Generalized megaviscera of lupus:Refractory intestinal pseudo-obstruction,ureterohydronephrosis and megacholedochus

dilated dysfunction involving multiple visceral organs has been reported in patients with systemic lupus erythematosus （SLE）. Chronic intestinal pseudoobstruction （CIPO） resulting from intestinal smooth muscle damage has presented in conjunction with ureterohydronephrosis and, more rarely, biliary dilatation （megacholedochus）. While the molecular pathogenesis is largely unknown, observed histopathologic features include widespread myositis, myocyte necrosis in the intestinal muscularis propria with subsequent atrophy and fibrosis, preserved myenteric innervations and little vasculitis. High dose immunosuppression usually results in resolution of symptoms with recovery of smooth muscle function, indicative of an autoimmune etiology. We report a patient with SLE who presented with intestinal pseudo-obstruction, ureterohydronephrosis and megacholedochus, and present images that illustrate megaviscera simultaneously involving all 3 visceral organs. Since the co-manifestation of all 3 is unusual and has been reported only once previously, we have termed this rare clinical syndrome generalized megaviscera of lupus （GML）. Although the SLE disease-activity parameters responded to aggressive immunomodulative therapy in our patient, clinical evidence of peristaltic dysfunction persisted in all involved viscera. This is a variation from the favorable outcomes reported previously in SLE patients with GML and we attribute this poor clinical outcome to disease severity and, most importantly, delayed clinical presentation. Since inflammation followed by atrophy and fibrosis are key aspects in the pathogenesis and natural history of GML, the poor response in our patient who presented late in the clinical course may be the result of ＇burnt out＇ inflammation with irreversible end-stage fibrosis. Thus, early recognition and timely initiation of treatment may be the key to recover visceral peristaltic function in patients with GML.

Conditioning and PPP processing of smartphone GNSS measurements in realistic environments

Smartphones typically compute position using duty-cycled Global Navigation Satellite System (GNSS) L1 code measurements and Single Point Positioning (SPP) processing with the aid of cellular and other measurements. This internal positioning solution has an accuracy of several tens to hundreds of meters in realistic environments (hand-held, vehicle dashboard, suburban, urban forested, etc.). With the advent of multi-constellation, dual-frequency GNSS chips in smartphones, along with the ability to extract raw code and carrier-phase measurements, it is possible to use Precise Point Positioning (PPP) to improve positioning without any additional equipment. This research analyses GNSS measurement quality parameters from a Xiaomi MI 8 dual-frequency smartphone in varied, realistic environments. In such environments, the system suffers from frequent phase loss-of-lock leading to data gaps. The smartphone measurements have low and irregular carrier-to-noise (C/N0) density ratio and high multipath, which leads to poor or no positioning solution. These problems are addressed by implementing a prediction technique for data gaps and a C/N0-based stochastic model for assigning realistic a priori weights to the observables in the PPP processing engine. Using these conditioning techniques, there is a 64% decrease in the horizontal positioning Root Mean Square (RMS) error and 100% positioning solution availability in sub-urban environments tested. The horizontal and 3d RMS were 20 cm and 30 cm respectively in a static open-sky environment and the horizontal RMS for the realistic kinematic scenario was 7 m with the phone on the dashboard of the car, using the SwiftNav Piksi Real-Time Kinematic (RTK) solu-tion as reference. The PPP solution, computed using the YorkU PPP engine, also had a 5-10% percentage point more availability than the RTK solution, computed using RTKLIB software, since missing measurements in the logged file cause epoch rejection and a non-continuous solution, a

Modeling fire ignition probability and frequency using Hurdle models: a crossregional study in Southern Europe

densely populated region of ganga Plain is facing aquifer vulnerability through waterborne pollutants and groundwater stress due to indiscriminate abstraction,causing environmental and socio-economic *** address long-term groundwater resilience,it is crucial to understand aquifer heterogeneity and connectivity,groundwater recharge sources,effects of groundwater abstraction *** this con-text,present study aims to understand factors responsible for vertical and spatial variability of groundwater chemistry and to identify groundwater recharge sources in an intensively exploited agrarian region of the ganga *** of chemometric,statistical,and isotopic analysis categorises the alluvial aquifer into zone 1(G1;ground surface to 100 m)and zone 2(G2;>100 m-210 m).The group G1 samples are characterized by a wide variation in hydrochemical species,noted with pockets of F-and NO3-rich groundwater,and fresh to more evolved water types,while group G2 groundwater is characterized by a sharp increase in freshwater types and limited variation in their isotopic and hydrochemical *** G1 groundwater chemistry is governed by soil mineralogy,local anthropogenic inputs(SO42-,Cl-,and NO3-),and manifested by multiple recharge sources(local precipitation,river,canal water,pond).The G2 group is dominated by geogenic processes and mainly recharged by the local *** signatures confirm more evolved water type for group G1 in northwestern region,while fresh-water type covers the rest of the study *** rich groundwater is attributed to sodic water under alkaline conditions and enrichedδ18O values emphasizing role of evaporation in F-mobilization from micas and amphiboles abundant in the *** findings provide insight into potential groundwater vulnerability towards inorganic contaminants,and ground water recharge *** outcome of this study will help to develop aquifer resilience towards indiscriminate groundwater extrac

