Aim:To explore whether local blockade of T-box expressed in T cells (T-bet)expression in the lungs could lead to airway ***:Twenty-fourrats were randomly divided into 4 groups:saline group,ovalbumin (OVA)-sensi-tized group,nonsense group,and the antisense *** OVA-sensitized ratswere sensitized and challenged with OVA,and the rats in the nonsense andantisense groups were subjected to an aerosol delivery of the nonsense andantisense oligonucleotides (AS-ODN) of T-bet (0.1%,w/v).The levels of inter-feron-γ(IFN-γ),interleukin-4(IL-4),and IL-5 in the bronchoalveolar lavage fluid(BALF) were detected by EliSA,and the mRNA and the protein expression ofT-bet and GATA-3 genes were examined by in situ hybridization and western blotanalysis,***:The expression of T-bet mRNA and protein in thelungs of the rats in the antisense group were inhibited *** lungs ofthe rats in the antisense and OVA-sensitized groups showed eosinophil and lym-phocyte inflammatory infiltration,and eosinophilia located predominantly aroundthe *** number of GATA-3 mRNA-positive cells and the level of GATA-3protein in the lungs of the rats in the antisense and the OVA-sensitized groupssignificantly *** level of IL-4 and IL-5 in the BALF in the antisenseand OVA-sensitized groups were elevated,but the level of IFN-γ ***:Antisense ODN-induced local blockade of T-bet expres-sion leads to airway inflammation with a selective alteration in patterns of cytokineexpression and recruitment of eosinophil cells similar to that in the OVA-sensitizedanimals.

BACKGROUND No guideline recommends antiviral therapy for hepatitis B e antigen(HBeAg)-positive chronic hepatitis B patients with persistently normal alanine aminotransferase levels and a high hepatitis B virus(HBV)DNA viral *** To evaluate the feasibility and safety of a chinese herbal formula as a therapeutic option for chronic HBV *** In total,395 patients(30–65 years old)with confirmed HBeAg-positive chronic hepatitis B infection and persistently normal alanine aminotransferase were randomized to receive either chinese herbal formula or placebo for 96 *** to evaluate therapeutic efficacy included:(1)HBV DNA levels decreased to less than 4 log10 IU/mL at weeks 48 and 96;and(2)HBeAg clearance and seroconversion rates at weeks 48 and *** HBV DNA levels≤4 log10 IU/mL were 10.05%at week 48 and 18.59%at week 96 in the treatment *** HBeAg clearance and conversion rates were 8.54%and 8.04%at week 48 and 16.08%and 14.57%at week 96,***,HBV DNA levels≤4 log10 IU/mL were 2.55%and 2.55%at weeks 48 and 96,respectively,and the HBeAg clearance rates were 3.06%and 5.61%at weeks 48 and 96,respectively,in the control *** quantitative hepatitis B surface antigen and HBeAg levels at baseline and changes during the treatment period as well as the alanine aminotransferase elevation at weeks 12 and 24 were strong predictors of HBeAg *** High rates of HBV DNA reduction,HBeAg clearance and seroconversion could be achieved with chinese herbal formula treatments,and the treatments were relatively safe for HBeAg-positive chronic hepatitis B-infected patients with persistently normal alanine *** ability of the compound to modulate host immune function probably contributed to this effect.

