Context: Atherothrombosis is the leading cause of cardiovascular morbidity and mortality around the globe. To date, no single international database has characterized the atherosclerosis risk factor profile or treatment intensity of individuals with atherothrombosis. Objective: To determine whether atherosclerosis risk factor prevalence and treatment would demonstrate comparable patterns in many countries around the world. Design, Setting, and participants: The Reduction of Atherothrombosis for Continued Health(REACH) Registry collected data on atherosclerosis risk factors and treatment. A total of 67 888 patients aged 45 years or older from 5473 physician practices in 44 countries had either established arterial disease(coronary artery disease[CAD], n=40 258; cerebrovascular disease, n=18843; peripheral arterial disease, n=8273) or 3 or more risk factors for atherothrombosis(n=12 389) between 2003 and 2004. Main Outcome Measures: Baseline prevalence of atherosclerosis risk factors, medication use, and degree of risk factor control. Results: Atherothrombotic patients throughout the world had similar risk factor profiles: a high proportion with hypertension(81.8% ), hypercholesterolemia(72.4% ), and diabetes(44.3% ). The prevalence of overweight(39.8% ), obesity(26.6% ), and morbid obesity(3.6% ) were similar in most geographic locales, but was highest in North America(overweight: 37.1% , obese: 36.5% , and morbidly obese: 5.8% ; ppatients were generally undertreated with statins(69.4% overall; range: 56.4% for cerebrovascular disease to 76.2% for CAD), antiplatelet agents(78.6% overall; range: 53.9% for ≥ 3 risk factors to 85.6% for CAD), and other evidence-based risk reduction therapies. Current tobacco use in patients with established vascular disease was substantial(14.4% ). Undertreated hypertension(50.0% with elevated blood pressure at baseline), undiagnosed hyperglycemia(4.9% ), and impaired fasting glucose(36.5% in those not known to

From single to combinatorial therapies in spinal cord injuries for structural and functional restoration

Spinal cord injury results in paralysis, sensory disturbances, sphincter dysfunction, and multiple systemic secondary conditions, most arising from autonomic dysregulation. All this produces profound negative psychosocial implications for affected people, their families, and their communities;the financial costs can be challenging for their families and health institutions. Treatments aimed at restoring the spinal cord after spinal cord injury, which have been tested in animal models or clinical trials, generally seek to counteract one or more of the secondary mechanisms of injury to limit the extent of the initial damage. Most published works on structural/functional restoration in acute and chronic spinal cord injury stages use a single type of treatment: a drug or trophic factor, transplant of a cell type, and implantation of a biomaterial. Despite the significant benefits reported in animal models, when translating these successful therapeutic strategies to humans, the result in clinical trials has been considered of little relevance because the improvement, when present, is usually insufficient. Until now, most studies designed to promote neuroprotection or regeneration at different stages after spinal cord injury have used single treatments. Considering the occurrence of various secondary mechanisms of injury in the acute and sub-acute phases of spinal cord injury, it is reasonable to speculate that more than one therapeutic agent could be required to promote structural and functional restoration of the damaged spinal cord. Treatments that combine several therapeutic agents, targeting different mechanisms of injury, which, when used as a single therapy, have shown some benefits, allow us to assume that they will have synergistic beneficial effects. Combination of hydrotreating and delayed coking technologies for conversion of residue

Residue conversion by combining catalytic hydrotreating and delayed coking has been evaluated comparatively with both processes *** operating conditions are defined to achieve the greatest economic benefit for upgrading an atmospheric residue from a heavy crude oil.A literature model was adapted to simulate the hydrotreating reactor,and for delayed coking,correlations reported in the literature were *** results with both approaches were employed to calculate the techno-economic feasibility of the combined process *** combination of hydrotreating and delayed coking presented an increase in light fractions of 29% and a reduction in coke production of 47.8%.Based on the calculated net benefit values,it was demonstrated that the combination of hydrotreating and delayed coking is technically and economically better than using each process alone,with highest benefit of 57.7 USD·m^(-3).

The use of living, alkyllithium-initiated anionic polymerization to prepare chain-end functionalized polymers and heteroarm, star-branched polymers is discussed. The scope and limitations of specific termination reactions with a variety of electrophilic species are illustrated for carbonation, hydroxyethylation, amination, and sulfonation. The methodology of using substituted 1,1-diphenylethylenes to provide a general, quantitative functionalization procedure is outlined and illustrated with examples of amine and phenol end-functionalization. A methodology is described for the synthesis of functionalized, star-branched copolymers with compositionally heterogeneous arms of controlled molecular weight and narrow molecular weight distribution using 1, 3-bis(1-pbenylethenyl) benzene.

