Acute respiratory distress syndrome and severe pneumonitis after atezolizumab plus bevacizumab for hepatocellular carcinoma treatment:A case report

BACKGROUND Hepatocellular carcinoma(HCC)is one of the most common cancers worldwide and has a high mortality.However,the treatment options for advanced HCC are limited to tyrosine kinase inhibitors,such as sorafenib and lenvatinib.Since previous regimens have an insufficient efficacy,the combination therapy of atezolizumab and bevacizumab(Ate/Bev)has been investigated,which showed an improvement in progression-free and overall survival.However,the adverse events of this combination therapy in advanced HCC have not been established.Herein,we report a novel case of an unresectable HCC and acute respiratory distress syndrome(ARDS)after a combination therapy of Ate/Bev.CASE SUMMARY An 82-year-old male visited our outpatient clinic for an incidentally detected liver mass.Liver magnetic resonance imaging and enhanced chest computed tomography(CT)were performed,which showed arterial hyperenhancement with washout in delayed phase suggesting HCC,and a well-defined metastatic solid nodule,respectively.F-18 fluorodeoxyglucose positron emission tomography(PET)-CT exhibited multiple hypermetabolic lesions in the iliac bone,lumbar vertebrae,and femur.Because of the high burden of the intrahepatic tumor,transarterial radioembolization was initially performed;after 37 d,a combination therapy of Ate/Bev was administered.The patient visited the emergency department three days after Ate/Bev treatment complaining of dyspnea.He was diagnosed with severe pneumonitis based on CT.Despite administering oxygen via a high-flow nasal cannula,the P/F ratio was only 74;therefore,the patient was diagnosed with ARDS based on the overall examination results.Low tidal volume with high positive end-expiratory pressure,sedative agents combined with a neuromuscular blocker,and a systemic steroid were promptly applied to manage the ARDS.However,the patient did not recover from the hypoxia and expired 31 h after being admitted.CONCLUSION Clinicians should be aware of severe pneumonitis due to the immune-related adverse events of this combination thera

One of the major obstacles for nucleic acid-based medicine is cellular uptake due to the concern of efficiencyand potential toxicity of the vehicle.In this work,we developed a novel approach to eliminate the need for a vehicle to allow for the cellular uptake of siRNA-based therapy with a unique modification strategy that also further enhances therapeutic efficacy.This discovery may be of great importance for the development of siRNA-based therapeutics as cellular uptake remains a critical challenge.In this study,we have tested the potential of this modification strategy for siRNA using a siRNA that targets B-cell lymphoma-2(BCL-2).

Numerical modeling and optimization of process parameters in continuous extrusion process through response surface methodology, artificial neural network & genetic algorithm

The product of high complex profile,high strength,high productivity and excellent material properties with infinite length can be produced by Continuous Extrusion(CE)process.The numerical simulation of Aluminum(AA 1100)feedstock material at different wheel velocities,product diameter,feedstock temperature,die temperature and friction condition has been carried out using 3D simulation tool Design Environment for Forming(DEFORM-3D)in this paper.The development of mathematical model is carried out to investigate the influence of wheel velocity,extrusion ratio,feedstock temperature,die temperature and friction conditions on total load required for the deformation and extrusion of feedstock material through Response Surface Methodology(RSM).The statistical significance of mathematical model is verified through analysis of variance(ANOVA).The most optimum value of extrusion load has been found to be 136.4 kN through iterative process of Genetic Algorithm(ga)using Artificial Neural Network(ANN).The optimized value of input process variables for minimum value of extrusion load obtained has been found to be 13 Revolutions per Minute(RPM)as wheel velocity,5 mm as product diameter,0.95 as friction condition,650◦C as feedstock temperature and 550◦C as die temperature.This paper with proposed methodology will be helpful for industries working in the area of CE in terms of minimizing energy consumption during production process of bus bars,tubes,wires,cables,sheets,plates,strips,etc.

