Background:?Treatment of frail elderly patients with pancreatic cancer is still a major problem due to intolerance to standard chemotherapy doses. Aim:?This study aims to compare the low-dose gemcitabine over 6 hours (LD6H) to the standard gemcitabine protocol in terms of clinical benefit, survival, and safety in the frail elderly patients with advanced pancreatic adenocarcinoma. Methods:?Patients enrolled in this trial were randomly assigned by in a 1:1 fashion via closed envelope method to either receive gemcitabine of 1000 mg/m2?over 30-minute infusion on days 1, 8, and 15 of every 4-week cycle (standard protocol arm) or gemcitabine as a weekly low-dose (250 mg/m2) over 6-hour infusion (LD6H arm). Results:?We enrolled eighty-two eligible frail elderly patients with advanced pancreatic cancer. The patients were randomly assigned to receive either standard gemcitabine protocol (40 patients) or low-dose (250 mg/m2) gemcitabine over 6-hour infusion, given weekly (42 patients). There was no significant difference between the standard group and low-dose group as regard of the overall response rate (p = 0.654), the disease control rate (DCR) (p = 0.845), the median progression-free survival (PFS) (p = 0.908) and the overall survival (OS) (p = 0.331). The low-dose regimen had a significantly lower incidence of adverse effects grades 3 or 4 when compared to the standard regimen: (p = 0.024 for fatigue, p = 0.027 for hypotension, p = 0.012 for each anemia as well as thrombocytopenia, and p = 0.006 for neutropenia). Conclusion:?Low-dose gemcitabine over 6-hour infusion is equally effective and less toxic when compared to standard gemcitabine protocol in frail elderly patients with advanced pancreatic adenocarcinoma. So, we recommend the low-dose gemcitabine for frail elderly patients with advanced pancreatic cancer.

Prevalence of Bovine Tuberculosis in Sheep at Sadar and Parbatipur Upazila under Dinajpur District in Bangladesh

Tuberculosis is Zoonotic disease and infected animals continue to spread the disease to other animals and humans by excreting the germs in milk, meat, feces, and respiratory droplets, this study used bPPD (Bovine purified protein derivative) to observe TST (Tuberculin skin test) reactivity in the Sheep population in the Sadar and Parbatipur Upazilas of Dinajpur District. The research conducted from January to June of 2018. There is no history for sheep tuberculosis in the study area. Overall, 5 percent (7/140) of 140 sheep were positive, 15% (21/140) were doubtful, and 80% (112/140) were negative based on sex, age, breed, and body condition score, while lactation stage and reproduction status prevalence was 6.4 percent, 14.9 percent, and 78.7% positive, doubtful, and negative, respectively. Our findings suggest the presence of a skin test reaction response when employing the bPPD reagent, but this needs to be validated by pathological and serological techniques. Nevertheless, my preliminary investigation suggests that abattoirs must implement effective control systems and raise public awareness.

The First Report on Sero-Prevalence of Toxoplasma gondii in Working Horses and Donkeys in the Sudan

A serological survey of Toxoplasma gondii was conducted using LAT （Latex Agglutination Test） on 205 sera collected from working horses and donkeys in Khartoum State, Sudan. The overall sero-prevalence of the investigated equines was 32.7%. Antibodies to T. gondii were found in 38% of 100 horses and 27.6% of 105 donkeys. The titers were 1：2 （4 heads）, 1：4 （11 heads）, 1：8 （13 heads）, 1：16 （17 heads）, 1：32 （5 heads）, 1：64 （10 donkeys） and 1：128 （7 donkeys）. Neither the area nor animal species showed significant differences between the investigated groups. However, age was reported to show significant （P 〈 0.05） effect on equine toxoplasmosis in the Sudan. This is the first report on equine toxoplasmosis in the Sudan.

以普通5W-30润滑油作为基础油,按一定配比添加纳米Al_2O_3和TiO_2粉末,通过实验台架高度模拟缸套-活塞环的工作环境,研究不同载荷下纳米添加剂润滑油对缸套-活塞环摩擦副摩擦磨损特性的影响.使用二分力拉压传感器和数据采集系统测量及保存摩擦副的受力;用0.1mg精度电子天平测量摩擦前后活塞环的质量损失;用三维表面轮廓仪扫描试样表面形貌;用场发射扫描电镜对样本进行微观观察及能谱分析.实验结果表明,纳米Al_2O_3和TiO_2添加剂润滑油能够显著的改善缸套-活塞环摩擦副的摩擦磨损性能,同5W-30基础油相比,纳米Al_2O_3润滑油使摩擦副的摩擦因数和磨损率分别降低48.5%和21.6%,而纳米TiO_2润滑油则分别降低49.9%和37.6%,表面平整度及表面磨痕也相应得到改善.