Modeling of particle transport and combustion phenomena in a large-scale circulating fluidized bed boiler using a hybrid Euler-Lagrange approach

The constantly developing fiuidized combustion technology has become competitive with a conventional pulverized coal （pC） combustion. Circulating fluidized bed （CFB） boilers can be a good alternative to pC boilers due to their robustness and lower sensitivity to the fuel quality. However, appropriate engineering tools that can be used to model and optimize the construction and operating parameters of a CFB boiler still require development. This paper presents the application of a relatively novel hybrid Euler-Lagrange approach to model the dense gas-solid flow combined with a combustion process in a large-scale indus- trial CFB boiler. In this work, this complex flow has been resolved by applying the ANSYS FLUENT 14.0 commercial computational fluid dynamics （CFD） code. To accurately resolve the multiphase flow, the original CFD code has been extended by additional user-defined functions. These functions were used to control the boiler mass load, particle recirculation process （simplified boiler geometry）, and interphase hydrodynamic properties. This work was split into two parts. In the first part, which is referred to as pseudo combustion, the combustion process was not directly simulated. Instead, the effect of the chemi- cal reactions was simulated by modifying the density of the continuous phase so that it corresponded to the mean temperature and composition of the flue gases, In this stage, the particle transport was simu- lated using the standard Euler-Euler and novel hybrid Euler-Lagrange approaches, The obtained results were compared against measured data, and both models were compared to each other. In the second part, the numerical model was enhanced by including the chemistry and physics of combustion. Numerical approach for modeling particle transport phenomena in a closed loop of a circulating fluidized bed

Numerical modeling of a large scale circulating fiuidized bed （CFB） imposes many complexities and difficulties. presence of a dense solid phase, a variety of spatial and time scales as well as complex model geometries requires advanced numerical techniques. Moreover, the appropriate selection of a numerical model capable of solving granular flow, and geometrical model simplification can have a huge impact on the predicted flow field within the CFB boiler. In order to reduce the cost of the numerical simulations, the complex CFB boiler geometry is reduced to that of the combustion chamber. However, a question arises as to bow much one can simplify the geometrical model without losing accuracy of numerical simulations. To accurately predict the gas-solid and solid-solid mixing processes within subsequent sections of the CFB boiler （combustion chamber, solid separator, drain section）, a complete 3D geometrical model should be used. Nevertheless, because of the presence of various spatial and temporal scales within subsequent boiler sections, the complete model of the 3D CFB boiler is practically unrealizable in numerical simulations. To resolve the aforementioned problems, this paper describes a new approach that can be applied for complete boiler modeling. The proposed approach enables complex particle transport and gas flow problems within each of the boiler sections to be accurately resolved, It has been achieved by dividing the CFB boiler geometry into several submodels, where different numerical approaches can be used to resolve gas-solid transport. The interactions between computational domains were taken into account by connecting the inlets/outlets of each section using a set of user-defined functions implemented into the solution procedure. The proposed approach ensures stable and accurate solution within the separated boiler zones.

particle transport phenomena in small-scale circulating fiuidized beds （CFB） can be simulated using the Euler-Euler, discrete element method, and Euler-Lagrange approaches. In this work, a hybrid Euler-Lagrange model known as the dense discrete phase model （DDpM）, which has common roots with the multiphase particle-in-cell model, was applied in simulating particle transport within a mid-sized experimental CFB facility. Implementation of the DDpM into the commercial ANSYS Fluent CFD package is relatively young in comparison with the granular Eulerian model. For that reason, validation of the DDpM approach against experimental data is still required and is addressed in this paper. Additional difficulties encountered in modeling fluidization processes are connected with long calculation times. To reduce times, the complete boiler models are simplified to include just the combustion chamber. Such simplifications introduce errors in the predicted solid distribution in the boiler. To investigate the conse- quences of model reduction, simulations were made using the simplified and complete pilot geometries and compared with experimental data. All simulations were performed using the ANSYSFLUENT 14.0 package. A set of user defined functions were used in the hybrid DDpM and Euler-Euler approaches to recirculate solid particles.
