Endoscopic ultrasound radiofrequency ablation of pancreatic insulinoma in elderly patients:Three case reports

BACKGROUND Endoscopic ultrasound(EUS)-guided radiofrequency ablation(RFA)has recently been proposed as a local treatment for functional pancreatic neuroendocrine neoplasms in patients unfit for surgery,in order to obtain clinical syndrome *** on the safety and long-term effectiveness of this approach are scarce,and EUS-RFA procedures are not *** SUMMARY The present case series reports 3 elderly patients with a pancreatic insulinoma and comorbidities,locally treated by EUS-guided RFA with clinical success in terms of hypoglycemic *** procedures were performed during deep sedation,under EUS control with a 19 G needle,an electrode 5-mm in size at a power of 30 W and multiple RFA applications during the same session in order to treat the whole area of the *** relief of symptoms was evident in 2 patients after the first EUS-RFA,while in the third patient a second endoscopic treatment was *** 3 patients are symptom-free without need of medications after 24 mo of follow-up with imaging follow-up showing no disease recurrence.A single adverse event of intraprocedural bleeding occurred,which was successfully treated *** EUS-RFA represents an effective and safe alternative to surgery for the treatment of insulinomas in elderly patients at high surgical ***,larger multicenter studies withlonger follow-up are needed in order to better assess its safety and clinical success.

Pragmatics in Second Language Learning: Current Developments

语际语用学(interlanguage pragmatics, ILP)或二语语用习得研究(L2 pragmatic acquisition)发端于1981年美国夏威夷大学gabriele Kasper教授撰写、出版的Pragmatische Aspekte in der Interimsprache。自此,第二语言习得研究开始考察学习者语用能力的培养,而语用学理论研究也有了新的实践价值。在过去的三十余年里,语际语用学研究一直是学界关注的焦点话题,刚刚落幕的第十届全国语用学研讨会见证了这一点。本栏收录了会议论文中与本主题相关的4篇论文。其中,gabriele Kasper教授的大会发言论文简要回顾了语际语用学的发展历程,并重点讨论了语际语用学研究的心理语言学视角和会话分析视角在间接言语理解方面和语用负迁移方面的应用。Caroline C. By exploiting the combined use of a heterogeneous recyclable palladium(II)-bis(N-heterocyclic carbene)catalyst and cyclopentyl methyl ether(CPME)as a convenient recoverable safer reaction medium,an effective wasteminimized approach has been developed for the intramolecular Pd-catalyzed C(sp3)-H activation of methyl pyrrole *** synthetic tools has allowed to access condensed N-heterocycles generally endowed with biological activities and representatively are the core motif of complex molecules such as Mitomycines and *** heterogeneous catalytic system could be recovered and reused up to representative five runs without any loss in *** target products(19 examples)have been obtained selectively and with excellent isolated yields up to 93%.The approach leads to the definition of a protocol with a very good E-factor(21)which is much lower(up to 98%)than those of comparable literature *** green metrics have been calculated and the data collected demonstrate that our newly developed protocol is very promising in terms of its environmental impact profile.

Probabilistic seismic response and uncertainty analysis of continuous bridges under near-fault ground motions

Performance-based seismic design can generate predictable structure damage result with given seismic ***,there are multiple sources of uncertainties in the seismic design process that can affect desired performance *** paper mainly focuses on the effects of near-fault pulse-like ground motions and the uncertainties in bridge modeling on the seismic demands of regular continuous highway *** modeling a regular continuous bridge with OpenSees software,a series of nonlinear dynamic time-history analysis of the bridge at three different site conditions under near-fault pulse-like ground motions are carried *** relationships between different Intensity Measure(IM)parameters and the Engineering Demand Parameter(EDP)are *** selecting the peak ground acceleration as the most correlated IM parameter and the drift ratio of the bridge column as the EDP parameter,a probabilistic seismic demand model is developed for near-fault earthquake ground motions for 3 different site *** this basis,the uncertainty analysis is conducted with the key sources of uncertainty during the finite element *** the results are quantified by the"swing"base on the specific distribution range of each uncertainty parameter both in near-fault and far-fault *** the ground motions are selected from PEER database,while the bridge case study is a typical regular highway bridge designed in accordance with the Chinese Guidelines for Seismic Design of Highway *** results show that PGA is a proper IM parameter for setting up a linear probabilistic seismic demand model;damping ratio,pier diameter and concrete strength are the main uncertainty parameters during bridge modeling,which should be considered both in near-fault and far-fault ground motion cases.

