Optimum Timing and Complication of Completion Thyroidectomy for Differentiated Thyroid Cancer

Background: Despite improved preoperative diagnostics, incidental postoperative detection of differentiated thyroid cancer in the final histology is still common. In most of these cases, completion thyroidectomy is recommended by national and international guidelines, although secondary surgery is associated with an increased operative risk. The optimal timing of completion thyroidectomy is still controversial. Patients and methods: The patients admitted to surgical oncology department, SECI, with diagnosis of differentiated thyroid cancer;during the period from January 2008 to December 2015;were rewired for age, sex, type of 1st operation, histopathological result, type of 2nd operation and time interval between the 2 operation, complication of 2nd operation and morbidity. 118 patients underwent completion thyroidectomy;those patients were divided according to timing of completion operation into 3 groups: Group A is from one week to 3 months and include 64 patients;Group B is from 3 - 6 months and include 30 patients;Group C is more than 6 months and include 24 patients. Clinical complications and oncologic outcomes were analyzed. The mean follow-up was 80 ± 10 months. Result: we record 118 patients under completion thyroidectomy. Ages range from 79 to 13 years. Papillary thyroid cancer were 96 and follicular thyroid cancer were 22. The overall rates of transient and persistent postoperative hypocalcemia were 19.5% and 4.2%, respectively. The rates of persistent hypocalcemia were found in group A and B but not in group C. Transient or persistent vocal cord paresis was observed in 9 (7.6%) and 3 patients (2.5%). The incidence of persistent vocal cord paresis (VCP) was significantly higher in groups A and B than in group C. There was no significant difference regarding survival among the 3 groups;however recurrence is higher in group A. Conclusion: Considering perioperative morbidity and oncologic outcomes, completion thyroidectomy should be performed at least 3 to 6

Outcome of patients undergoing early versus late endoscopic intervention for acute oesophageal variceal bleeding at Air Force Hospital, Egypt. A population based cohort study

Background: Whether on-call gastroenterologists should perform emergency endoscopic treatment or delay endoscopy the next day has not been investigated. This study aims to investigate the effect of timing of endoscopy on patients with acute esophageal variceal bleeding. Patients and methods: We included 167 patients with acute variceal bleeding in the gas-troenterology unit of the air force hospital, Cairo, Egypt, from 15 January till 15 July 2010. Endoscopy was scheduled once the patient’s hemodynamics permitted. Time from admission to endoscopy was recorded. Patients were stratified into 2 groups: Early endoscopy: when done ≤ 12 hours and late endoscopy: when done > 12 hours. Patients were followed up for 42 days. Results: The study included 102 males (61.1%) and 65 females (38.9%) (mean age 54 ± 11 years). 151 patients received endoscopic treatment. Early endoscopy group included 85 patients (56.3%), and the late endoscopy group included 66 patients (43.7%) (p = 0.14). Early endoscopy patients were transfused a mean of 2.9 (±1.8) units of blood versus 4.3 (±0.14) units in the late endoscopy patients p = 0.001. mean hospital stay was 7.1 (±3.3) days versus 9.3 (±4.6) days in the early and late groups respectively p: 0.001. 5-day mortality was reported in 6 (±7) cases in the early endoscopy group and 23 (±34.8) cases in the late endoscopy group p: 0.001. Conclusion: Early endoscopic intervention for patients with acute variceal bleeding is associated with a more favorable outcome in terms of morbidity and mortality. On call gastroenterologists should comply with the 12-hour period rather than delaying cirrhotic patients with acute variceal bleeding to the following day.

秘密共享是一种在一组用户间共享秘密的方式,每个用户都拿到了秘密的一个份额。当足够多的数据份额组合在一起,通过一定的组合方式,秘密就可以被重建。Shamir的(t,n)秘密共享阈值理论是一种非常有名的秘密共享范例,它提供了一种非常简洁并且有效的方式,可以在任意n个用户中选取t个参与人来共享数据。有时,一些遗失的数据可能被攻击者接截获,或者一个参与人本身就是攻击者。然而,有一种情况是,攻击者本身即使没有一个有效的份额,在多于t个参与人帮助参与数据重建的时候,Shamir秘密共享理论就不能阻止攻击者获得秘密。为解决这个问题,我们1)提出了面向组的秘密共享的概念,给出一个关于面向组的(t,m,n)-秘密共享(Group Oriented Secret Sharing,GOSS)的形式化定义。2)基于中国的剩余定理(Chinese Remainder Theorem,CRT)建立了一个(t,m,n)-GOSS 方案。它利用随机组件(Randomized Components,RC)来保护每一个秘密份额,并且确保只有所有在一个紧密耦合组中的m(m≥t)个参与者都具有有效份额并且诚实发布有效的RC情况下,秘密才能被重构。分析表明,在不依赖任何数学难题或单向函数存在性的情况下,(t,m,n)-GOSS能够确保即使攻击者拥有t-1个秘密份额或者mm-1个RC仍然无法获得秘密。

