Diagnosis and Treatment Outcomes of Tuberculosis in Relation to Gender and HIV STATUS in South Benin

Background: In Benin, little is known about the influence of both gender and HIV-status on diagnostic patterns and treatment outcomes of Tuber-culosis (TB) patients. Objective: To assess whether differences in gender and HIV status affect diagnostic patterns and treatment outcomes of TB patients. Methods: Retrospective cohort study of patients registered in 2013 and 2014 in the three largest TB Basic Management Units in south Benin. Results: Of 2694 registered TB patients, 1700 (63.1%) were male. Case notification rates were higher in males compared with females (96 vs 53/100,000 inhabitants). The male to female ratio was 1:1 in HIV positive patients, but was 2:1 among HIV negative cases. In HIV-positive patients, there were no differences in TB types between men and women. In HIV-negative patients, there were significantly higher proportions of females with clinically diagnosed pulmonary TB (p = 0.04) and extrapulmonary TB (p < 0.001). Retreatment TB was 4.65 times higher amongst males compared with females. For New bacteriologically confirmed pulmonary TB, no differences were observed in treatment outcomes between genders in the HIV positive group;but significantly more unfavorable outcomes were reported among HIV negative males, with higher rates of failure (p < 0.001) and loss-to-follow up (p = 0.02). Conclusion: The study has shown that overall TB notification rates were higher in males than in females in south Benin, with more females co-infected with HIV. Unfavorable outcomes were more common in HIV-negative males.

Introduction: Since the advent of antiretroviral therapy, the vital prognosis of people living with HIV (PLWHA) has improved significantly. However, the risk of metabolic complications is high, thus making the bed of cardiovascular disease. Our objective was to compare the prevalence of metabolic abnormalities among PLWHA receiving ARVs to that observed in those who are not treated. Methods: We conducted a cross-sectional study (January to April 2010) at the PLWHA ambulatory care center of national university hospital (CNHU-Hubert K. Maga) in Cotonou, Bénin. We recruited 420 PLWHA (210 treated for at least 6 months and 210 untreated). We determined the prevalence of metabolic syndrome (MS) defined by the criteria of NCEP-ATP III, and the prevalence of abnormal glucose and lipid, and lipodystrophy. Association between metabolic syndrome and ARVs used was analyzed by binomial regression. Confidence intervals were calculated at 95% and 5% alpha level. Results: The prevalence of MS was 16% (18% of patients treated vs. 13% of non-treated, p = 0.18). That of hyperglycemia was 18% (30% of patients treated vs. 6% of untreated;p ade to the initiation of antiretroviral therapy and during the therapeutic monitoring.

The location of a primary duodenal adenocarcinoma is exceptional. Only few cases have been reported in the literature. We hereby report a case of primary adenocarcinoma of the duodenum with a 60 years old patient admitted because of chronic iron deficiency anemia. Endoscopic examination revealed a budding and ulcerated process in the second duodenum. The biopsy analysis showed primary adenocarcinoma. No other primary tumor was found. The staging directed showed cephalic pancreatic invasion. The patient died in the aftermath of a surgical management.

Mammals can show conspicuous behavioral responses to thermal variation,including changes in movement *** used an integrative approach to understand how environmental temperature can drive the movement behavior of a mammal with low capacity for physiological thermoregulation,the giant anteater(Myrmecophaga tridactyla).We tracked 52 giant anteaters in 7 areas throughout the Brazilian *** estimated the distance moved,area used,use of forest areas,and mean environmental temperature for each monitoring day of each *** modeled these data with Mixed Structural Equations—considering the possible interactions between our variables and controlling for sex and body *** anteaters reduced displacement and increased forest use with decreasing environmental temperature,probably because of their low body heat *** is possible that they reduce distance moved and area used by reducing the duration of *** decreasing temperature,forest habitats become warmer than open ones,besides buffer rain and chilly *** displacement and using forests are important strategies to reduce body heat loss and the energetic costs of ***,decreasing movement can limit food access and,consequently,***,we highlight the importance of forests as thermal *** increasing frequency and intensity of extreme weather events,we showed the need to preserve forest patches to offer suitable conditions for tropical mammals’behavioral *** this context,policies favoring deforestation on Brazilian territory are especially ***,we emphasize the need of integrative approaches to understand the complex interactions between organisms and the environment.

我们在西班牙南部的一个卫生保健地区开展了一项为期1年的Ⅱ型糖尿病病人卫生资源消耗的描述性与观察性研究。总共对517名患者进行了评估,患者的平均发病时间为(9.7±8)年。每一名患者每年总成本为4278欧元,其中直接成本2504欧元,间接成本1774欧元。多元回归分析显示,总成本与以下变量之间独立相关:肥胖、性别、糖尿病相关住院次数、永久残疾、大血管并发症,以及微血管和大血管并发症同时发生。我们的研究结果一方面证实了Ⅱ型糖尿病治疗高昂的经济成本,另一方面也证实了糖尿病的疾病经济成本与肥胖、性别、糖尿病相关住院次数、永久残疾和慢性并发症等因素之间的直接关系。
