Background Vitamin D(25-OHD)has a role in bone health after treatment for cancer.25-OHD deficiency has been associated with risk factors for cardiovascular disease,but no data focusing on this topic in childhood cancer survivors have been *** investigated the 25-OHD status in children treated for acute lymphoblastic leukemia(ALL),and evaluated its influence on vascular *** 25-OHD levels were evaluated in 52 ALL survivors and 40 matched healthy *** were grouped according to 25-OHD level(m or≥20 ng/ml).Auxological parameters,biochemical and hemostatic markers of endothelial function(AD,HmW-AD,ET-1,vWFAg,TAT,D-dimers,Fbg,and hs-CRP),ultrasound markers of vascular endothelial function(flow-mediated dilatation,FmD,common carotid intima-media thickness,C-ImT,and antero-posterior diameter of infra-renal abdominal aorta,APAO)were evaluated in the *** Cases showed higher prevalence of 25-OHD deficiency than controls(p=0.002).In univariate analysis via mean comparisons,25-OHD deficient(ml)patients showed higher C-ImT values compared to the 25-OHD non-deficient(≥20 ng/ml)group(P=0.023).Significant differences were also found for ET-1(P=0.035)and AD-HmW(P=0.015).In the multiple regression models controlling for some confounders,25-OHD still was associated with C-ImT(P=0.0163),ET-1(P=0.0077),and AD-HmW(P=0.0008).Conclusions Childhood ALL survivors show higher prevalence of 25-OHD deficiency as compared to *** 25-OHD levels appear to be linked to indicators of endothelial and vascular *** monitoring of 25-OHD balance may help to prevent cardiovascular diseases in childhood ALL survivors,characterized by high cardiovascular risk.

Transanal total mesorectal excision:Towards standardization of technique

AIm: To describe the role of Transanal total mesorectal excision(Ta TmE) in minimally invasive rectal cancer surgery, to examine the differences in patient selection and in reported surgical techniques and their impactson postoperative outcomes and to discuss the future of Ta TmE. mETHODS: mEDLINE(Pub med), EmBASE, and The Cochrane Library were systematically searched through the 1st of march 2015 using a predefined search strategy. RESULTS: A total of 20 studies with 323 patients were included. most studies were single-arm prospective studies with fewer than 100 patients. multiple transanal access platforms were used, and the laparoscopic approach was either multi- or single port. The procedure was initiated transanally or transabdominally. If a simultaneous approach with 2 operating surgeons was chosen, the operative time was significantly reduced. CONCLUSION: Ta TmE was also associated with better TmE specimens and a longer distal resection margin. Ta TmE is thus feasible in expert hands, but the learning curve and safety profile are not well defined. Longterm follow-up regarding anal function and oncological outcomes should be performed in the future.

Stacking transition in rhombohedral graphite

Few-layer graphene (FLG) has recently been intensively investigated for its variable electronic properties, which are defined by a local atomic arrangement. While the most natural arrangement of layers in FLG is ABA (Bernal) stacking, a metastable ABC (rhombohedral) stacking, characterized by a relatively high-energy barrier, can also occur. When both types of stacking occur in one FLG device, the arrangement results in an in-plane heterostructure with a domain wall (DW). In this paper, we present two approaches to demonstrate that the ABC stacking in FLG can be controllably and locally turned into the ABA stacking. In the first approach, we introduced Joule heating, and the transition was characterized by 2D peak Raman spectra at a submicron spatial resolution. The transition was initiated in a small region, and then the DW was controllably shifted until the entire device became ABA stacked. In the second approach, the transition was achieved by illuminating the ABC region with a train of 790-nm-wavelength laser pulses, and the transition was visualized by transmission electron microscopy in both diffraction and dark-field imaging modes. Further, using this approach, the DW was visualized at a nanoscale spatial resolution in the dark-field imaging mode.

