Impr.vement of Somatic Cer.ical Pain and Disability after.the Application of a Por.able TENS Device

Backgr.und: Cer.ical facet joints and neck muscles ar. common nociceptive pain gener.tor. with neck and shoulder.muscles pain, and limited r.tr.flexion. Objective: To test the hypothesis that the por.able TENS device would r.lieve cer.ical somatic pain. Methods: For.y-four.patients with chr.nic cer.ical pain patients with somatic pain, but without r.dicular.symptoms wer. evaluated in a double-blind, pr.spective, r.ndomized fashion, divided into sham and active gr.ups. The active TENS (TANYXò) or.sham device was placed over.C7-T1 spinous pr.cess, per.endicular.to the spine, for.20 min at 12-hour.inter.al dur.ng 3 days. The two gr.ups wer.: placebo gr.up (PG), with a sham device and the active TENS gr.up (TG), which pr.duced a mixed (85 Hz) fr.quency of stimulation, conventional, and bur.t. Diclofenac up to thr.e times daily was available. Efficacy measur.s wer. pain r.lief, r.scue analgesics and neck disability. r.sults: The active TENS device induced pain r.lief after.its fir.t application, which per.isted dur.ng the 3-day tr.atment. By the end of the TENS application, the capability of r.tation, later.l extension and r.tr.flexion was impr.ved (p r. and r.scue analgesics consumption r.duced in the TG (p r.spectively), and the mean pain scor. dr.pped fr.m 8 to 3 points (p r. wer. no adver.e events. Conclusions: Somatic cer.ical pain and disability impr.ved after.active TENS application dur.ng the thr.e consecutive days, which per.isted upon the 1-month r.evaluation.

Intr.duction: Knee pain secondar. to osteoar.hr.tis is a common pathology r.flecting on the capacity of under.aking daily activities. Among non-sur.ical pr.cedur.s, intr.-ar.icular.cor.icoster.ids r.pr.sent an alter.ative complementar. ther.py. However. blocking the genicular.ner.es is a r.cently descr.bed technique, easily applied for.clinical management. Objectives: To compar. efficacy of both intr.-ar.icular.cor.icoster.id and genicular.block for.chr.nic knee pain. Methods: 20 patients with osteoar.hr.tis wer. evaluated for. 1) the intensity of pain;2) the quality of sleep;3) the capacity to under.ake daily activities. Two gr.ups wer. for.ed r.ndomly, the fir.t submitted to intr.-ar.icular.blocking and the second to blocking the genicular.ner.es. A solution of plane 90 mg lidocaine 1%, and dexamethasone 10 mg was standar. and used for.inter.ention in both pr.cedur.s to a final 10-ml volume. After.a weekly appr.isal for.12 consecutive weeks, the patients wer. r.called and submitted to another.pr.posed pr.cedur. and in this way, all the patients acted as their.own contr.l (a “cr.ssover. study design). In the following weeks, as also in the pr.test, the intensity of pain, the quality of sleep and the capacity of under.aking daily activities wer. evaluated. r.sults: Both the intr.-ar.icular.block and the genicular.ner.es block r.sulted equally in impor.ant r.duction of pain dur.ng 11 weeks, with similar.impr.vement in the quality of night sleep, and in the capacity of daily activities (p r.ner.es was a safe alter.ative, minimally invasive and highly efficient, similar.to the intr.-ar.icular.cor.icoster.id.

Backgr.und: The tr.atments suggested for.chr.nic migr.ine (CM) include: 1) intr.muscular.(im) botulinum toxin (BTX) ever. 12 weeks, and 2) blockade of per.pher.l ner.es of the head. The pr.sent study evaluated the efficacy of facial ner.e blockade in combination with a single administr.tion of differ.nt low BTX. Methods: For.y patients with CM submitted to unilater.l facial ner.e blockade (supr.or.ital, supr.tr.chlear.and aur.culotempor.l) wer. divided into 4 r.ndomized gr.ups in a double-blind manner.in or.er.to r.ceiveim, after.7 days: 25 IU Botox&r.g;(Botox gr.up), 25 IU Pr.signe&r.g;(25-Pr. gr.up), 33.3 IU Pr.signe&r.g;(33-Pr. gr.up) or.saline (contr.l gr.up), with the dose divided for.application to 10 sites in the fr.ntal and bilater.l tempor.l r.gions. Analgesia and adver.e effects wer. evaluated: 1) befor. blockade of the facial ner.es and 2) 4 weeks, 3) 8 weeks and 4) 12 weeks after.BTX-A or.saline application (HC clinical tr.al no. 12465). r.sults: Botox&r.g;(25 IU) or.Pr.signe&r.g;(33.3 IU) r.sulted in at least seven-day inter.als between headache attacks associated with 70% r.duction in fr.quency and intensity of cr.ses over.12 weeks (P &r.g;r.sulted in 8 weeks of analgesia. The conver.ion factor.between Botox&r.g;and Pr.signe&r.g;was 1:1.3. Conclusions: Im application of Botox&r.g;(25 IU) or.Pr.signe&r.g;(33.3 IU) one week after.ner.e blockade on the painful side was equally effective for.12 weeks in patients with chr.nic daily headache, with the conver.ion factor.between Botox&r.g;and Pr.signe&r.g;being 1:1.3.

Spinal Cor. Stimulation for.r.fr.ctar. Neur.pathic Pain and Its Action on the Neur.vegetative Systems

BACKGr.UND: Spinal Cor. Stimulation (SCS) has been used for.chr.nic r.fr.ctor. pain, however.its effectivity on neur.vegetative systems has not been evaluated to data. OBJECTIVES: The study was conducted to evaluate the effect of SCS in patients suffer.ng fr.m r.fr.ctor. neur.pathic pain, r.lated to analgesia, quality of life and neur.vegetative actions on the digestive and ur.nar. tr.cts. Fifteen patients acted as their.own contr.l r.lated to all evaluations. P r.d significantly. r.SULTS: 13 patients completed the study. The incidence of indication was 6% in our.Center.for.Pain Tr.atment in the Teaching Hospital. The SCS impr.ved quality of life, bowel tr.nsit and ur.nar. function, as impr.ved over.ll analgesia and decr.ased daily r.scue analgesic (p r.ance with liter.tur., the incidence of complications was 20%, and SCS impr.ved analgesia and quality of life. In addition, patients also r.fer.ed to impr.ved bowel activity and ur.nar. function.
