Endoscopy is of low yield in the identification of gastrointestinal neoplasia in patients with dermatomyositis: A cross-sectional study

To determine the prevalence of gastrointestinal neoplasia among dermatomyositis patients who underwent an esophagogastroduodenoscopy and/or colonoscopy. METHODsA cross-sectional study examining the results of upper endoscopy and colonoscopy in adults with dermatomyositis at an urban, university hospital over a ten year period was performed. Chart review was performed to confirm the diagnosis of dermatomyositis. Findings on endoscopy were collected and statistical analyses stratified by age and presence of symptoms were performed. REsULTsAmong 373 adult patients identified through a code based search strategy, only 163 patients had dermatomyositis confirmed by chart review. Of the 47 patients who underwent upper endoscopy, two cases of Barrett’s esophagus without dysplasia were identified and there were no cases of malignancy. Of the 67 patients who underwent colonoscopy, no cases of malignancy were identified and an adenoma was identified in 15% of cases. No significant differences were identified in the yield of endoscopy when stratified by age or presence of symptoms. CONCLUsIONThe yield of endoscopy is low in patients with dermatomyositis and is likely similar to the general population; we identified no cases of malignancy. A code based search strategy is inaccurate for the diagnosis of dermatomyositis, calling into question the results of prior population-based studies. Larger studies with rigorously validated search strategies are necessary to understand the risk of gastrointestinal malignancy in patients with dermatomyositis.

Background The conversion of forests into agricultural lands can be a threat because the forests carbon stored could be a source of emissions. The capacity to improve the predictions on the consequences of land use change depends on the identification of factors that influence carbon pools. We investigated the key driving factors of tree biomass and soil carbon pools in xerophytic forests in northeastern Argentina. Based on analyses of forest structure variables and abiotic factors (topography and soil properties) from 18 mature forests, we evaluated carbon pools using uniand multivariate (redundancy analysis) methods. Results The total carbon pool was estimated at 102.4 ± 24.0 Mg ha−1. soil organic carbon storage is the single largest carbon pool relative to tree biomass, representing 73.1% of total carbon. Tree canopy cover and basal area were positively correlated with biomass carbon pool (r = 0.77 and r = 0.73, p spectively), proving to be significant drivers of carbon storage in this compartment. slope, soil clay content and cation-exchange capacity had a better explanation for the variability in soil carbon pools, and all showed significant positive correlations with soil carbon pools (r = 0.64, 0.60 and 0.50;p spectively). The vertisols showed a 27.8% higher soil carbon stock than alfisols. Conclusions The relevance of our study stems from a dearth of information on carbon pools and their drivers in xerophytic forests, and in particular, the importance of this ecosystems’ type for Argentina, because they cover 81.9% of native forest area. Basal area and tree canopy cover exert a strong effect on the carbon pool in tree biomass but not in the soil. The results suggests that there is a potentially major sOC accumulation in forests located in slightly sloping areas and soils with higher topsoil clay content, such as vertisols. This could provide an important reference for implementing forestry carbon sink projects.

Nitrification inhibitors are widely used in agriculture to mitigate nitrous oxide(N_(2)O)emission and increase crop ***,no concrete information on their mitigation of N_(2)O emission is available under soil and environmental conditions as in Pakistan.A field experiment was established using a silt clay loam soil from Peshawar,Pakistan,to study the effect of urea applied in combination with a nitrification inhibitor,nitrapyrin(2-chloro-6-tri-chloromethyl pyridine),and/or a plant growth regulator,gibberellic acid(GA_3),on N_(2)O emission and the nitrogen(N)uptake efficiency of *** experimental design was a randomized complete block with five treatments in four replicates:control with no N(CK),urea(200 kg N ha^(-1))alone,urea in combination with nitrapyrin(700 g ha^(-1)),urea in combination with GA_3(60 g ha^(-1)),and urea in combination with nitrapyrin and GA_*** N_(2)O emission,yield,N response efficiency,and total N uptake were measured during the experimental *** treatment with urea and nitrapyrin reduced total N_(2)O emission by 39%–43%and decreased yield-scaled N_(2)O emission by 47%–52%,relative to the treatment with urea *** maize plant biomass,grain yield,and total N uptake increased significantly by 23%,17%,and 15%,respectively,in the treatment with urea and nitrapyrin,relative to the treatment with urea alone,which was possibly due to N saving,lower N loss,and increased N uptake in the form of ammonium;they were further enhanced in the treatment with urea,nitrapyrin,and GA_3 by 27%,36%,and 25%,respectively,probably because of the stimulating effect of GA_3 on plant growth and development and the reduction in biotic and abiotic *** results suggest that applying urea in combination with nitrapyrin and GA_3 has the potential to mitigate N_(2)O emission,improve N response efficiency,and increase maize yield.

The novel coronavirus, sARs-CoV-2, has the potential to cause natural ventilation systems in hospital environments to be rendered inadequate, not only for workers but also for people who transit through these environments even for a limited duration. studies in of the fields of geosciences and engineering,when combined with appropriate technologies, allow for the possibility of reducing the impacts of the sARs-CoV-2 virus in the environment, including those of hospitals which are critical centers for healthcare. In this work, we build parametric 3D models to assess the possible circulation of the sARs-CoV-2 virus in the natural ventilation system of a hospital built to care infected patients during the COVID-19 pandemic. Building Information Modeling(BIM) was performed, generating 3D models of hospital environments utilizing Revit software for Autodesk CFD 2021. The evaluation considered dimensional analyses of 0°, 45°, 90° and 180°. The analysis of natural ventilation patterns on both internal and external surfaces and the distribution of windows in relation to the displacement dynamics of the sARs-CoV-2 virus through the air were considered. The results showed that in the external area of the hospital, the wind speed reached velocities up to 2.1 m/s when entering the building through open windows. In contact with the furniture, this value decreased to 0.78 m/s. In some internal isolation wards that house patients with COVID-19, areas that should be equipped with negative room pressure, air velocity was null. Our study provides insights into the possibility of sARs-CoV-2 contamination in internal hospital environments as well as external areas surrounding hospitals, both of which encounter high pedestrian traffic in cities worldwide.

Biliary complications like cholelithiasis and choledocholithiasis are more common in bariatric surgery patients due to obesity and rapid weight *** with a body mass index>40 face an eightfold risk of developing *** surgery,especially after laparoscopic Roux-en-Y gastric bypass(LRYGB),30%of patients develop biliary disease due to rapid weight *** aim of this review is to analyze the main biliary complications that occur after bariatric surgery and its management.A review of the literature was conducted mainly from 2010 up to 2023 with regard to biliary complications associated with bariatric patients in sciELO,PubMed,and *** undergoing LRYGB have a higher incidence(14.5%)of symptomatic calculi post-surgery compared to those undergoing laparoscopic sleeve gastrectomy at 4.1%.Key biliary complications within 6 to 12 months post-surgery include:Cholelithiasis:36%;Biliary colic/dyskinesia:3.86%;Acute cholecystitis:0.98%-18.1%;Chronic cholecystitis:70.2%;Choledocholithiasis:0.2%-5.7%and Pancreatitis:0.46%-9.4%.surgeons need to be aware of these complications and consider surgical treatments based on patient symptoms to enhance their quality of life.