Area-wide Detection of Anti-Toxoplasma gondii Antibodies in Dairy Animals from the Khartoum State, Sudan

Toxoplasmosis is an important cause of reproductive failure in man and farm animals resulting in significant socio-economic losses worldwide. The aim of this study was to avail large scale data-regarding the sero-prevalence of T. gondii infection in dairy animals from the Khartoum State, Sudan using the commercially available Latex Agglutination Diagnostic kits. The study revealed that the overall seroprevalence of T. gondii infection at herd level of dairy animals in the State was 92.7%. The within herd seroprevalence was ranging from 8% up to 100% with mean of 51.3% in different herds of different dairy animals species in the State. Only 6 herds, 3 herds, 3 herds and 1 herd of cattle, camel sheep and goat respectively were found to be clean from anti-T, gondii antibody in the present study. Interestingly, most of the clean herds （12 herds） are located in the Khartoum district. The differences between the three districts, the seven localities and the four animal species were statistically highly significant （P 〈 0.01）. The overall sero-prevalence of T. gondii infection in dairy animals--at individual level--was 45.3%. Sheep scored the highest seroprevalence rate （75.0%） followed by goats （64.0%）, camels （54.1%） and cattle （40.9%） with high statistically significant differences （P 〈 0.01）. The highest level of antibody titration was reported in sheep and goats. This is the first area-wide and large scale report on seroprevalence of T. gondii infection in dairy animals from the Sudan. It was concluded that dairy animals in the Sudan are widely exposed to T. gondii and people in the Sudan should be aware of the possibility of hyper-prevalence of human toxoplasmosis through these food animals. Research on clinical toxoplasmosis in dairy animals and human in the Sudan is recommended to evaluate the role T. gondii infection in economical losses in dairy farm industry and for building strategy of sustainable toxoplasmosis control.

Geophysical Evaluation for Evidence of Recharging the Pleistocene Aquifer at el-Nubariya Area, West Nile Delta, Egypt

With ever increasing demands on water resources, Egypt is facing increasing water needs, demanded by a rapidly growing population, by higher standards of living and by an agricultural policys. West Nile Delta represents a new agricultural area where the reclaimed lands are irrigated with groundwater in addition to surface water. The evaluation of groundwater aquifers in West Nile Delta area requires information about the extension, depth and distribution and source of recharging these aquifers. In this work, an integrated study using geoelectric, geomagnetic, hydrogeologic and hydrochemistry investigations has been applied to evaluate groundwater aquifers and to delineate subsurface structures predominant in the basement rocks at el-Nubariya area, West Nile Delta, Egypt. Thirty-five Vertical electrical Soundings (VES) have been applied to identify the subsurface geoelctric layers predominant in the study area. As well as a detailed land magnetic survey has been carried out along the study area using two automatic proton precession magnetometers with an accuracy of 1 nT. These surveys allowed us to establish the subsurface structures and their relation with the detected aquifers. The geoelectric results are illustrated along seven geoelectric profiles and correlated with the lithologic and hydrogeologic data of drilled wells. The third geoelectric layer represents the water saturated zone (aquifer) with thickness varies from 14 m to 58 m, increasing to east and southeast directions. The hydrchemisty anlysis shows that the salinity of the water aquifer ranges from 600 ppm in the southeast to 3900 ppm in the south. The major dominant dissolved salt is Nacl. In the east and southeast, the water type becomes more NaSO4 which refers to dissolution of gypsum in addition to fertilizers. The magnetic data have been analyzed using horizontal gradient and 3D magnetic modeling along the locations of geolectric profiles. The results indicate that the fault structures are trendin

Complication versus Radicality in Papillary Thyroid Cancer Surgery: How to Keep the Balance?

Background: Controversy exists over the extent of surgical treatment of papillary thyroid *** may carry a significant risk of surgical complications. The aim of this study is to investigate the complications following surgical treatment of thyroid cancer and the association between the?extent of surgery and complication rates. Methods: A total of 196 patients with papillary thyroid cancer were retrospectively reviewed to identify extent of surgery and associated complications, between 2009 and 2018 atSouth Egypt Cancer Institute Assiut University. Results: Primary procedures included less-than-total thyroidectomy (near total thyroidectomy in 5.1%, subtotal thyroidectomy in 4.1%, thyroid lobectomy in 3.1%) and total thyroidectomy in 87.8% (18.9% of them are completion thyroidectomy). No lymph nodes dissection was done in 10.7%, Berry picking in 6.1%, central compartment neck dissection in 41.8%, central compartment neck dissection with modified ipsilateral radical neck dissection in 12.2%, central compartment neck dissection with modified bilateral radical neck dissection in 26.0%, central compartment dissection with both modified bilateral radical neck dissection and superior mediastinal lymph node dissection 3.1%. The most common surgical complication were transient hypoparathyroidism (16.7% - 45.8%) and 2.0% permanent, transient vocal cord palsy (7.3% - 16.7%), and 3.9% permanent, hematoma 1.5%, seroma 7.1% and chyle fistula in 2.6% of patients. Conclusion: Thyroid cancer surgeries are safe. The transient complication rates of hypoparathyroidism increased significantly with increasing the extent of surgery.

