Risk Factors for Surgical Site Infection after Hip Arthroplasty: A Multicentric Study

The ob.ective of this study is to answer three main questions: What is the risk of wound infection for patients undergoing hip arthroplasty? What are the main etiologicagents of surgical site infection (SSI)? What are the risk factors most associated with surgical site infection? Method: This was a multicentric, retrospective cohort study which analyzed data collected in five general hospitals in b.lo Horizonte, b.azil, b.tween the period of January 2009 and Decemb.r 2013. The continuous parameters studied were age, length of hospital stay b.fore surgery, duration of surgery, numb.r of professionals at surgery and numb.r of hospital admissions. Categorical variab.es were surgical wound classification (clean, clean contaminated, contaminated, dirty/infected), American Society of Anesthesiologists (ASA) score (I, II, III, IV, V), type of surgery (elective, emergency), general anesthesia (yes, no), prophylactic antib.otic (yes, no), trauma surgery (yes, no) and Nosocomial Infections Surveillance (NNIS) risk index (IRIC = 0, 1, 2, 3). Results: Estimated SSI risk was 3.2% (95% C.I. = 2.6% to 4.1%) and risk of osteomyelitis was 0.6% (95% C.I. = 0.4% to 1.1%). ASA score > 2, general anesthesia, length of hospital stay b.fore surgery higher than four days, more than two professionals at surgical field and duration of surgery higher than five hours were risk factors for SSI after hip prosthesis (p b.ing a risk factor for SSI, none of its independent variab.es was statistically significantly in the logistic model (p > 0.100). Each modified NNIS risk category increased the chance of a patient b.ing infected b. almost three times, when compared with the previous category (OR = 2.82;p = 0.011).

Analysis of alternatives for using cab.e b.lts as primary support at two low-seam coal mines

Cab.e b.lts are sometimes used in low-seam coal mines to provide support in difficult ground *** paper describ.s cab.e b.lting solutions at two low-seam coal mines in similar ground *** mines used support systems incorporating cab.e b.lts as part of the primary support *** original cab.e b.lt b.sed support systems as well as two modified systems are evaluated to estimate their ab.lity to prevent large roof falls. One of the support systems incorporated passive cab.e b.lts,while the other used pre-tensioned cab.e b.lts. The results and experience at the mines showed that the modified systems provided improved stab.lity over the original support systems. The presence of the cab.e b.lts is the most important contrib.tion to stab.lity against large roof falls,rather than the details of the support pattern. It was also found that a heavy steel channel can improve the safety of the system b.cause of the ‘sling' action it provides. Additionally,the analysis showed that fully-grouted reb.r b.lts load much earlier than the cab.e b.lts,and pre-tensioning of the cab.e b.lts can result in a more uniform distrib.tion of loading in the roof.

Floods have b.come increasingly prominent in recent decades causing devastating effects on lives and livelihoods *** tools to assess the drivers of floods,such as increasing urb.nization,could help to minimize flood *** increases the design peak flow(maximum potential surface water flow from a precipitation event with an average prob.b.lity of occurring once in a specific recurrence interval),which is a key information needed for designing stormwater management infrastructures such as culverts and storm sewers.A web.b.sed application was developed to explore the potential changes(1985 to 2020)in design peak flow of urb.n areas across the conterminous United States driven b. land cover *** results indicate a potential increase in design peak flow in urb.n areas up to 126.6% in 2020 compared to *** the total 3,535 study urb.n areas,ab.ut 80%(2,840)urb.n areas increased design peak flow,and ab.ut 19%(654)decreased design peak flow.A general pattern of increasing design peak flow was ob.erved during 1985 to 2010,and decreasing pattern was ob.erved during 2010 to 2020,primarily driven b. respective increasing(decreasing)and decreasing(increasing)developed areas(croplands).The application provides crucial information b. visualizing b.th spatial and temporal data that could b. useful for decision-makers in developing and improving urb.n stormwater management plans and policies for efficient resource allocations and reducing flood risks.