After short introducing the crucial role of e‐fuels to meet net‐zero emissions targets,this perspective paper discusses the differences between reactive catalysis(electro‐,photo‐and plasma‐catalysis,with focus on the first for conciseness)and thermal catalysis used at *** main point is to evidence that to progress in producing e‐fuels,the gap is not in terms of scaling‐up and pilot testing,but rather in the fundamental needs to turn the current approach and methodologies to develop reactive catalysis,including from a mechanistic perspective,to go beyond the current methods largely derived from thermal *** thus new fundamental bases to understand reactive catalysis is the challenge to accelerate the progress in this area to enable the potential role towards a sustainable net‐zero emissions *** novel aspects are highlighted,but the general aim is rather to stimulate discussion in rethinking catalysis from an alternative perspective.

Outcome of non-variceal acute upper gastrointestinal bleeding in relation to the time of endoscopy and the experience of the endoscopist: A two-year survey

AIM: To prospectively assess the impact of time of endoscopy and endoscopist's experience on the outcome of non-variceal acute upper gastrointestinal (GI) bleeding patients in a large teaching ***: All patients admitted for non-variceal acute upper GI bleeding for over a 2-year period were potentially eligible for this study. They were managed by a team of seven endoscopists on 24-h call whose experience was categorized into two levels (high and low) according to the number of endoscopic hemostatic procedures undertaken before the study. Endoscopic treatment was standardized according to Forrest classification of lesions as well as the subsequent medical therapy. Time of endoscopy was subdivided into two time periods: routine (8 a.m.-5 p.m.) and on-call (5 p.m.-8 a.m.). For each category of experience and time periods rebleeding rate, transfusion requirement, need for surgery, length of hospital stay and mortality we compared. Multivariate analysis was used to discriminate the impact of different variables on the outcomes that were ***: Study population consisted of 272 patients (mean age 67.3 years) with endoscopic stigmata of hemorrhage. The patients were equally distributed among the endoscopists, whereas only 19% of procedures were done out of working hours. Rockall score and Forrest classification at admission did not differ between time periods and degree of *** analysis showed that higher endoscopist's experience was associated with significant reduction in rebleeding rate (14% vs 37%), transfusion requirements (1.8±0.6 vs 3.0±1.7 units) as well as surgery (4% vs 10%), but not associated with the length of hospital stay nor mortality. By contrast, outcomes did not significantly differ between the two time periods of *** multivariate analysis, endoscopist's experience was independently associated with rebleeding rate and transfusion requirements. Family history of irritable bowel syndrome is the major determinant of persistent abdominal complaints in young adults with a history of pediatric recurrent abdominal pain

AIM： To assess the late outcome of teen-agers with a previous history of recurrent abdominal pain （RAP） or irritable bowel syndrome （IBS）. METHODS： A group of 67 children with RAP referred to the department from January 1986 to December 1995 was followed up between 5 and 13 years after the initial diagnosis by means of a structured telephone interview. We hypothesized that those patients with persistent adult IBS-like symptoms would be significantly more likely to report a family history oflBS in comparison with adults with no persistent abdominal complaint. RESULTS： Out of the 52 trackable subjects, 15 were found to present IBS-like symptoms at follow-up （29%） whereas the majority （37 subjects） did not. Subjects with IBS-like symptoms were almost three times more likely to present at least one sibling with similar symptoms compared to subjects not complaining （40.0% vs 16.0%）, respectively （P 〈 0.05 at Student t test）. Subjects with IBS-like symptoms also reported a higher prevalence of extra-intestinal symptoms, such as back pain, fibromyalgia, headache, fatigue and sleep disturbances. CONCLUSION： The study confirms previous observations indicating that pediatric RAP can predict later development of IBS. The latter appears to be greatly influenced by intrafamilial aggregation of symptoms, possibly through the learning of a specific illness behavior.