Background Malnutrition is common in patients with cancer,and can negatively impact their quality of life(QoL)and even ***,there is currently no large data available on the prevalence of malnutrition in chinese cancer *** study evaluated the prevalence of malnutrition and the QoL of chinese patients with locoregional,recurrent or metastatic *** We conducted a nationwide observational,multi-center,hospital-based cross-sectional study within the chinese Society of Nutritional Oncology(CSNO)*** of the patients were diagnosed with one of the following 18 different types of malignant tumors:lung cancer,gastric cancer,liver cancer,colorectal cancer,breast cancer,esophageal cancer,cervical cancer,endometrial cancer,nasopharyngeal carcinoma,malignant lymphoma,leukemia,pancreatic cancer,ovarian cancer,prostate cancer,bladder cancer,brain cancer,biliary tract malignant tumors or gastrointestinal stromal *** patients were enrolled from 72 hospitals located in different regions of *** patients’nutritional status was evaluated based on the body mass index(BMI),loss of bodyweight,laboratory measurements and patient generated-subjective global assessment(PG-SGA)*** cancer patients’physical status and QoL were assessed by the Karnofsky Performance Status(KPS)questionnaire and the European Organization for Research and Treatment of Cancer(EORTC)QLQ-C30 questionnaire,*** From December 2013 to April 2016,23,994 patients hospitalized for cancer treatment(such as surgery,chemotherapy or radiotherapy)were enrolled in the *** patients included 12,494(52.9%)males and 11,124(47.1%)*** mean age was 55.8±13.7 *** proportions of patients in cancer stagesⅠ,Ⅱ,Ⅲ,Ⅳand uncertain were 11.5%,20.3%,27.5%,30.2%and 10.5%,*** the 23,994 inpatients,the proportions of patients who were underweight(BMI5 kg/m2),normal(18.5 kg/m2

Background Malnutrition is common in patients with cancer,and this adversely affects the survival and quality of life of *** Society for Nutritional Oncology issued a multi-center,large-scale,long-term follow-up prospective study,the Investigation on Nutrition Status and Clinical Outcome of Patients with Common Cancers in china.INSCOC study)since *** is an extension to the previous 2013-2020 study *** study still sought to:①address the prognostic impact of nutritional factors and quality of life on cancer patient survival;②describe the overall and cancer-specific incidence and/or distribution of malnutrition and different measurements of patient quality of *** and study design This is an observational,multi-centered,hospital-based prospective cohort *** collection will be performed at baseline(within 48 hours after patient admission),during the hospital stay and 30 days after hospital ***-up will be conducted for 1-20 years after *** primary outcome will be the all-cause mortality/overall survival,and secondary outcomes will be the length of hospital stay and costs of *** factors will include demographic characteristics,tumor characteristics,information about chronic diseases,hematological measurements(e.g.,red blood cell count,total lymphocyte counts,hemoglobin,albumin,prealbumin,creatinine,C-reactive protein,IL-6),anthropometric measurements(e.g.,height,weight,arm circumference,arm muscle circumference,triceps skinfold thickness,and waist circumference),body composition parameters,PG-SGA scores,quality of life(as indicated by the QLQ-C30 questionnaire),muscle mass(as indicated by the calf circumference),muscle strength(as indicated by the handgrip strength),muscle function(as indicated by the six-meter walking speed test)and physical status assessments(as indicated by the Karnofsky Performance Status scores).This clinical study protocol was approved by local Ethics Committees of

基于NGF/p38 MAPK/TRPV1信号通路探讨热敏灸治疗膝骨关节炎的外周敏化效应机制

目的:观察热敏灸对膝骨关节炎(KOA)兔背根神经节NGF/p38 MAPK/TRPV1信号通路的影响,探讨其治疗KOA的作用机制。方法:36只普通级雄性新西兰兔,采用随机数表法随机分为空白组(6只)、假手术组(6只)、造模组(24只)。采用木瓜蛋白酶溶液注射膝关节腔法造模15 d,假手术组关节腔内注射等量的0.9%氯化钠溶液。造模成功后,选取6只为模型组,其余艾灸犊鼻穴,每日1次,每次40 min,共7 d,根据兔艾灸过程中温度变化分为热敏灸组和非热敏灸组,各6只;空白组、假手术组、模型组不予干预措施。干预结束后,观察各组兔行为学变化;采用EliSA法检测血清神经生长因子(NGF)含量;western blot法检测各组大鼠背根神经节p-p38 MAPK、TRPV1蛋白的表达。结果:造模后,与空白组比较,假手术组、模型组、非热敏灸组、热敏灸组兔Lequesne MG评分均显著升高(P5,P5)。与空白组比较,模型组p-p38 MAPK、TRPV1蛋白含量显著升高(P38 MAPK、TRPV1蛋白表达显著下降(P38 MAPK、TRPV1蛋白表达显著下降(P38 MAPK/TRPV1通路的激活,降低炎性局部的痛敏反应而发挥其改善KOA的作用。