Neuropsychiatric Systemic Lupus Erythematosus Complicated with Acute Pericarditis in Patient with Hyperprolactinaemia: A Case Report

Neuropsychiatric systemic lupus erythematosus (NP-SLE) is one of the major cause of morbidity in systemic lupus erythematosus patients and its treatment depends on identification of pathogenic mechanisms. We describe the rare case of neuropsychiatric systemic lupus erythematosus (NP-SLE) complicated by pericardial effusion combined to low C4 level persisting and hyperprolactinaemia. A cyclophosphamide therapy showed a good response in a 21-year old woman with disturbances in thought processes and an acute confusional state with sierositis. This paper confirms that a cyclophosphamide therapy contributes to control a disease activity by a mechanism of prolactin level reduction. Other studies occur to evaluate this hypothesis.

Sol-Gel Relative Humidity Sensors: Impact of Electrode Geometry on Performance in Soil Suction measurements

This paper describes the experimental procedure followed to fabricate and validate sol-gel based RH sensors which will be incorporated in soil specimens for standard laboratorial tests. It is the first time such sensors were used for soil suction measurement. They are microfabricated relative humidity sensors (footprint area 11,000 μm × 22,000 μm) operating based on changes in electrical resistivity detected by a cerium doped silica titania film deposited using a sol-gel technique. Their design required gathering experts in several engineering specialties. The working principle of the sensors is based on water vapour equilibrium between the air in the soil and in the sol-gel pores, due to the contact between the two porous materials. The spacing between interdigitated aluminium electrodes was optimized to improve the sensing properties of the sol-gel. The calibration of the different prototypes was done against compacted clay, varying the spacing between 100 and 700 μm. The sensors were also incorporated in soil samples for suction measurement during wetting and drying paths. They were validated by comparing the readings with those from a water dew point potentiometer. From this study it was possible to determine the optimum electrodes spacing of 200 μm. Error was explained by sol-gel heterogeneity effect and by the resolution of the sensing area provided by the electrodes spacing. When comparing with other sensors operating inside soil specimens in standard laboratorial tests, these sol-gel sensors extend the operation range available with the alternative technologies: while conventional tensiometers measure suction ranges from 0 to 1.8 mPa, our sensors demonstrate good results between 1 to 10 mPa (and higher).

Transperineal prostate biopsies for diagnosis of prostate cancer are well tolerated： a prospective study using patient-reported outcome measures

We aimed to determine short-term patient-reported outcomes in men having general anesthetic transperineal （TP） prostate biopsies. A prospective cohort study was performed in men having a diagnostic TP biopsy. This was done using a validated and adapted questionnaire immediately post-biopsy and at follow-up of between 7 and 14 days across three tertiary referral hospitals with a response rate of 51.6%. Immediately after biopsy 43/201 （21.4%） of men felt light-headed, syncopal, or suffered syncope. Fifty-three percent of men felt discomfort after biopsy （with 95% scoring 〈5 in a 0-10 scale）. Twelve out of 196 men （6.1%） felt pain immediately after the procedure. Despite a high incidence of symptoms （e.g., up to 75% had some hematuria, 47% suffered some pain）, it was not a moderate or serious problem for most, apart from hemoejaculate which 31 men suffered. Eleven men needed catheterization （5.5%）. There were no inpatient admissions due to complications （hematuria, sepsis）. On repeat questioning at a later time point, only 25/199 （12.6%） of men said repeat biopsy would be a significant problem despite a significant and marked reduction in erectile function after the procedure. From this study, we conclude that TP biopsy is well tolerated with similar side effect profiles and attitudes of men to repeat biopsy to men having TRUS biopsies. These data allow informed counseling of men prior to TP biopsy and a benchmark for tolerability with local anesthetic TP biopsies being developed for clinical use.

