Radiologic-histological correlation of hepatocellular carcinoma treated via pre-liver transplant locoregional therapies

BACKGROUND:Locoregional therapies(LRTs) are treatments to achieve local control of hepatocellular carcinoma(HCC).Correlation between radiologic response to LRT and degree of induced tumor necrosis is not well understood.The aim of this study was to evaluate different levels of radiologic response after pre-liver transplant(LT) LRT and its correlation with percentage of tumor necrosis on explanted histopathology.METHODS:Institutional Review Board approved LT database was queried for treated HCC in patients undergoing LT.Radiologic response was evaluated to predict tumor necrosis in the explanted liver.Tumor response was evaluated 1 to 3 months after LRT with computed tomography or MRI via Response Evaluation Criteria in Solid Tumors(RECIST),and European Association for the Study of the Liver(EASL) guidelines.LRT was repeated as needed until time of LT.Histological tumor necrosis was graded as complete(100%),partial(50%-99%),or poor(elated with explant pathology in 73(57%) of the patients.Underestimated tumor stage was noted in 49(38%) patients,and overestimated tumor stage in 6(5%) patients.The post-LT 3-year overall survival and disease free survival were 82% and 80%,and the rates for complete and partial tumor necrosis were 100% vs 78%(P=0.02) and 100% vs 75%(P=0.03),respectively.CONCLUSIONS:In the current era,interpretation of radiologic response after LRT for HCC doe

Protective compartments are typically used to protect some specific structures from internal explosions,such as industrial buildings that contain devices that may explode in certain circumstances.This research investigates how the response of reinforced concrete(RC)compartment structures subjected to internal blast loads are affected by the following aspects:introduction of material nonlinearity in the analysis,reinforcement ratio,and aspect ratio of the compartment.To achieve this goal,a calibrated and sophisticated FE numerical model is introduced,and a parametric study for the intended aspects is carried out.A discussion of the results and conclusions are offered,which show the role of each aspect in the dynamic performance of the compartment structures.The main conclusions are as follows:introduction of material nonlinearity in this type of analysis and for these structures is very important and significant in obtaining accurate outputs that are similar to actual behavior;the reinforcement ratio has a significant effect on the response and its effect varies depending on the thickness of the compartment;in general,increasing the reinforcement ratio enhances the behavior and reduces the stresses in the compartment;and the aspect ratio of the compartment does not show a clear pattern on the response of such structures under internal blast loads.

Unified Fracture Design(UFD)bridges the gap between practices and theory in the hydraulic fracturing industry.It represents a technique to design hydraulic fracturing treatment with a particular amount of proppant.This design could provide the maximum fold-of-increase(FOI)in productivity-index(PI)after hydraulic fracturing treatment.The UFD optimization tool is very effective,but it has assumptions like any other model.One assumption of UFD optimization technique;is a single-layer assumption.This assumption does not align with the limited entry fracturing design concept.In limited entry fracturing,the frictional pressure is employed to offset the stress differences between multi-layers reservoirs to attain fluid injection through these layers,intended to deliver an optimal fracture conductivity in all layers.The drawback of this assumption is the underestimation of the actual value of FOI in PI.This paper aims to recast the original unified fracture design approach to extend the optimal UFD to a multilayer reservoir to predict the FOI in PI after limited entry fracturing treatment.The recasting tool for this problem to find the optimum solution is Artificial Neural Networks(ANN).The architected ANN model is based on actual historical data of limited entry fracturing treatments.A statistical comparison between the proposed ANN model and classical UFD technique demonstrates that ANN model solution has a more reliable estimation of FOI in PI with the actual historical data.

Background:Population-based studies on the determinants of COVID-19 seroprevalence constitute a cornerstone in guiding appropriate preventive measures.Such studies are scarce in Egypt,thus we conducted this study to explore risk factors for SARS-CoV-2 seropositivity.Methods:This survey included 2919 participants from 10 Egyptian governorates.Sera were tested for SARS-CoV-2 spike(S)and nucleocapsid(N)antibodies.Univariate and multivariate analyses were performed to identify associated factors and predictors of seropositivity regarding sociodemographic factors,clinical data,and personal practices of participants.A subgroup analysis was performed to investigate the occupational risks of seropositivity.Results:Seropositivity was recorded in 1564 participants(53.6%).Independent predictors of seropositivity in-cluded non-smokers(aOR=1.817;95%CI:1.407-2.346,p=0.000),having blood group A(aOR=1.231;95%CI:1.016-1.493,p=0.034),a history of COVID-19 infection(aOR=2.997;95%CI:2.176-4.127,p=0.000),COVID-19 vaccination(aOR=4.349;95%CI:2.798-6.759,p=0.000),higher crowding index(aOR=1.229;95%CI:1.041-1.451,p=0.015),anosmia and/or ageusia(aOR=3.453;95%CI:2.661-4.481,p=0.000)and history of fever(aOR=1.269;95%CI:1.033-1.560,p=0.023).Healthcare worker and Obesity/overweight were additional significant predictors of seropositivity among the working participants(aOR=1.760;95%CI:1.301-2.381,p=0.000 and aOR=1.384;95%CI:1.059-1.808,p=0.019,respectively).Additional factors showing association with seropositivity in the univariate analysis were:female gender,age group(15-39 years),higher educational level(preparatory and above),lack of environmental disinfection and having roommates at the workplace.There was a positive correlation between the titers of both antibodies.Age was weakly corre-lated with anti-S titer,while anti-N was significantly correlated with the number of protective measures applied by the participants.Both antibodies were significantly correlated with adult BMI,while both were significantly negatively correlated with the

In this work, a highly efficient algorithm is developed for solving the parabolic partial differential equation (PDE) with the nonlocal condition. For this purpose, we employ orthogonal Chelyshkov polynomials as the basis. The convergence analysis of the proposed scheme is derived. Numerical experiments are carried out to explain the efficiency and precision of the proposed scheme. Furthermore, the reliability of the scheme is verified by comparisons with assured existing methods.