严重急性呼吸综合征冠状病毒2(SARS-CoV-2)导致的新型冠状病毒肺炎(COVID-19)自暴发以来迅速在全球蔓延,造成数以千万人感染及数十万患者死亡。基于研究证据的临床实践指南是诊治COVID-19的有力武器。在疫情早期阶段,该团队制定并发布了《新型冠状病毒(2019-nCoV)感染的肺炎诊疗快速建议指南(标准版)》。疫情暴发后大量相关临床研究陆续开展并发表,新的证据可能会改变以前的推荐意见,更新指南的时机已经成熟。因此,该团队成立了由临床专家和方法学家组成的工作组,通过咨询一线临床专家提出并确定了与COVID-19管理相关的29个临床问题。通过系统检索有关COVID-19预防及诊疗的直接证据,使用GRADE方法评估证据质量并制订推荐意见,最终形成34条建议,其中6条强推荐,14条弱推荐,3条弱不推荐,11条为基于共识的建议;共涵盖4个方面,分别为药物预防,诊断(包括临床表现、RT-PCR检测、呼吸道标本、IgM和IgG抗体检测、CT检查、胸部X线片及无症状感染的CT特征),治疗方法[包括洛匹那韦-利托那韦、阿比朵尔、法匹拉韦、干扰素、瑞德西韦、抗病毒药的组合、羟氯喹/氯喹、白介素-6抑制剂、白介素-1抑制剂、糖皮质激素、清肺排毒汤、连花清瘟胶囊/颗粒、康复者血浆、肺移植、有创或无创通气、体外膜肺氧合支持(ECMO)]以及出院管理(包括复阳患者管理及出院标准)。此外,还制订了针对推荐意见的指南实施工具流程图,以期有力支持医护人员对COVID-19患者的照护。

目的:观察电针"百会""曲池""足三里"对脑缺血再灌注损伤大鼠大脑皮层区自噬的影响,探讨电针预处理改善大鼠脑缺血再灌注损伤的机制。方法:SD雄性大鼠随机分为假手术组、模型组、电针组,每组11只。电针组电针"曲池""百会""足三里",每次30 min,每天1次,连续5 d。模型组和电针组大鼠末次电针结束30 min后,采用线栓法制备大脑中动脉栓塞模型,使大鼠脑缺血1.5 h,拔栓再灌注24 h后进行神经功能缺损评分,2,3,5-三苯基氯化四氮唑染色法检测脑梗死体积,高尔基染色法检测神经元树突棘密度,电镜观察大脑皮层缺血区神经元结构及自噬小体形成情况,western blot法检测大脑皮层缺血区中微管相关蛋白1轻链3(LC3)和选择性自噬接头蛋白sequestosome-1(SQSTM1/p62)的表达水平。结果:与假手术组相比,模型组大鼠神经功能缺损评分明显升高(P3-Ⅱ/LC3-Ⅰ显著增加(P62的表达显著下调(P3-Ⅱ/LC3-Ⅰ显著降低(P62的表达显著上调(P<0.01)。结论:电针预处理可能通过抑制自噬改善脑缺血再灌注损伤。

目的探讨竹节参总皂苷对脑缺血再灌注损伤大鼠海马组织中自噬的影响。方法 40只SD雄性大鼠随机分为假手术组、模型组、竹节参总皂苷低剂量组(50 mg/kg)、竹节参总皂苷高剂量组(100 mg/kg)。提前连续给药7 d。末次给药30 min后,采用线栓法制备大脑中动脉闭塞模型使大鼠缺血1.5 h,再灌注24 h后进行神经功能评分。TTC染色检测脑梗死范围百分比,电镜观察自噬小体,western blot检测海马组织中微管相关蛋白1轻链3(LC3)、自噬底物蛋白p62的蛋白表达。结果与假手术组相比,模型组大鼠神经功能评分和脑梗死范围百分比增加(P3-Ⅱ/LC3-Ⅰ增加(P62表达下调(P3-Ⅱ/LC3-Ⅰ降低(P62表达上调(P<0.01)。结论竹节参总皂苷可能通过抑制自噬的过度激活,从而减轻脑缺血再灌注损伤的发生。