Bioassay-guided in vitro study of the antileishmanial and cytotoxic properties of Bixa orellana seed extract

Objective:To investigate the leishmanicidal effect of the Bixa orellana crude seed extract and its fractions against Leishmania amazonensis.Methods:Four main fractions(BO-A,BO-B,BO-C and BO-D)were obtained by exhaustion with solvent with increased polarity from the Bixa orellana crude seed extract and 28 sub-fractions.The antileishmanial activity was evaluated in intracellular amastigotes and the cytotoxicity was assessed in murine intraperitoneal macrophages.Results:The BO-A and BO-B fractions showed a good antileishmanial activity with IC50 values of(12.9±4.1)and(12.4±0.3)μg/mL,respectively.The sub-fractions BO-B1(IC_(50)=(11.8±3.8)μg/mL)and BO-B3[IC_(50)=(13.6±4.7)μg/mL]also proved to have a good leishmanicidal effect.In general,the sub-fractions showed a lower toxicity than the crude extract.A selectivity index of 9 indicated a moderate selectivity of the BO-A,BO-B and BO-C fractions and BO-B1 sub-fraction.Conclusions:Potential of this plant against cutaneous leishmaniasis should be further investigated.

Deciphering the Origins of P1-Induced Power Losses in Cu(In_(x),ga_(1–x))Se2(CIGS)Modules Through Hyperspectral Luminescence

In this report,we show that hyperspectral high-resolution photoluminescence mapping is a powerful tool for the selection and optimiz1ation of the laser ablation processes used for the patterning interconnections of subcells on Cu(Inx,ga1-x)Se2(CIGS)modules.In this way,we show that in-depth monitoring of material degradation in the vicinity of the ablation region and the identification of the underlying mechanisms can be accomplished.Specifically,by analyzing the standard P1 patterning line ablated before the CIGS deposition,we reveal an anomalous emission-quenching effect that follows the edge of the molybdenum groove underneath.We further rationalize the origins of this effect by comparing the topography of the P1 edge through a scanning electron microscope(SEM)cross-section,where a reduction of the photoemission cannot be explained by a thickness variation.We also investigate the laser-induced damage on P1 patterning lines performed after the deposition of CIGS.We then document,for the first time,the existence of a short-range damaged area,which is independent of the application of an optical aperture on the laser path.Our findings pave the way for a better understanding of P1-induced power losses and introduce new insights into the improvement of current strategies for industry-relevant module interconnection schemes.

Effect of Pre-Harvest Application of Gibberellic Acid on Fruit Quality and Shelf Life of Mandarin (Citrus reticulata Blanco)

Mandarin (Citrus reticulata Blanco) is a promising fruit crop gaining popularity for its human nutrition and economic importance in Nepal. The qualitative losses during pre-harvest stage hinder the productivity and subsequently shorten the on-tree storability. An experiment was conducted to assess the effect of gibberellic acid on quality and shelf life of the mandarin fruit. ga3 at 10, 20, and 30 ppm as against of control were evaluated. Observations on fruit weight (g), fruit firmness (kg/cm2), rind colour (1 - 5 index), juice recovery (%), TSS/acid ratio, PLW (%), decay loss (%), and ascorbic acid (mg/100 ml) were recorded at three harvesting dates i.e. 20 Nov, 5 Dec, and 20 Dec and storage condition. It has been revealed that the fruits treated with ga3 at 20 ppm retained higher fruit weight (128.6 g), more firmness (3.54 kg/cm2), better juice recovery (57.75%), and greater TSS/acid ratio (21.24) at the end of study (20 December). The PLW was found less with ga3 at 30 ppm in both ambient (5.17%) and cellar (6.69%) condition as against untreated fruits (9.52% and 11.76%). Similarly, the decay loss was minimum in the fruits treated with ga3 at 30 ppm both with ambient (1.02%) and cellar condition (8.21%) as against control with ambient (5.54%) and cellar (21.58%).

具有正交Ho_(6)Co_(2)ga型晶体结构的富稀土金属间化合物因其具备独特的磁性能而被关注,但它们的磁有序、磁相变类型以及磁热性能尚未见系统报道.本文通过对RE_(6)Co_(2)ga化合物中的稀土元素类型的调控,实现了材料相变类型从反铁磁(AF)到顺磁(PM)的变磁调控为铁磁(FM)到顺磁(PM)的转变.此外,通过实验观察结合密度泛函理论(DFT)计算证明了Gd_(6)Co_(2)ga的基态为铁磁态.利用磁热效应的场依赖性关系结合Banerjee准则的判断标准研究表明,Ho_(6)Co_(2)ga和Dy_(6)Co_(2)ga同时具备二级相变和一级相变特征,而在Gd_(6)Co_(2)ga中只存在二级相变.在0-5 T的磁场变化下,Ho_(6)Co_(2)ga和Gd_(6)Co_(2)ga的磁熵变分别在26和75 K附近达到最大值10.1和9.1J kg^(-1)K^(-1),而这两个温度分别接近于H_(2)液化和N_(2)液化温度.优异的磁热性能使得RE_(6)Co_(2)ga体系在低温磁制冷领域具有潜在的应用前景.

