Background: Happy hypoxia is a new feature found in COVID-19 patients. It consists of the presence of severe hypoxemia but normal breathing rate. Failure to identify this hypoxia may have negative consequences on the survival of the patient. The objective of the present study was to measure the frequency of patients with happy hypoxia and to evaluate their survival at the Kinshasa University Hospital (KUH). Methods: This was a historical cohort of 141 hospitalized patients with COVID-19 at KUH from March 23 to June 15, 2020. Happy hypoxia was defined as oxygen saturation below 90% without dyspnea. Socio-demographic data, co-morbidities, follow up time of hospitalization and outcomes were studied. Survival was assessed using the Kaplan Meier curve. Results: Out of 141 hospitalized patients with COVID-19, 79 (56%) patients were at the severe or critical stage and 9 (6.4%) had a happy hypoxia on admission. Patients who had happy hypoxia on admission were generally older than 60 years of age (55.6%) (p = 0.023). Comparison of survival curves, based on the presence or absence of happy hypoxia, shows a statistically significant difference (p = 0.001). The presence of happy hypoxia reduces survival. Conclusion: The frequency of happy hypoxia among COVID-19 patients was low. Survival was reduced in patients with happy hypoxia. Prehospital pulse oximetry could serve as an early warning signal for the detection of happy hypoxemia in COVID-19 patients.

From single to combinatorial therapies in spinal cord injuries for structural and functional restoration

Spinal cord injury results in paralysis, sensory disturbances, sphincter dysfunction, and multiple systemic secondary conditions, most arising from autonomic dysregulation. All this produces profound negative psychosocial implications for affected people, their families, and their communities;the financial costs can be challenging for their families and health institutions. Treatments aimed at restoring the spinal cord after spinal cord injury, which have been tested in animal models or clinical trials, generally seek to counteract one or more of the secondary mechanisms of injury to limit the extent of the initial damage. Most published works on structural/functional restoration in acute and chronic spinal cord injury stages use a single type of treatment: a drug or trophic factor, transplant of a cell type, and implantation of a biomaterial. Despite the significant benefits reported in animal models, when translating these successful therapeutic strategies to humans, the result in clinical trials has been considered of little relevance because the improvement, when present, is usually insufficient. Until now, most studies designed to promote neuroprotection or regeneration at different stages after spinal cord injury have used single treatments. Considering the occurrence of various secondary mechanisms of injury in the acute and sub-acute phases of spinal cord injury, it is reasonable to speculate that more than one therapeutic agent could be required to promote structural and functional restoration of the damaged spinal cord. Treatments that combine several therapeutic agents, targeting different mechanisms of injury, which, when used as a single therapy, have shown some benefits, allow us to assume that they will have synergistic beneficial effects. In particular, we prove that any monomial semiprime k-tame fc-tame coalgebra is string. We also prove a version of Eisenbud-Griffith theorem for coalgebras, namely, any hereditary semiprime strictly quasi-finite coalgebra is serial.

A case study on the efficacy of different roof bolting schemes in Lhoist North America’s Crab Orchard Mine

Roof bolting has long been used in underground mines across the world to provide ground *** roof bolts are cheap and easy to install with the use of specialized machines as a part of the production cycle. Lhoist North America’s Crab Orchard Mine is an underground room and pillar limestone mine that uses mechanically anchored roof bolts for ground support. The mine currently employs two different roof bolting patterns: a standard 1.5 × 1.5 m pattern, and another 0.8 × 0.8 m pattern for use in areas with particularly hazardous roof conditions. The purpose of this study is to evaluate the relative effectiveness of each bolting pattern. A series of numerical models were created using Roc Science’s RS2. The models were based on a symmetrical section of the mine at its deepest point, and were modeled using generalized Hoek-Brown failure criterion along with a discrete fracture network. A series of sensitivity analyses were performed on the models by varying parameters such as joint friction angle, crack persistence, joint randomization, and tensile strength of the limestone. Based on the results of the original models and sensitivity analyses, it appeared that the standard bolting pattern provided sufficient roof support capacity under almost all the expected conditions at the mine, since safety factors below the design value of 1.5 were only found for individual bolts in a few of the worst test cases *** results can help improve the mine’s productivity and reduce operating costs without compromising safety.

Modeling behaviors of a coal pillar rib using the progressive S-shaped yield criterion

Spalling of pillar ribs has been a major hazard in the mining industry for *** the absence of rib support guidelines,accidents have continued to occur in recent *** effective support guidelines requires a complete understanding of complex pillar damage *** models represent a convenient tool for analyzing this problem,but the behavior of such models is dependent of the choice of the constitutive *** this study,a recently proposed constitutive model was used to simulate the rib fracturing process in a longwall chain pillar at West Cliff *** calibration,the model was able to capture the rib displacement profiles for multiple locations of the longwall face and the stress evolution 4 m into the *** rib bolts in the model were found to be yielding over 60% of their length under the headgate loading *** model also predicted a steady damage accumulation in the rib for certain face locations,which is consistent with the description of the rib at the *** was localized along the upper part of the pillar and underscored the role that the dirt band played in controlling rib deterioration at the *** ability of the numerical model to replicate field measurements provides confidence in the capabilities of the new constitutive ***,the need of using multi-point calibration is highlighted by comparing the results of the calibrated model to an alternative model calibrated to a smaller amount of data.