Performance of Analysis Cognitive Radio with Cooperative Sensing under malicious Attacks over Nakagami Faded Channels

The different realistic propagation channels are faced frequently the multipath fading environments. The main goal of this system design (cognitive radio network) is to improve the efficiency of spectrum access on a non-interfering basis. This system achieves high utilization for the limited spectrum in order to fulfill needs for all users’ demands which are considered as a problem in wireless communications due to rapidly increasing in wireless applications and service. This system is exposed to attack due to the vulnerabilities existence in this system. So, the main outcome of this paper is to investigate the performance of the cooperative sensing in cognitive radio networks under malicious attacks over different channel impairments, and to illustrate the most suitable individual probability of detection in real faded channel by using Nakagami model. This paper illustrates the effectiveness of the attacks and fading on the performance of spectrum sensing process.

Performance Assessment for LTE-Advanced Networks with Uniform Fractional Guard Channel over Soft Frequency Reuse Scheme

Dropping probability of handoff calls and blocking probability of new calls are two important Quality of Service (QoS) measures for LTE-Advanced networks. Applying QoS for Cell edge users in soft frequency reuse scheme in LTE system is a challenge as they already suffer from limited resources. Assigning some resources for handover calls may enhance dropping probability but this is in price of degradation in the blocking probability for new calls in cell-edge. Uniform Fractional Guard Channel (UFGC) is a call admission policy that provides QoS without reserving resources for handover calls. In this paper, the performance of Soft Frequency Reuse (SFR) in presence of Uniform Fractional Guard Channel (UFGC) will be investigated using queuing analysis. The mathematical model and performance metrics will be deduced in this assessment. The impact of UFGC will be evaluated in edge and core part separately. Then the optimal value for the parameter of UFGC will be obtained to minimize the blocking probability of new calls with the constraint on the upper bound on the dropping probability of handoff calls.

New Look on Ureterocolic Diversion with Adjuvant Radiotherapy;SECI (South Egypt Cancer Institute) Experience

Background: Ureterosigmoidostomy was the first form of continent urinary diversion and had broad popularity during the first half of the 20th century. Primary enthusiasm was followed by disappointment when serious problems such as pyelonephritis, electrolyte imbalance, renal calculi and renal function deterioration become evident. In the 1950s these life threatening complications led to avoid this urinary diversion in favor of uretro-intestinal conduits. Objectives: Our aim was to evaluate our experience in ureterocolic shunt after radical cystectomy for bladder cancer. methodology: This study was conducted in surgical oncology department, South Egypt cancer institute, Assiut university;in the period from January 2012 to January 2017 and including 115 patients (75 males & 40 females). Results: This is retrospective study including 115 patients (75 males & 40 females). Operative time was 130 minutes ranging between 2 to 2.5 hours. Average blood loss was 450 ml. After removal of rectal tube all patients were continent except 15 patients who were improved after 1 - 3 days. Within a month 12 patients had night incontinence and 5 patients had day time incontinence. After that day and night continence gradually improved in all patients except two females who had persistent night soiling and was in need for night time rectal tube. Conclusion: Ureterosigmoidostomy regains the interest of surgical oncology and urologist because of its simplicity and absence of appliance as many patients refusing cutaneous stoma and others are not suitable for orthotopic substitutes.

