Corneal biomechanical data and biometric parameters measured with Scheimpflug-based devices on normal corneas

Striking elevation in incidence and prevalence of inflammatory bowel disease in a province of western Hungary between 1977-2001

AIM:An investigation into inflammatory bowel disease and colorectal cancer in Veszprem Province was conducted from 1977 to ***: Both hospital and outpatient records were collected and reviewed comprehensively. The majority of patients were followed up ***:The population of the province was decreased from 386000 to 376000 during the period. Five hundred sixty new cases of ulcerative colitis (UC), 212 of Crohn's disease (CD), and 40 of indeterminate colitis (IC) were diagnosed. The incidence rates increased from 1.66 to 11.01 cases per 100 000 persons for UC, from 0.41 to 4.68 for CD and from 0.26 to 0.74 for IC. The prevalence rate at the end of 2001 was 142.6 for UC and 52.9 cases per 100 000 persons for CD. The peak onset age in UC patients was between 30 and 40 years, in CD between 20 and 30 years. A family history of IBD was present in 3.4% in UC and 9.9% in CD *** increased the risk for CD (OR=1.94) while it decreased the risk for UC (OR=0.25). Twelve colorectal carcinomas were observed in this cohort, the cumulative colorectal cancer risk after 10 years in UC was 2%, after 20 years 8.8%, after 30 years 13.3%.CONCLUSION:The incidence and prevalence rates of IBD have increased steadily in Veszprem Province, now equivalent to that in Western European countries. Rapid increase in incidence rates supports a probable role for environmental factors. The rate of colorectal cancers in IBD is similar to that observed in Western countries.

Optimization and design of an aircraft's morphing wing-tip demonstrator for drag reduction at low speed, Part Ⅰ–Aerodynamic optimization using genetic, bee colony and gradient descent algorithms

In this paper, an ‘in-house＇ genetic algorithm is described and applied to an optimization problem for improving the aerodynamic performances of an aircraft wing tip through upper surface morphing. The algorithm＇s performances were studied from the convergence point of view, in accordance with design conditions. The algorithm was compared to two other optimization methods,namely the artificial bee colony and a gradient method, for two optimization objectives, and the results of the optimizations with each of the three methods were plotted on response surfaces obtained with the Monte Carlo method, to show that they were situated in the global optimum region. The optimization results for 16 wind tunnel test cases and 2 objective functions were presented. The 16 cases used for the optimizations were included in the experimental test plan for the morphing wing-tip demonstrator, and the results obtained using the displacements given by the optimizations were evaluated.

Do patients with gastroesophageal reflux disease and somatoform tendencies benefit from antireflux surgery?

BACKGROUND The clinical presentation of gastroesophageal reflux disease(GERD) shows a large symptom variation also in different intensities among patients. As several studies have shown, there is a large overlap in the symptomatic spectrum between proven GERD and other disorders such as dyspepsia, functional heartburn and/or somatoform *** To prospectively evaluate the GERD patients with and without somatoform disorders before and after laparoscopic antireflux *** In a tertiary referral center for foregut surgery over a period of 3 years patients with GERD, qualifying for the indication of laparoscopic antireflux surgery, were investigated prospectively regarding their symptomatic spectrum in order to identify GERD and associated somatoform disorders. Assessment of symptoms was performed by an instrument for the evaluation of somatoform disorders[Somatoform Symptom Index(SSI) > 17]. Quality of life was evaluated by Gastrointestinal Quality of Life Index(GIQLI).RESULTS In 123 patients an indication for laparoscopic antireflux surgery was established and in 43 patients further medical therapy was suggested. The portion of somatoform tendencies in the total patient population was 20.48%(34 patients).Patients with a positive SSI had a preoperative GIQLI of 77(32-111). Patients with a normal SSI had a GIQLI of 105(29-140)(P < 0.0001). In patients with GERD the quality of life could be normalized from preoperative reduced values of GIQLI102(47-140) to postoperative values of 117(44-144). In patients with GERD and somatoform disorders, the GIQLI was improved from preoperative GIQLI 75(47-111) to postoperative 95(44-122)(P < 0.0043).CONCLUSION Patients with GERD and associated somatoform disorders have significantly worse levels of quality of life. The latter patients can also benefit from laparoscopic fundoplication, however they will not reach a normal level.

