2 cases (3.5%). Hyperthermia was the clinical sign of aggravation in 22 cases (38.6%) and we diagnosed 2 cases (3.5%) of tetanus. The mortality rate was 15.7% (9 cases) and 30 cases (52.7%) of sequelae after recovery. The average length of hospitalization was 38 days. Hospital care was provided by the social welfare service in 35 cases (61.4%). Nineteen (19) patients (39.5%) were unable to resume their socio-professional activity. Okra is one of the most widespread vegetable crops in the world, particularly in West Africa. However, several factors influence okra crops as biotic and abiotic factors. Among the diseases affecting its culture, okra leaf curl disease is a major threat. This study aims to assess begomoviruses from okra plants with symptoms like leaves curl by molecular approach. A total of thirteen serologically positive samples were tested by PCR assay, and one sample was sequenced among them. The begomovirus was found in six isolates, Bhendi yellow vein mosaic virus (BYVMV) in three isolates, and Okra enation leaf curl virus (OELCuV) in three isolates, respectively. The begomovirus isolate sequences shared 90% identity with the Cotton leaf curl Gezira virus. Mixed infections between these viruses were found. Thus, these results highlight the need to monitor the spread of these disease-causing viruses for okra crops in Mali. In addition, they can also lead to a considerable loss in okra fields in Koulikoro, which is an agricultural region by excellence.

The volvulus of the sigmoid is known since ancient Egypt. In the 5th century BC, hippocrates laid the foundations for its management. The first observation of this condition was not reported until 1836 by Von Rokitansky, and then in 1859, Melchior described its physiopathological consequences. It was the Norwegian Brusgaard who reported, for the first time, in 1947, the effectiveness of a non-operative treatment. Purpose: Describe morbidity, mortality and the impact of co-morbidity factors on the choice of operative techniques. Patients and Methods: This is a retrospective cross-sectional study performed at the Fousseyni Daou Hospital in Kayes from January 2014 to December 2021. We included all patients operated for sigmoid volvulus without necrosis. The parameters studied were the comorbidity factor, surgical modalities, morbidity and mortality. Results: We collected 31 patients, of whom 29 were men and 2 were women, for a sex ratio of 14.5. The mean age was 55 years with extremes (29 - 78 years). Sigmoidectomy with colorectal anastomosis was performed in 19 cases (61.3%), the average age of these patients was 46.16 years, the comorbidity factor was 1 case (3.2%), the postoperative course was simple in 12 cases (63.1%), the morbidity was 6 cases (26.3%) and mortality 1 case (3.2%). Untwisting with colopexy was performed in 11 cases (35.5%), the average age was 65.91 years, comorbidities were present in 9 cases (82%), the postoperative course was simple in 9 cases (82%), morbidity was 1 case (9%) and mortality 1 case (9%). Hartman colostomy was performed in 1 case (3.2%), the age was 60 years, the morbidity was nil. Overall, the postoperative course was simple in 22 cases (71%), the overall morbidity was 7 cases (22.6%) and mortality 2 cases (6.4%). Conclusion: At the end of our study, we can note that the choice of the operative technique for the volvulus of the sigmoid without necrosis can be influenced by the presence of major comorbidity and the morbi-mortality depends on it.

Aïnhum or spontaneous dactylolysis is a progressive constriction of the digito-plantar fold of the fifth toe which leads after several years of evolution to a spontaneous amputation of the toe. Classically, a distinction is made between “true Aïnhum” of unknown cause, which affects only blacks and those of African (sub-Saharan) descent;“pseudo-Aïnhum”, which follows various causes such as an inflammatory flange or foreign body constriction;and finally, palmo-plantar keratoderma, of genetic origin, such as Vohwinkel disease. We report a case of Aïnhum received at stage III of the pathology and who benefited from an amputation of the fifth toe.

Situs inversus is a rare congenital anomaly referring to the mirror image of the abdominal and thoracic viscera. It can be discovered routinely or on the occasion of a complication related or not to situs inversus. We report a case of Situs inversus discovered incidentally during surgery for acute intestinal obstruction caused by an iatrogenic uterine perforation.

Identifying Risk Factors for Pulmonary Tuberculosis Diagnosis Delays in Mali a West-African Endemic Country

Background: Tuberculosis was the deadliest infectious agent before covid-19;1.5 million deaths in 2020. Despite, a variety, of easy and cheap diagnostic tools, detection rates still fall below 90%;diagnosis delays are long exceeding 30 days in many continents. This study aimed to determine risk factors for pulmonary TB diagnosis delays in Mali. Methods: A cross-sectional study was conducted in Bamako to include pulmonary TB patients at treatment initiation centers. Verbal consent was obtained before the interview. Demographics, clinical, treatment cost, and patient, medical, and diagnostic delays were computed using SPSS 25.0 considering a significance level p Results: In total 266 patients were included, 80.8% were male, mean age was 40.5 ± 12 years, primary education level was 50.4%, treatment cost before diagnosis was 100 - 200 thousand CFA in 65.4%, smokers were 42.1%, median patient, medical and total diagnostic delays were 58, 57 and 114 days respectively. Education level below university, social reasons, and non-request of health workers were identified as independent risk factors for diagnostic delay > 100 days in Mali. Conclusion: Diagnostic delay is relatively very long in Mali, there is an urgent need for identification and action to shorten the delays to limit the transmission chain and avoid disabling pulmonary sequels.

Effects of Stone Barriers on Soil Physicochemical Characteristics under Cotton (<i>Gossypium hirsutum</i>L.) Cropping Fields Northern of Côte d’Ivoire

In the north of Cote d’Ivoire, the rural economy is heavily dependent on cotton cultivation which is characterized by low yield. One of the main causes of low productivity is low level of soil fertility caused by erosion. The stone barriers technique is used to fight against erosion and to improve soil fertility. This work was carried out to assess the effects during 1-year-old stone barriers on selected soil physicochemical properties on two geographical locations where soil samples from 0 to 30 cm depth were selected. A total of 60 composite soil samples (30 × 2) were collected. One year after installing the stone bunds in cotton cultivation, the soil was sampled under the same conditions. The soil samples were analyzed for pH (H2O), organic carbon (SOC), total nitrogen (TN), available phosphorus (Av.P), cation exchange capacity (CEC), exchange bases (Ca, K, Mg, Na) and particle size (clay, coarse silt and sand) contents. The results show that the stone bunds significantly increased the water pH, the soil levels of calcium, magnesium, silt and coarse sand. On the other hand, they significantly lowered the levels of assimilable phosphorus, clay and fine sand in soils. CEC and soil carbon, nitrogen, potassium, sodium and fine silt content did not vary significantly. This study must be repeated in the years to come in order to determine the impact of stone barriers in the medium and long term on soil fertility in the north of Côte d’Ivoire.
