To the Editor,Sarcoidosis is a disease known for over a century and remains enigmatic in etiology and pathophysiology.1 While the pulmonary manifestation prevails in more than 90%of cases,sarcoidosis can affect any ***,it targets the eyes,skin,lymph nodes,and,in rare instances,the liver and spleen.2 The development of the disease is significantly influenced by genetic and environmental factors,as well as gene variations on chromosome ***,first-degree relatives of patients have a heightened risk factor of *** adverse prognostic factors involve being of Black race,female gender,and having a lower *** increased incidence of sarcoidosis among firefighters who responded to the 9/11 disaster highlighted environmental triggers,including agricultural employment,metalworking,firefighting,exposure to inorganic and silica dust,and handling building supplies.1 Clinical symptoms can vary widely depending on the affected *** often relies on characteristic chest radiography findings,but approximately 10%of patients show noncaseating granulomas on biopsy without pulmonary abnormalities.2 Herein,we present a unique case of multiorgan sarcoidosis,where the sole manifestations were severe thrombocytopenia,weight loss,and a history of cerebral vascular events.

Renal elimination of organic anions in cholestasis

The disposition of most drugs is highly dependent on specialized transporters. OAT1 and OAT3 are two organic anion transporters expressed in the basolateral membrane of renal proximal tubule cells, identified as contributors to xenobiotic and endogenous organic anion secretion. It is well known that cholestasis may cause renal damage. Impairment of kidney function produces modifications in the renal elimination of drugs. Recent studies have demonstrated that the renal abundance of OAT1 and OAT3 plays an important role in the renal elimination of organic anions in the presence of extrahepatic cholestasis. Time elapsed after obstructive cholestasis has an important impact on the regulation of both types of organic anion transporters. The renal expression of OAT1 and OAT3 should be taken into account in order to improve pharmacotherapeutic efficacy and to prevent drug toxicity during the onset of this hepatic disease.

Hepatocellular carcinoma and multidrug resistance: Past, present and new challenges for therapy improvement

Hepatocellular carcinoma(HCC) is the most frequent form of liver cancer and the third most common cause of cancer-related death in the world. The main risk factor worldwide for this type of malignancy is chronic hepatitis caused by hepatitis B virus and hepatitis C virus infections. Advances in early detection and treatmenthave improved life expectancy of patients with HCC. However, this disorder remains as a disease with poor prognosis. In fact, epidemiological studies have revealed that there is an 8-mo median survival rate in patients, approximately 20% of whom survive one year while only 5% remain alive after three years. Additionally, HCC is particularly difficult to treat because of its high recurrence rate, and its resistance to conventional chemotherapy is due, among other mechanisms, to several members of the ATP-Binding Cassette protein family involved in drug transport being overexpressed. Fortunately, there is evidence that these patients may benefit from alternative molecular-targeted therapies. This manuscript intends to provide further insight into the etiology and molecular mechanisms related to HCC development and the latest therapeutic approaches to treat this malignancy. The development of effective delivery systems of antitumor drugs able to target the liver parenchyma is also assessed. Finally, the prospects in the development of more efficient drug therapies to overcome multidrug resistance are also examined.

海洋岛屿因其丰富的生物多样性著称。而在人类聚居的岛屿中,生态系统不断变化、内部元素有机融合,恰似生态环境与人类社会相互依存的实验基地。海洋岛屿的生态系统正遭受巨大压力:生境破碎化严重,自然保护区和人类聚居地之间的空间连通性不强。本研究方法的核心是基于城市生态网络,重新构建海洋岛屿网络系统的框架,解决海洋岛屿的连通性问题。通过在岛屿的社会生态系统中构建有形的连接系统,这一全新的城市生态规划工具有助于促进社会、生态系统间的融合和平衡。基于空间关系和功能关系、生态功能和城市结构,以完善岛屿网络系统内的区域组合,这种思路有助于推动海洋岛屿中自然系统与人类系统的有效融合,但尚未得到深入研究。本论文以科隆群岛的圣克鲁斯岛为研究案例,聚焦鹈鹕湾分水岭以内区域,对海洋岛屿的建筑区域和自然区域之间的景观连通性加以分析。本文所提及的空间网络元素均基于“土地元素是动态的”这一概念。本文希望可以通过在社会生态网络中构建全新的景观系统策略,进而明确空间元素的相互依存关系。此外,针对海洋岛屿,构建一个全新的土地使用网络的概念既有价值,也有必要。这一概念可能有助于生成实现环境平衡、连通的有形结构,既能满足社会需求,又有助于生态建设,继而有效推动城市生态保护和发展。

Neurotoxicity of the pesticide rotenone on neuronal polarization: a mechanistic approach

Neurons are the most extensive and polarized cells that display a unique single long axon and multiple dendrites, which are compartments exhibiting structural and functional differences. Polarity occurs early in neuronal development and it is maintained by complex subcellular mechanisms throughout cell life. A well-defined and controlled spatio-temporal program of cellular and molecular events strictly regulates the formation of the axon and dendrites from a non-polarized cell. This event is critical for an adequate neuronal wiring and therefore for the normal functioning of the nervous system. Neuronal polarity is very sensitive to the harmful effects of different factors present in the environment. In this regard, rotenone is a crystalline, colorless and odorless isoflavone used as insecticide, piscicide and broad spectrum pesticide commonly used earlier in agriculture. In the present review we will summarize the toxicity mechanism caused by this pesticide in different neuronal cell types, focusing on a particular biological mechanism whereby rotenone could impair neuronal polarization in cultured hippocampal neurons. Recent advances suggest that the inhibition of axonogenesis produced by rotenone could be related with its effect on microtubule dynamics, the actin cytoskeleton and their regulatory pathways, particularly affecting the small RhoGTPase RhoA. Unveiling the mechanism by which rotenone produces neurotoxicity will be instrumental to understand the cellular mechanisms involved in neurodegenerative diseases influenced by this environmental pollutant, which may lead to research focused on the design of new therapeutic strategies.

