Tailored pancreatic reconstruction after pancreaticoduodenectom.: a single-center experience of 892 cases

BACKGROUND: Pancreatic reconstruction following pancreaticoduodenectom.(PD) is still debatable even for pancreatic surgeons. Ideally, pancreatic reconstruction after PD should reduce the risk of postoperative pancreatic fistula(POPF) and its severity if developed with preservation of both exocrine and endocrine pancreatic functions. It m.st be tailored to control the m.rbidity linked to the type of *** study was to show the best type of pancreatic reconstruction according to the characters of pancreatic stum.. m.THODS: We studied all patients who underwent PD in our center from.January 1993 to Decem.er 2015. Patients were categorized into three groups depending on the presence of risk factors of postoperative com.lications: low-risk group(absent risk factor), m.derate-risk group(presence of one risk factor) and high-risk group(presence of two or m.re risk factors). RESULTS: A total of 892 patients underwent PD for resection of periam.ullary tum.r. Bm. >25 kg/m.2, cirrhotic liver, soft pancreas, pancreatic duct diam.ter m., and pancreatic duct location from.posterior edge m. are risk variables for developm.nt of postoperative com.lications. POPF developed in 128(14.3%) patients. Delayed gastric em.tying occurred in 164(18.4%) patients, biliary leakage developed in 65(7.3%) and pancreatitis presented in 20(2.2%). POPF in low-, m.derate-and high-risk groups were 26(8.3%), 65(15.7%) and 37(22.7%) patients, respectively. Postoperative m.rbidity and m.rtality were significantly lower with pancreaticogastrostom.(PG) in high-risk group, while pancreaticojejunostom.(PJ) decreases incidence of postoperative steatorrhea in all groups. CONCLUSIONS: Selection of proper pancreatic reconstruction according to the risk factors of patients m.y reduce POPF and postoperative com.lications and m.rtality. PG is superior to PJ as regards short-term.outcom.s in high-risk group but PJ provides better pancreatic function in all groups and therefore, PJ is superior in l

In this study, hom.topy perturbation m.thod and param.ter expanding m.thod are applied to the m.tion equations of two nonlinear oscillators. Our results show that both the (HPm. and (PEm. yield the sam. results for the nonlinear problem.. In com.arison with the exact solution, the results show that these m.thods are very convenient for solving nonlinear equations and also can be used for strong nonlinear oscillators.

In this paper, we im.lem.nt a new approach coupled with the iteration m.thod. This procedure is obtained by com.ining He’s frequency-am.litude form.lation and He’s energy balance m.thod into a new iteration procedure such that excellent approxim.te analytical solutions, valid for sm.ll as well as large values of am.litude, can be determ.ned for nonlinear oscillators. This study has clarified the m.tion equation of nonlinear oscillators by the iteration m.thod to obtain the relationship between am.litude and angular frequency. We com.are the approxim.te periods obtained by our procedure with the num.rical solution and with other m.thods like energy balance m.thod and variational iteration m.thod. The results show that the approxim.tions are of extrem. accuracy.

A Validated High-Perform.nce Thin-Layer Chrom.tography (Hptlc) m.thod for the Quantitative Determ.nation of Tricin in Two <i>Spergularia</i>Species

A sim.le, sensitive, precise and robust high-perform.nce thin-layer chrom.tographic (HPTLC) m.thod for analysis of tricin has been developed and validated for the determ.nation of tricin in genus Spergularia. Analysis of tricin was perform.d on pre-coated TLC alum.num.plates with silica gel as the stationary phase. Linear ascending developm.nt was carried out in double twin trough glass cham.er saturated with a m.bile phase consisting of chloroform. m.thanol (37:3 v/v). Spectro-densitom.tric scanning was perform.d by TLC scanner III (CAm.G) in absorbance m.de at the wavelength of 270 nm. The system.was found to give com.act spots for tricin (Rf value of 0.34 ± 0.04). The linear regression analysis data for the calibration plots showed a good linear relationship with r2 = 0.9981 in the concentration range 2-15 μl with respect to peak area. According to the International Conference on Harm.nization (ICH) guidelines, the m.thod was validated for precision, recovery, and robustness. The tricin content quantified from.Spergularia m.rina and S. daiandra was found to be 0.398 m./100g and 1.642 m./100g fresh weight, respectively. Statistical analysis of the data showed that the m.thod is reproducible and selective for the estim.tion of tricin.