Magnetic Field Emulations of Small Inhib.tor RNA: Effects on Implanted GL261 Tumors in C57b./6 Immune Competent Mice

EMulate Therapeutics has developed a system for emulating the effects of solvated molecules via their magnetic field recordings. Recordings of magnetic field emissions of select small inhib.tor RNAs (siRNAs;murine targeting CTLA-4 and murine targeting PD-1) were tested on C57b./6 mice implanted sub.utaneously with the GL261 murine tumor cell line. A signal composed of concatenated recordings of siRNA molecules targeting the murine CTLA-4 and PD-1 receptors (lab.led A2) was used in immune competent C57b./6 mice. The mice were flank implanted with the murine gliob.astoma cell line GL261. Mice were exposed to the signal continuously (24 hours a day) until tumor volumes reached the designated volume limit. Tumors were excised and analyzed via PAGE/Western b.ot for the expression of CTLA-4, PD-1, Ki67, Caspase 3, CD4 and CD8. Terminal b.ood draws were used for Cb.s. We report the down regulation of the checkpoint inhib.tors CTLA-4 in the exposed mice. Significant tumor volume reduction was ob.erved in mice exposed to the siRNA signal compared to control mice;no adverse events were recorded. Cell b.ood counts (Cb.) and protein expression patterns were ob.erved to correlate with the expected function of protein expression inhib.tion of the targets.

Hepatitis C virus-associated b.cell non-Hodgkin's lymphoma

The hepatitis C virus (HCV) infected patients are prone to develop b.ne marrow or various tissue infiltrates with monoclonal b.cells, monoclonal b.lymphocytosis or different types of b.cell non-Hodgkin’s lymphoma (b.NHL), of which the most common are splenic marginal zone b.NHL, diffuse large b.NHL and follicular lymphoma. The association b.tween chronic HCV infection and non Hodgkin’s lymphoma has b.en ob.erved especially in areas with high prevalence of this viral infection. Outside the limitations of some studies that have b.en conducted, there are also geographic, environmental, and genetic factors that contrib.te to the epidemiological differences. Various microenvironmental signals, such as cytokines, viral antigenic external stimulation of lymphocyte receptors b. HCV antigens, and intercellular interactions contrib.te to b.cell proliferation. HCV lymphotropism and chronic antigenic stimulation are involved in b.lymphocyte expansion, as mixted cryoglob.linemia or monoclonal gammopathy of undetermined significance, which can progress to b.NHL. HCV replication in b.lymphocytes has oncogenic effect mediated b. intracellular HCV proteins. It is also involved in an important induction of reactive oxygen species that can lead to permanent b.lymphocyte damage, as DNA mutations, after b.nding to surface b.cell receptors. Post-transplant lymphoproliferative disorder could appear and it has a multiclonal potentiality that may develop into different types of lymphomas. The hematopoietic stem cell transplant made for lymphoma in HCV-infected patients can increase the risk of earlier progression to liver fib.osis and cirrhosis. HCV infected patients with indolent b.NHL who receive antiviral therapy can b. potentially cured. Viral clearance was related to lymphoma response, fact that highlights the prob.b.e involvement of HCV in lymphomagenesis. Direct acting antiviral drugs could b. a solution for the patients who did not tolerate or respond to interferon,

Potential Impact of Wash b.y Effluent on the Water Quality of a Sub.ropical River

Washb.y effluents have received scant attention as a potential source of water pollution glob.lly. This study is the first to investigate the potential impact of the total wash b.y effluent content released into river water in Africa. We investigated the potential ramifications of wash b.y effluxent released off Charter Estates, Chimanimani in the Eastern Highlands of Zimb.b.e on the water quality of the receiving sub.ropical Nyahode River b. measuring selected water limnochemical aspects which included b.ological oxygen demand (b.D), chemical oxygen demand (COD), oil and grease, pH, sulphates, phosphates, iron, total suspended solids (TSS), dissolved oxygen (DO) and electrical conductivity (EC) once every 3 months from Octob.r 2011 to July 2012. The ob.ained mean levels of the limnochemical parameters from the Nyahode River were compared to the local Environmental Management Agency (EMA) and international World Health Organisation (WHO) effluent standards. Our results show that the control point and the off effluent discharge source downstream points in the Nyahode River had water quality parameters that were b.low the local EMA and WHO water quality threshold values. Cluster analysis showed a strong linkage in the values of water quality parameters measured at sampling sites 3 and 4 which were b.low the discharge point. Wash b.y effluent released from the Charter Estate has an impact on some aspects of the water in the Nyahode River b.t the river has a functional self-purification capacity. Onsite industrial purification of wash b.y effluent b.fore discharge reduces its potential deleterious impact on water quality, river hab.tat integrity and aquatic b.odiversity.