Objective:To assess the in-vitro antihacterial activity and anti-inflammatory activity of orally administered different extracts(Hydro-alcoholic,methanolic,ethyl acetate and hexane)of Rauvolfia tetraphylla(***)root bark in Carrageetiaii induced acute inflammation in ***:In-vitro antibacterial activity was evaluated for extracts against four Gram positive and four Gram negative bacteria by using cylinder plate ***-alcoholic extract(70%v/v ethanol)at 200,400 and 800 mg/kg doses and methanolic,ethyl acetate and hexane extracts at doses 100,200 and 400 mg/kg were tested for anti-inflammatory activity in Carrageenan induced rat paw oedema model and paw thickness was measured every one hour up to 6 ***:All extracts of *** root bark showed good zone of inhibition against tested bacterial *** Carrageenan induced inflammation model,hydro-alcoholic and methanolic extract of *** root bark at three different doses produced significant(Pduction when compared to vehicle treated control group and hexane,ethyl acetate ***:In the present study extracts of *** root bark shows good in-vitro antibacterial activity and in-vivo anti-inflammatory activity in rats.

Pentoxifylline(PTX)is reported to provide beneficial outcomes in the treatment of neonatal sepsis and serious inflammatory conditions.1,2 In the neonatal intensive care setting,PTX is administered by intravenous(IV)infusion and is routinely co-administered via Ysite with other parenteral medications and/or *** the physicochemical compatibility of PTX with a range of parenteral drugs,IV fluids,and lipid-free parenteral nutrition solutions has been reported,there is an emerging demand for more PTX compatibility data against previously untested medications(ampicillin,cloxacillin,fentanyl,fluconazole,furosemide,hydrocortisone,and ibuprofen lysine)and high-concentration IV drugs(alprostadil,dopamine,epinephrine,midazolam,milrinone,morphine,and norepinephrine)which may be used in fluid-restricted,vulnerable patients.3–5 In order to address this requirement for contemporary information,we investigated the physical and chemical compatibility of PTX with a panel of 20 drugs,in 51 PTX-drug combinations.

Assessing land transformation and associated degradation of the Indian ganga River Basin using forest cover land use mapping and residual trend analysis

The Indian ganga River Basin（IGRB） has experienced continuous land transformation since the Indus Valley Civilisation shifted from the Indus basin to the ganga basin. Particularly in the last few decades the land transformation has increased many-folds due to the changing climate and rapid increase in population. In this paper, we assessed land transformation and associated degradation in the IGRB based on the forest cover land use（FCLU） mapping and residual trend analysis（RTA）. The FCLU maps for 1975 and 2010 were generated using 216 Landsat satellite images and validated using 1509 ground points. We mapped 29 forest and 18 non-forest types and estimated a total loss of 5571 km2forest cover and expansion in settlement areas（5396 km2）. Other major changes mapped include a decrease in wetlands and water bodies, while an increase in agriculture and barren lands with an overall mapping accuracy of 85.3%（kappa, 0.82） and 88.43%（kappa, 0.84） for 1975 and 2010, respectively. We also performed the RTA analysis using GIMMS-NdVI3 g to identify areas of significant negative vegetative photosynthetic change as an indicator for land degradation. All the RTA models showed monotonic nature of the residual trends and resulted as moderately positive but highly significant（Pd degradation in the form of barren land accompanied by a decline in vegetation quality and coverage was found prominent in the basin with a possibility of an accelerated rate of land degradation in future due to the rapid loss of permanent forest cover.

Estimation of Melt Contribution to Total Streamflow in River Bhagirathi and River dhauliganga at Loharinag Pala and Tapovan Vishnugad Project Sites

Many of the major rivers in India originate from the Himalayas. These rivers have significant contribution from snow and ice which makes these rivers perennial. due to steep slopes, all such streams have potential sites for hydropower generation. There is a requirement of estimation of the contribution from snow and glacier melt, rainfall contribution and sub surface contribution in the total runoff for sustainable supply of water to the hydropower plants. Considering this aspects, in this study a snowmelt runoff simulation model SNOWMOd suitable for Himalayan basins developed earlier has been modified and applied for simulation of flows. Input to the model such as glacier cover, permanent snow cover, seasonal snow cover generated through remote sensing techniques were used in conjunction with daily maximum and minimum temperature, rainfall and discharge. Two hydropower dam sites on major tributaries (Bhagirathi and dhauliganga) of River ganga have been selected for determination of different runoff components. However, though the data available was for a very limited period but the results indicate that these tributaries have significant contribution from snow and ice for long term sustainability of flows to these schemes.