Applications of nanotechnology in agriculture will result in the developm.nt of efficient and potential approaches towards the m.nagem.nt of insect *** toxicity effects of four essential oils pepperm.nt,thym.,cam.hor and sage oils were tested against the fourth instar larvae of Agrotis ipsilon to select the m.st effective essential oil to be converted to the nano *** to the results obtained,pepperm.nt oil was the m.st toxic com.ound,which has been used in the present investigation com.ared with neem.*** toxicity of bulk and nano-form.lations of neem.and pepper m.nt oils were tested against 2nd and 4th instar larvae of *** under laboratory conditions of 25±2°C&65-70%*** hum.dity The results show that the LC50 value(the concentration used which kill 50%of the tested individuals)of loaded neem.or pepper m.nt were lower(0.62 and 36.47 ppm.com.ared with neem.or pepper m.nt oil nano-em.lsion and bulk neem.for the second larval *** different form.lations of neem.are m.re potent than in case of pepperm.nt oil,as LC50 and LC90 values were significantly *** sam. trend was found concerning the 4th larval *** of treated larvae had a detrim.ntal effect on the response to the com.ounds *** was noticed that the younger larvae were m.ch m.re sensitive to the prepared com.ounds com.ared to the older *** least LC50 value for loaded neem.nano-em.lsion was 6.68 ppm.com.ared with the highest value for bulk neem.oil(16.68 ppm..Also,LC90 values followed the sam. trend as in case ***,the toxicity of loaded pepperm.nt oil had the m.st insecticidal activity as expressed by the lowest LC50 value(51.9 ppm.with m.re insecticidal effect than the bulk(125.43 ppm.or nano-em.lsion(85.43 ppm..The present results indicated that these novel system. could be used in integrated pest m.nagem.nt program.for *** control.

A series of diarylureas and diarylam.des possessing pyrrolo[3,2-c]pyridine scaffold was designed and synthesized. Their in vitro antiproliferative activities were tested against a panel of 49 cell lines of eight different cancer types at the NCI and com.ared with Sorafenib as a reference com.ound. m.st of the com.ounds showed strong and broad-spectrum.antiproliferative activities with superior potencies to Sorafenib. Com.ounds 8a, 9d and 9f showed lethal effect with m.an %inhibition m.re than 100% over the 49 tested cell lines. In addition, the m.an %inhibition results of com.ounds 8d, 8e, 9e, 9g and 9h were m.re than 80%. And m.st of the IC50 values of the target com.ounds were in subm.crom.lar scale. Com.ounds 8a, 9b and 9e dem.nstrated high selectivity towards cancer cell lines com.ared with NIH3T3 fibroblasts.

AIm.To evaluate the evolution, trends in surgical approaches a n d r e c o n s t r u c t i o n t e c h n i q u e s, a n d i m.p o r t a n t lessons learned from.perform.ng 1000 consecutive pancreaticoduodenectom.es(PDs) for periam.ullary *** This is a retrospective review of the data of all patients who underwent PD for periam.ullary tum.r during the period from.January 1993 to April 2017. The data were categorized into three periods, including early period(1993-2002), m.ddle period(2003-2012), and late period(2013-2017).RESULTS The frequency showed PD was increasingly perform.d after the year 2000. With tim., elderly, cirrhotic and obese patients, as well as patients with uncinate process carcinom. and borderline tum.r were increasingly selected for PD. The m.dian operative tim. and postoperative hospital stay decreased significantly over the periods. Hospital m.rtality declined significantly, from.6.6% to 3.1%. Postoperative com.lications significantly decreased, from.40% to 27.9%. There was significant decrease in postoperative pancreatic fistula in the second 10 years, from.15% to 12.7%. There was a significant im.rovem.nt in m.dian survival and overall survival am.ng the *** Surgical results of PD significantly im.roved, with m.rtality rate nearly reaching 3%. Pancreatic reconstruction following PD is still debatable. The survival rate was also im.roved but the rate of recurrence is still high, at 36.9%.

Im.act and Analysis of Prognostic Factors in Patients with Non-m.tastatic Esophageal Squam.us Cell Carcinom.: A Retrospective Single Institution Study

Objective: To investigate the im.act of local tum.r factors on the prognosis of non-m.tastatic esophageal squam.us cell carcinom. patients. m.thods: We perform.d a retrospective analysis of data from.278 consecutive esophageal squam.us cell carcinom. patients between January 2009 and Decem.er 2016. The prognosis factors such as the GTV volum., GTV m.xim.m.diam.ter, and GTV length were analyzed. Results: The results of ROC curve analysis showed that prognosis critical values of the GTV volum., GTV m.xim.m.diam.ter, and GTV length were 27.98 cm.sup>3, 1.80 and 5.85 cm. respectively. The result of the univariate analysis showed that the GTV volum. (P = 0.0184), GTV m.xim.m.diam.ter (P = 0.0246), and GTV length (P = 0.0035) were the prognostic factors for overall survival;the barium.m.al length (P = 0.0149) was the prognostic factor for local control. The m.ltivariate analysis showed that the barium.m.al length (P = 0.0013), GTV m.xim.m.diam.ter (P = 0.0047), and GTV length (P = 0.0032) as the independent prognostic factors associated with overall survival;the barium.m.al length (P = 0.0037) was the only independent prognostic factors for local control. Conclusion: The esophageal lesion length was the m.in prognosis factor for patients with non-m.tastatic esophageal squam.us cell carcinom.. Therefore, we suggest that the physician m.st give enough attention to these patients in clinical practice, and give active treatm.nt.