Convergence of Discrete Adomian Method for Solving a Class of Nonlinear Fredholm Integral Equations

In recent papers the solution of nonlinear Fredholm integral equations was discussed using Adomian decomposition method (ADM). For case in which the integrals are analytically impossible, ADM can not be applied. In this paper a discretized version of the ADM is introduced and the proposed version will be called discrete Adomian decomposition method (DADM). An accelerated formula of Adomian polynomials is used in calculations. Based on this formula, a new convergence approach of ADM is introduced. Convergence approach is reliable enough to obtain an explicit formula for the maximum absolute truncated error of the Adomian’s series solution. Also, we prove that the solution of nonlinear Fredholm integral equation by DADM converges to ADM solution. Finally, some numerical examples were introduced.

Background: Hypovitaminosis D is reported through the literature to be involved in autoimmune diseases such as multiple sclerosis (MS). In the last decade, numerous studies have investigated the association of single nucleotide polymorphisms (SNPs) with MS, including rs2248359 (CYP24A1) and rs703842 (CYP27B1) that are involved in vitamin D metabolic pathway. However, results were conflicting, probably due to ethnic differences between the studied populations. In this context, the present study aimed to analyze the association between these two SNPs and MS within the Moroccan population. Methods: rs2248359 and rs703842 were genotyped in 113 patients and 146 healthy controls. To assess their association with the disease risk, we compared the genotypic and allelic frequencies between the study groups. We also explored their possible influence on certain clinical features (age at onset, type, disability status and severity score) and with vitamin D3 serum level (DSL) by comparing mean values of these variables between the different genotypes. Results: No statistically significant differences in the distribution of both SNPs were found between patients and controls. A trend has emerged concerning the minor G allele of rs703842 which appears to have a protective effect on developing MS, but this result remained slightly below significance. Also, the two polymorphisms had no impact on the clinical features tested and the DSL. Conclusion: There is no convincing evidence that rs2248359 and rs703842 are associated with MS risk, its clinical features or vitamin D level in Moroccans. Further larger investigations are needed to confirm these findings.

Risk-Adapted, Combined-Modality Therapy for Unfavorable Pediatric Hodgkin Lymphom

Background and Objectives: Risk-adapted therapy for children with HL is directed toward high survival, minimal toxicity and optimal quality of life, with long term follow up. We assess the impact of prognostic factors associated with local treatment failure of pediatric HL patients with unfavorable criteria treated with combined modality: Alternating ABVD (Doxorubicin, Bleomycin, Vinblastine and Decarbazine) and COEP (Cyclophosphamide, Oncovin, Etoposide and Prednisone) chemotherapy and response-based, involved-field radiation for newly diagnosed unfavorable pediatric HL patients, also will detect toxicities and long-term complications observed in the patients. Methods: This prospective study was carried out from January 2010 to January 2018, with a median follow up of 74 months (range 8 - 103 months). 54 patients were eligible for this study stratified into two groups: intermediate risk (IR) and high-risk group (HR). Patients were treated with (4 - 6 cycles) and (6 - 8 cycles) respectively of alternating ABVD/COEP chemotherapy followed by involved-field radiation therapy (IFRT): 15 Gy for patients achieved complete response, and 25.5 Gy for those achieved a partial response. Results: 27 patients were IR and 27 patients were HR. There were 16 treatment failures;5 patients had progressive disease;and 11 patients had a relapse. 9 patients died from their disease progression. The 5-year overall survival (OS) and event-free survival (EFS) rates (±SE) were 81.8% ± 5.7% and 71.8% ± 6.2% respectively. Multivariate analysis revealed that the only independent factor for inferior OS was radiotherapy. Conclusion: Treatment results of unfavourable HL patients in our study are satisfactory for with IR group but not for HR group who needs intensification of therapy. Radiotherapy is considered as a cornerstone in the treatment of the patients with unfavourable criteria with better assessment of early responders needed by PET-CT to identify patients at risk for relapse.