Viral infection is one of the most important causes of myocarditis both in adult and pediatric age. We describe acute myocarditis due to SARS-CoV-2 infection in an otherwise healthy 12-years-old Caucasian boy. He was admitted because of a history of 2 days of fever, dry cough, diarrhea, abdominal pain and fatigue. In spite of a family history of COVID-19 patient strict contact, a first nasopharyngeal swab for SARS-CoV-2 RNA resulted negative. After 4 days, his clinical condition evolved, with the onset of interstitial pneumonia, as documented by chest tomography and tachycardia. Blood analysis showed an NTproBNP and TnT increasing, transthoracic echocardiography revealed normal left ventricle (LV) dimension, altered myocardial texture, diffuse hypokinesis and impaired left ventricular function. A nasopharyngeal swab tested positive for SARS-CoV-2 infection at day 6 after admission. Heart failure conventional treatment associated with heparin, glucocorticoids and Remdesivir were administered with progressive improvement of clinical and instrumental conditions. No residual heart damage was documented during the following 3 monthes cardiological outpatient follow-up. Although COVID-19 infection has a milder course of disease in children than in adults, severe forms of the disease may occur and myocarditis should be carefully considered in the pediatric age.

Plants in natural ecosystems are exposed to a combination of UV radiation,ionizing radiation(IR)and other abiotic *** factors change with the *** investigated DNA alterations of some wild plants of different plant families in natural ecosystems at three altitudes in Rila mountain,Bulgaria(1500,1782,and 2925 m above sea level(a.s.l.)exposed to UV radiation,IR and other abiotic stresses,to assess the tolerance of plant species to the changing environmental conditions in three successive growth *** this purpose,physicochemical,cytogenetic,and molecular methods were *** damage was assessed by micronucleus test and molecular method comet assay adapted and applied by us to wild plant species from Onagraceae,Rosaceae,Boraginaceae,Saxifragaceae,Orobanchaceae,Asteraceae and Poaceae families,growing at three different *** in the DNA sensitivity and the level of tolerance was observed among the plant species in response to combined abiotic factors assessed by induced DNA damage and gross beta *** studied representatives of Poaceae were less susceptible than the other studied species at all three altitudes and showed close level of DNA injuries to that of unaffected control plant grown in laboratory *** lower levels of DNA damage of these wild plant species corresponded to their lower ability to accumulate *** was a particularly pronounced low level of DNA injuries in the plant species at the highest *** level of DNA damage showed correlation with the values of some abiotic environmental *** results would contribute to the elucidation of the extent of adaptation of plant species to the continuously changing environment and would be useful in selecting sensitive herbaceous monitor species for environmental impact assessment at mountain and alpine sites.

Physical Stress Echocardiography Evaluation in Athletes with Bicuspid Aortic Valves

Objective: Bicuspid Aortic Valve (BAV) is normally asymptomatic for a long time in both the general population and regularlytrained athletes. The study aimed to evaluate the role of physical stress echocardiography in early identification of any possible dissimilarities in the Left Ventricle (LV) performance and the valve functioning of asymptomatic BAV athletes as compared to the Tricuspid Aortic Valve (TAV) athletes. Design: Data were collected for BAV and TAV athletes from echocardographic examinations to evaluate any possible differences between them. Setting: Sport medicine Center University of FlorenceItaly. Participants: 66 male BAV athletes and 45 TAV athletes Assessment of risk factors: decrease in LV performance and aortic valve dysfunction during stress test. main outcome measures: measure of the standard echocardiographic parameters. Comparison of LV Ejection Fraction (EF) and Aortic Peak Flow Velocity (APFV) data in the two groups at rest and after stress. Results: At rest, values were normal in both groups, although LV systolic diameters tended to be higher in BAV. After physical effort, APFV and EF showed a significant increase in both groups, and for the former the values were at the upper limits in BAV (2.55 m/s BAV 2.12 m/s TAV). Conclusions: The data confirm normal LV performance in asymptomatic BAV athletes. Considering the tendency of this group to show slight enhancement of both APFV and LV dimensions in this group periodical evaluation with physical stress echography might be proposed as a followup procedure for early detection of any additional effect of physical exercise.
