Background: The impact of perioperative intravenous fluid administration on surgical outcomes has been documented in literature, but not specifically studied in the context of hepato-pancreato-biliary(HPB) surgery. This study aimed to investigate the impact of postoperative intravenous fluid administration on intensive care unit(ICU), in this subgroup of patients. Methods: A single-center retrospective cohort of 241 HPB patients was assessed, focusing on intravenous fluid administration in ICU, during the first 24 h. Intravenous fluid variables were compared to hospital stay and postoperative complications. Data were assessed using Spearman's correlation test for bivariate correlations and logistic regression for multivariate analysis. Results: The median volume of intravenous fluid administered in the first 24 h postoperatively was 4380 mL, of which 2200 mL was crystalloid, 1500 mL colloid and 680 mL "other" fluid. Patients with one or more complications had a higher median total intravenous fluid input(4790 vs. 4300 mL), higher colloid volume(20 0 0 vs. 150 0 mL), lower urine output(1595 vs. 1900 mL) and greater overall fluid balance( + 3040 vs. + 2553 mL) than those without complications. There were correlations between total intravenous fluid volume administered( r = 0.278, P < 0.001), intravenous colloid input( r = 0.278, P < 0.001), urine output( r =-0.295, P < 0.001), positive fluid balance( r = 0.344, P < 0.001) and length of hospital stay. Logistic regression model was constructed to predict the occurrence of one or more complications; total intravenous fluid volume and overall fluid balance were both independent significant predictors(OR = 2.463, P = 0.007; OR = 1.001, P = 0.011; respectively). Conclusions: Administration of high volumes of intravenous fluids in the first 24 hours post-HPB surgery, along with higher positive fluid balance is associated with a higher rate of complications and longer hospital stay. Effects of 7 years of field weathering on biochar recalcitrance and solubility

How weathering affects the physiochemical properties of biochar and its long-term carbon(C)sequestration potential remains *** this study,we measured changes in biochar recalcitrance and solubility after 7 years of weathering in a cultivated *** recalcitrance and biodegradability of weathered and unweathered hardwood biochar mixtures were determined by thermal analysis(differential scanning calorimetry)and evolved gas *** in biochar solubility and the chemical composition of biochar-derived dissolved organic carbon(DOC)were determined by repeated laboratory leaching and UV-Vis *** surface carbon-oxidation state(Cox)of biochar increased by 117.6-158.2%with weather-ing in the field,and there was an average of 0.9-1.2%decrease in biochar C contents per ***,thermal indices of biochar recalcitrance and biodegradability,which suggested intermediate C sequestration potential,were not significantly different between weathered and unweathered *** O:C ratio increased with weathering suggesting an increase in biodegradability,however,both weathered and unweathered biochars were still estimated to have half-lives of over 1000 ***-soluble organic carbon(WSOC)concentrations from the unweathered biochar rapidly decreased during laboratory leaching to levels similar to the field-weathered biochars,and aromaticity(SUVA_(254))of WSOC increased from 5.9%in the unweathered biochar to 42%aromaticity in the biochars weathered for 7 *** conclude that during short-term(years)weathering under field conditions,there is continued solubilization of increasingly aromatic biochar-C compounds,however,this accounts for a relatively small proportion of biochar C such that there is little-to-no change in biochar stability or C sequestration potential after field application.

The search for alternatives to traditional sources of electric energy opens the way for a new market in the world, and for Brazil in particular. Still in its first steps, but with immense potential, the generation of energy from solar irradiation and hydroelectric plants in hybrid systems is an important alternative. On the other hand, single source power systems, when designed to meet a particular demand without fail, lead to low market acceptance due to the availability of resources and low efficiency in performance that rewards high initial investment costs. One solution to balance and optimize energy supply is the use of more than one energy resource when sources can be complementary. Among several possible combinations reported in several studies, the hybrid photovoltaic hydroelectric system is considered to be an optimal and interesting combination. In this context, the present article makes a technical and economic pre-feasibility analysis of a hydroelectric photovoltaic hybrid system, operating photovoltaic panels on floating structures on the water surface to allow the use of the Laranjeiras dam. The study was conducted based on simulations with HOMER. The solution indicated as optimal was the installation of a hybrid energy system, implementing a hydroelectric power plant at the base of the dam, with 1497 kW of installed capacity, operating simultaneously with a set of photovoltaic modules, on the water surface of the dam, with 180 kW of installed capacity, and a power limit for the purchase and sale to the grid equal to 400 kW, to supply the demand of consumer loads up to 40 MWh per day. This combination would result in an initial cost of US$3984.885 per kW and an energy cost of US$0.026 per kWh.

Seasonal variability coupled with the intermittency of renewable energy sources makes reservoir hydroelectric plants an interesting option to consider in hybrid energy systems, especially in cases of dams that have not been completed or have been abandoned and which still have some potential for reuse. The Laranjeiras dam was completed in the 1960s and the original project for hydroelectric power generation was not completed, made impossible by economic changes during the construction years. A recent study proposed the implementation of a hydroelectric photovoltaic hybrid system with lower horsepower to allow the dam to be made useful again. This paper presents the results of the computational simulations with the software Homer, considering the operation of the hydroelectric component (of the proposed hybrid system) with reservoir, playing the role of energy storage device when the production exceeds the demand at a given moment, reducing the loss of energy due to unavailability of demand. The study suggested to implement a hydroelectric power plant with power house at the base of the dam that has a height of 20 m, operating at a minimum flow of 9171 L/s, with reservoir operating as a device for energy storage, operating with a photovoltaic system of 360 kW, and a power limit for the purchase of energy from the grid equal to 200 kW, providing consumer loads up to 40 MWh per day, with cost of energy equal to US$0.021 per kWh and a capital cost of US$3285.617.

Dose of Telehealth to Improve Community-Based Care for Adults Living with Multiple Chronic Conditions: A Systematic Review

The purpose of this systematic review is to identify evidence of the appropriate dose of telehealth intervention services provided to community dwelling adults experiencing chronic illness or disability related to effectiveness, quality, safety, and cost. Academic Search Complete, CINAHL, MEDLINE, Cochrane, and JBI were searched using combinations of “telehealth or telemedicine or telemonitoring or telepractice or telenursing or telecare AND chronic illness or chronic disease”. Of the identified 449 articles, 47 articles met the inclusion criteria. Most study designs were quasi-experimental one group pre-test post-test (N = 16) with few Randomized Controlled Trials (N = 12). Twenty-three published articles studied the effect of telehealth for one chronic condition (49.9%) while 24 (51.1%) examined the effectiveness of telehealth for multiple chronic conditions. Measurement of telehealth outcomes varied and included efficacy, healthcare utilization, quality, adherence, cost, and safety. No standard measure of dose could be extrapolated. Length of intervention was measured and reported differently in each study. The dose of telehealth services that improve care effectiveness, quality, safety, and cost is still unknown for community dwelling adults experiencing chronic illness. The findings from this systematic review do indicate that longer duration of telehealth services (51 weeks), regardless of modality, produced positive outcomes as opposed to those with shorter durations (37 - 38 weeks) that produced neutral or mixed results. Collecting and reporting data related to clinical workflow such as dose of intervention specific to disease and type of modality is recommended. Rigorous study design including standard measurement at the RCT and Comparative Effectiveness level is still needed.