Analysis of Different Call Admission Control Strategies and Its Impact on the Performance of LTE-Advanced Networks

The call admission control (CAC) optimizes the use of allocated channels against offered traffic maintaining the required quality of service (QoS). Provisioning QoS to user at cell-edge is a challenge where there is limitation in cell resources due to inter-cell interference (ICI). Soft Frequency Reuse is ICI mitigation scheme that controls the distribution of resources between users. In this paper, the Impact of four CAC schemes (Cutoff Priority scheme (CP), Uniform Fractional Guard Channel (UFGC), Limited Fractional Guard Channel (LFGC), New Call Bounding (NCB) scheme) at cell-edge have investigated using queuing analysis in a comparative manner. The comparison is based on two criteria. The first criterion guarantees a particular level of service to already admitted users while trying to optimize the revenue obtained. The second criterion determines the minimum of number of radio resources that provides hard constraints in both of blocking and dropping probabilities. The four schemes are compared at different scenarios of new and handover call arrival rates.

Assessment of Remotely Sensed Digital Elevation models (DEms) Compared with DGPS Elevation Data and Its Influence on Topographic Attributes

Accuracy and quality of DEm are of a great interest for a wide range of applications. In this study, quality of ASTER GDEm and SRTm DEms were assessed in comparison with DGPS measurements. Impact of DEm resolution upon the accuracy of terrain representation and topographic attributes was also discussed. The study deduced that vertical error has a strong effect on error propagation and this highly obvious in higher elevations as the absolute standard error (SE) ranges between is ±0 - 2.5 and ±0 - 2.4 m for ASTER GDEm and SRTm respectively. This is reflected on slope and aspect as the vertical errors increase and uncertainty is relatively high in flat and low areas. Error propagation in low lands influenced drainage extraction and resulted in isolated and truncated water courses.

Nutritional Quality of Biscuit Supplemented with Wheat Bran and Date Palm Fruits (<i>Phoenix dactylifera</i>L.)

Fruits of date palm (Phoenix dactylifera L.) are consumed throughout the world and are a vital component of the diet in most Arabian countries. The effect of partial replacement of wheat flour with a 1:1 mixture of wheat bran and date palm fruit powder on rheological properties of the biscuit dough was studied. Levels of mixture used were 10%, 20%, 30% and 40%. Dough rheology (farinograph, extensograph properties) and biscuit quality (physical properties, colour, sensory evaluation) were assessed. Results obtained indicated that water absorption gradually increased by increasing the levels of wheat bran: palm date powder, meanwhile mixing tolerance index decreased. Dough stability, which indicates the dough strength, also found to decrease by increasing mixture of wheat bran and date powder (1:1). Furthermore, dough development time gradually increased by increasing the mixture at all levels. It was 6.04 min at level of 40% compared to the control (2.5 min). Extensograph results showed that dough energy and dough resistance to extension and proportional number also increased compared with control sample. The proportional number (R/E) ratios increased largely from 3.20 to 5.27 at the level of 40.0%. Ash and fibre contents gradually increased as the mixture levels increased. However, mineral content progressively increased by increasing the level of date powder in the mixture. Incorporation of wheat bran and date powder mixture decreased the spread of the biscuits from 55.66 to 52.82 mm without change in the thickness of the biscuits. Organoleptic properties revealed that quality of biscuits was acceptable at mixture level of 30%.

Energy Efficiency Behavior in Heterogeneous Networks under Various Operating Situations of Cognitive Small Cells

Recently, several approaches were followed for the enhancement and better resource utilization in mobile networks;this is to achieve energy efficient consumption for production and delivery of an information bit. Using Cognitive Femto cells (as a member of the small base stations’ family) proves that, it is an efficient solution for achieving this goal[1]. The use of Energy Efficiency term η has become one of the major indices for measuring the performance of these systems. η is the measure of the overall system Capacity (C)?in bps/Hz versus the Consumed Energy (E) in Joules [2]. In consistence with many researches, analytic models and empirical measurements, η will be investigated throughout the course of this work. Cognitive Base Stations (CBS) (as an element of the system model) which performs the traffic offloading operations is proved to enhance η performance. In this work, a combination of both analytic and simulation models are used to construct a practical system model. The obtained model is then used to illustrate the effect of different operational parameters that are involved in the η problem. On the other hand, the current paper tries to focus on the selection criteria that may be used to design the cooperative cognitive networks in order to achieve the best η indices. Both of CBSs radii as well as the inter-separation distances (between CBSs and mBS location) are examined to obtain best η index for different operation scenarios;in addition, both of capacity and energy consumption are taken into consideration based on practical operating measures. This work proposed several nonlinear equations with fixed parameters to be used by field engineers to achieve the results with minimum reduced computation complexity. So, the current work may be of importance for the regulator bodies as well as the cognitive mobile operators.