Laparoscopic approach in gastrointestinal emergencies

This review focuses on the laparoscopic approach to gastrointestinal emergencies and its more recent indications. Laparoscopic surgery has a specific place in elective procedures, but that does not apply in emergency situations. In specific emergencies, there is a huge range of indications and different techniques to apply, and not all of them are equally settle. We consider that the most controversial points in minimally invasive procedures are indications in emergency situations due to technical difficulties. Some pathologies, such as oesophageal emergencies, obstruction due to colon cancer, abdominal hernias or incarcerated postsurgical hernias, are nearly always resolved by conventional surgery, that is, an open approach due to limited intraabdominal cavity space or due to the vulnerability of the bowel. These technical problems have been solved in many diseases, such as for perforated peptic ulcer or acute appendectomy for which a laparoscopic approach has become a wellknown and globally supported procedure. On the other hand, endoscopic procedures have acquired further indications, relegating surgical solutions to a second place; this happens in cholangitis or pancreatic abscess drainage. This endoluminal approach avoids the need for laparoscopic development in these diseases. Nevertheless, new instruments and new technologies could extend the laparoscopic approach to a broader array of potentials procedures. There remains, however, a long way to go.

Stainless steel is a basic raw material used in many *** can be customized by generating laser-induced periodic surface structure(LIPSS)as subwavelength ***,we present the capabilities of an LIPSS on stainless steel to modify the polarization state of the reflected radiation at the IR *** structures have been modeled using the finite element method and fabricated by femtosecond laser *** Stokes parameters have been obtained experimentally and a model for the shape has been used to fit the simulated Stokes values to the experimental *** birefringence of the LIPSS is analyzed to explain how they modify the polarization state of the incoming *** find the geometry of the subwavelength grating that makes it work as an optical retarder that transforms a linearly polarized light into a circularly polarized *** addition,the geometrical parameters of the LIPSS are tuned to selectively absorb one of the components of the incoming light,becoming a linear axial *** selecting the geometrical parameters and orientation of the fabricated LIPSS makes it possible to obtain an arbitrary pure polarization state when illuminated by a pure linearly polarized state oriented at an azimuth of 45°.The overall reflectance of these transformations reaches values close to 60%with respect to the incident intensity,which is the same reflectivity obtained for non-nanostructured stainless steel flat surfaces.

Understanding the microscopic structure and thermodynamic properties of electrode/electrolyte interfaces is central to the rational design of electric-double-layer capacitors(EDLCs).Whereas practical applications often entail electrodes with complicated pore structures,theoretical studies are mostly restricted to EDLCs of simple geometry such as planar or slit pores ignoring the curvature effects of the electrode *** gaps exist regarding the EDLC performance and the interfacial *** the classical density functional theory(CDFT)is used to study the capacitance and interfacial behavior of spherical electric double layers within a coarse-grained *** capacitive performance is associated with electrode curvature,surface potential,and electrolyte concentration and can be correlated with a regression-tree(RT)*** combination of CDFT with machine-learning methods provides a promising quantitative framework useful for the computational screening of porous electrodes and novel electrolytes.

Concordance in the Interpretation of Cervical Cytology for the Early Diagnosis of Cervical Cancer

Objective: To identify the diagnostic concordance in the interpretation of cervical smears. Material and Methods: Cross-sectional study from October 2011 to January 2013. 50 samples were read by every 4 cyto-technologists who are the total staffs that read and interpret cytological specimens from a public health institution of the city of San Luis Potosí, México. The cytological diagnosis was confirmed by a certified pathologist. Checklist was developed to determine the quality of the interpretation composed of three sections: adequate sample, inadequate sample and cytological diagnosis. The diagnostic report was made by Bethesda System 2001 classification. Diagnostic concordance was measured through the Kappa coefficient. To establish the differences in cytological diagnosis between each cyto-technologist, chi square test was applied. Results: The concordance is acceptable in the classification of samples as inadequate (k = 0.66). The negative diagnosis was a significant concordance between each cytotechnologist and pathologist;however, no case had very good concordance. In the epithelial abnormalities, only the cytotechnologist 2 had significant concordance with the pathologist, however, its concordance is low. In the case of glandular abnormalities, only cytotechnologist 1 had no significant concordance with the pathologist. The percentage of true negatives was 30%, true positives 20%, false negatives 50% and false positives 0 %. There are significant differences between the readings of cyto-technologist 1 and 2 (p < 0.008), between 1 to 4 (p < 0.001), between 2 and 3 (p < 0.05), and finally between 3 and 4 (p < 0.003). Conclusions: The concordance between cyto-technologists and pathologists is below the required minimum level set by national and international standards.