目的探讨临床诊断为干眼症患者的睑板腺状态。方法非干预性、观察性研究。2010年3至12月在北京大学第三医院眼科诊断为干眼的194例（388只眼）作为研究对象。其中男性69例（138只眼），女性125例（250只眼），总体年龄范围7～85岁，平均年龄（44±18）岁，男性平均年龄（39±18）岁，女性平均年龄（47±17）岁。检查指标包括主观症状评分、泪液分泌试验、泪膜破裂时间（BUT）及角膜荧光染色情况，同时利用非接触红外线睑板腺照相系统进行睑板腺检查。结果观察的194例（388只眼）主观症状平均（5．47±2．14）分，SIaT值为（5．58±4．22）mm，BUT为（3．09±2．34）s，角膜荧光素染色评分（4．10±3．79）分。睑板腺照相评分为1分占3．09％（12只眼），2分占9．28％（36只眼），3分占20．10％（78只眼），4分占26．03％（101只眼），5分占21．91％（85只眼），6分占19．59％（76只眼）。结论睑板腺功能障碍是于眼症的重要病因之一。非接触红外线睑板腺照相系统可以客观评价睑板腺形态。

Community Hospital Experience of Surgical Times and Outcomes in Patients Undergoing Cesarean Deliveries for Non-Reassuring Fetal Tracing: A Retrospective Cohort

Background: Current guidelines recommend regional anesthesia versus general as a method of choice for women undergoing cesarean deliveries (CS). However, little is known about the surgical times in the operating room and a choice of anesthesia for cesarean deliveries. Objective: This study was designed to compare times from the arrival to the OR to the delivery of the fetus between regional and general anesthesia along with maternal and fetal outcomes, for patients undergoing cesarean sections for non-reassuring fetal tracing. Study Design: Records were reviewed for patients who underwent cesarean delivery for non-reassuring fetal heart rate tracing from February 2012 to May 2018. A total of 190 charts were selected. Seven patients who received epidural or spinal anesthesia and then converted to general anesthesia (ga) were excluded. The primary outcomes were: 1) entering the operating room to skin incision (min);2) the time from entering the operating room to delivery of the fetus (min). These times were compared among the patients who underwent epidural, spinal and general anesthesia. The secondary criteria included time from skin incision to delivery of the fetus (min), estimated blood loss (ml), Apgars scores, Arterial/venous cord pH, NICU admissions and fetal complications. ANOVA or Kruskal-Wallis Test was used for the continuous variable and Fisher’s exact test was used for the categorical variable to test the differences between groups. Logistic regression model was used for the binary outcomes after adjusting for age, BMI and number of prior laparotomies. Results: Infants in the ga group were delivered significantly faster when compared to epidural and spinal group separately with a P-value of 0.001. The mean time from arrival to OR to delivery of the newborn in ga group was 12.7 minutes, compared to 27 minutes in epidural group and 32.7 minutes in the spinal group. To the Editor： Here we report a craniotomy under general anesthesia successfully performed at a 4350-m altitude and the patient＇s follow-up results. To the best of our knowledge, this is one craniotomy carried out at the highest altitude in the world to date.A 21-year-old man occurred spontaneous light headache in the Ali area at a 4350-m altitude in the Tibet autonomous region of China on October 23,2011, and his headache became severe gradually. Three days later, the patient was in a coma with 5 scores （E1V1M3） on the Glasgow Coma Scale （GCS）; his left pupil diameter was 5.0 mm and right pupil diameter was 3.0 ram, both direct and indirect light reflex of the bilateral pupils disappeared. Craniocerebral computed tomography （CT） scan indicated a large acute subdural hematoma located in the left fronto-temporo-parietal region, widespread subarachnoid hemorrhage and brain herniation [Figure l a-c]. Emergency treatment was given for the patient in time, including continuous mask oxygen inhalation, strong dehydration （20% mannitol 250 ml ＋ furosemide 10 mg ＋ dexamethasone 5 rag, fast intravenous drip, once per 5 h）, and medical hemostasis.