A 78-year-old man with a history of mitral valve prolapse underwent echocardiography during his cardiological check-up examination in 2011 in a symptom-free stage. Echocardiography revealed akinesis of the inferior septum and inferobasal free wall as a novel finding suggesting a distal right coronary artery （RCA） lesion （Figure 1）. The systolic left ventricular function was normal. Earlier echocardiographies did not show wall motion abnormalities.

Ocular biomechanical measurements on post- keratoplasty corneas using a Scheimpflug-based noncontact device

AIM： To analyse ocular biomechanical properties, central corneal thickness （CCT） and intraocular pressure （lOP） in post-keratoplasty eyes, as compared to normal subjects, with a new Scheimpflug-based technology. Moreover, biomechanical data were correlated with the size and age of the donor and recipient corneas. METHODS： Measurements were conducted on 46 eyes of 46 healthy patients without any corneal pathology （age： 53.83 ±20.8y） and 30 eyes of 28 patients after penetrating keratoplasty （age： 49.43 ±21.34y）. Ten biomechanical parameters, the CCT and lOP were recorded by corneal visualization Scheimpflug technology （CorVis ST） using high-speed Scheimpflug imaging. Keratometry values were also recorded using Pentacam HR system. Scheimpflug measurements were performed after 43.41± 40.17mo （range： 11-128mo） after the keratoplasty and after 7.64±2.34mo （range： 5-14mo） of suture removal. RESULTS： Regarding the device-specific biomechanical parameters, the highest concavity time and radius values showed a significant decrease between these two groups （P=0.01 and P 〈0.001）. None of other biomechanical parameters disclosed a significant difference. The CCT showed a significant difference between post keratoplasty eyes as compared to normal subjects （P= 0.003） using the CorVis ST device. The lOP was within the normal range in both groups （P=0.84）. There were no significant relationships between the keratometric data, the size of the donor and recipient, age of the donor and recipient and biomechanical properties obtained by CorVis ST. CONCLUSION： The ocular biomechanics remain stable after penetrating keratoplasty according to the CorVis ST measurements. Only two from the ten device-specific parameters have importance in the follow-up period after penetrating keratoplasty.

Bistability and oscillations in co-repressive synthetic microbial consortia

Background： Synthetic microbial consortia are conglomerations of genetically engineered microbes programmed to cooperatively bring about population-level phenotypes. By coordinating their activity, the constituent strains can display emergent behaviors that are difficult to engineer into isogenic populations. To do so, strains are engineered to communicate with one another through intercellular signaling pathways that depend on cell density. Methods： Here, we used computational modeling to examine how the behavior of synthetic microbial consortia results from the interplay between population dynamics governed by cell growth and internal transcriptional dynamics governed by cell-ceil signaling. Specifically, we examined a synthetic microbial consortium in which two strains each produce signals that down-regulate transcription in the other. Within a single strain this regulatory topology is called a ＂co-repressive toggle switch＂ and can lead to bistability. Results： We found that in co-repressive synthetic microbial consortia the existence and stability of different states depend on population-level dynamics. As the two strains passively compete for space within the colony, their relative fractions fluctuate and thus alter the strengths of intercellular signals. These fluctuations drive the consortium to alternative equilibria. Additionally, if the growth rates of the strains depend on their transcriptional states, an additional feedback loop is created that can generate oscillations. Conclusions： Our findings demonstrate that the dynamics of microbial consortia cannot be predicted from their regulatory topologies alone, but are also determined by interactions between the strains. Therefore, when designing synthetic microbial consortia that use intercellular signaling, one must account for growth variations caused by the production of protein.