BACKGROUND Aneurysmal subarachnoid hemorrhage is an emergency that can lead to a high mortality rate and many severe complications.It is critical to make a rapid radiological evaluation of ruptured intracranial aneurysms(RIAs)to determine the appropriate surgical treatment.AIM To assess the reliability of computed tomography angiography(CTA)in assessing different features of ruptured intracranial aneurysm and its impact on patient management.METHODS The final cohort of this study consisted of 146 patients with RIAs(75 male and 71 female)who underwent cerebral CTA.Their age ranged from 25 to 80,and the mean age±SD was 57±8.95 years.Two readers were asked to assess different features related to the aneurysm and perianeurysmal environment.Inter-observer agreement was measured using kappa statistics.Imaging data extracted from non-contrast computed tomography and CTA were considered to categorize the study population into two groups according to the recommended therapeutic approach.RESULTS The inter-observer agreement of both reviewers was excellent for the detection of aneurysms(K=0.95,P=0.001),aneurysm location(K=0.98,P=0.001),and(K=0.98,P=0.001),morphology(K=0.92,P=0.001)and margins(K=0.95,P=0.001).There was an excellent interobserver agreement for the measurement of aneurysm size(K=0.89,P=0.001),neck(K=0.85,P=0.001),and dome-to-neck ratio(K=0.98,P=0.001).There was an excellent inter-observer agreement for the detection of other aneurysm-related features such as thrombosis(K=0.82,P=0.001),calcification(K=1.0,P=0.001),bony landmark(K=0.89,P=0.001)and branch incorporation(K=0.91,P=0.001)as well as perianeurysmal findings including vasospasm(K=0.91,P=0.001),perianeurysmal cyst(K=1.0,P=0.001)and associated vascular lesions(K=0.83,P=0.001).Based on imaging features,87 patients were recommended to have endovascular treatment,while surgery was recommended in 59 patients.71.2%of the study population underwent the recommended therapy.CONCLUSION CTA is a reproducible promising diagnostic imaging modality for detecting and c

Research shows that 30 percent of banks cited customer loyalty as their biggest challenges. Thus, given that customer loyalty is completely connected to customer delight. Objective:To evaluate some agricultural waste extracts as insecticide and their effects on enzyme activities in liver and kidney of male mice.Methods:The insecticidal activity of five tested compounds(one crude extract and 4 waste compounds) was bioassay against the 3rd instars of the Culex pipiens(Cx.pipiens) larvae in the laboratory.The LC_(50) values of eucalyptol,apricot kernel.Rice bran,corn,black liquor and white liquor are 91.45.1166.1,1203.3.21449.65.4025.78 and 6343.18 ppm,respectively.Selection of the compounds for the subsequent studies was not only dependent on LC_(50) values but also on the persistence of these wastes products on large scale.Results:White and black liquor did not produce any gross effect at 200 mg/Kg body weight.No apparent toxic symptoms were observed in tested animals during the whole period of the experimenl which run out for 14 days.No statistically significance was observed in the enzyme choliuesterase activity,the activities of liver enzymes and kidney function in treated mice with black and while liquors.While,no and slighl inhibition was observed after the 2weeks ol treatment period with deltamethrin and lenilrothion reached to about 24%in plasma cholinesterase enzyme activity.Significantly increase in the activities ol liver enzymes and kidney function in treated mice with deltamethrin and fenitrothion.Conclusions:Black liquor can be used efficiently In control Cx.pipiens larvae under laboratory condition.Environmental problem caused by rice straw can be solved by converting the waste material to beneficial natural selective insecticide.

Background: The ideal agent for cervical ripening would induce adequate cervical ripening with minimal adverse effects to the mother and the fetus;the most favorable method for cervical ripening is not fully agreed till now;however, vaginal administration of isosorbide mononitrate (IMN) is considered a low-risk method of labor induction for post term. Our study was designed to assess the effect of IMN on cervical ripening and labor induction among 41 weeks pregnant women. Objectives: To assess the efficacy of the nitric oxide donor isosorbide mononitrate on cervical ripening at 41 weeks gestation. Materials and Methods: This study was conducted on 100 pregnant women recruited from the outpatient clinic fulfilling the inclusion criteria. Cases were divided into 2 groups. In first group 40 mg isosorbide mononitrate (IMN) tablet was applied vaginally in posterior fornix, and in second group placebo was applied vaginally in posterior fornix. Following up the cervical status after 24 hours of administration, the patient were asked about new symptoms especially headache, palpitation, dizziness or abdominal pain and the mode of delivery was assessed. Results: There was a significant improvement in the bishop score in the first group rather than the placebo group. No significant difference between the two groups was as regards the mode of delivery. Conclusion: IMN may be used for cervical preparation only before induction of labor in post term cases.
