Acute Aortic Occlusion in a Critically Ill Adult Presenting to the Emergency Department

Acute Aortic Occlusion (AAO) is a rare, life-threatening event so far described mainly in small-scale series. The most common causes of AAO are large saddle emboli to the aortic bifurcation, in-situ thrombosis of an atherosclerotic aorta, and occlusion of previous surgical reconstruction. We present the case of a 52-year-old female with rheumatic heart disease, ischemic cardiomyopathy with restricted left ventricular function, atrial fibrillation, and previous cardio-embolic stroke, who was brought to the Emergency Department (ED) with sudden-onset dyspnea and lower backache radiating to both the legs. On arrival at the ED, the patient was electively intubated and mechanically ventilated in view of hypoxia and altered mental status, attributed to respiratory failure secondary to acute cardiogenic pulmonary oedema. The secondary survey revealed absence of bilateral femoral and popliteal artery pulsations. A computed tomography (CT) aortogram showed a complete lumen occlusion thrombus in the infra-renal region of the abdominal aorta at the level of L3-L4 lumbar vertebrae. An emergency embolectomy was performed successfully, following which the patient was started on heparin infusion and managed in the Intensive Care Unit (ICU). In the ICU, she suffered a torsade-cardiac arrest, with return of spontaneous circulation (ROSC) following rapid defibrillation. She was extubated on Day 3. Three weeks later, she was discharged from the hospital. At the time of discharge, she had developed ischemic lumbosacral radiculoplexus neuropathy, for which neuro-rehabilitation was advised. In our case report, we would like to highlight the following key points: 1) The importance of a detailed secondary survey in the Emergency Department (ED). 2) An inter-disciplinary approach to a complex syndrome that ensures the highest probability of a good outcome. .

A Geospatial Approach to Climatic Zone Specific Effective Horticultural Planning in East Khasi Hills District of Meghalaya, India

Climatic zone is one of the most important units for horticultural planning. Survival and failure of particular land use or farming system in a given region heavily relies on careful assessment of agroclimatic resources. Large variety of crops is being grown in Meghalaya traditionally, based on the needs and cultural practices of people without any consideration to the suitability of the climate. This study attempts to make adjustments with the climate based upon scientific knowledge to make the best use of the natural resources so as to recommend more area under horticultural crops. This has been made by identifying existing land use and appropriate land use strategies mainly for horticultural crops for each agro-climatic region. Modern Geospatial technology such as satellite Remote Sensing (RS) and Geographical Information System (GIS) and GPS have been used effectively to provide newer dimensions in identifying suitable sites for various horticultural crops and also for appropriate monitoring and management of land resources in an integrated manner with reference to agro-climatic condition.

Prolonged existence of SARS-CoV-2 RNAs in the extracellular vesicles of respiratory specimens from patients with negative reverse transcription-polymerase chain reaction

Background and aim:Severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)is primarily in the respiratory tract,particularly in patients with underlying *** study aimed to investigate the presence of the virus inside the extracellular vesicles(EVs)in patients with and without chronic liver disease(CLD).Methods:Eighty patients with positive SARS-CoV-2,including twenty-four patients with CLD and fiftysix patients without CLD,and five healthy controls with negative SARS-CoV-2 were *** swab specimens were tested for the detection of SARS-CoV-2 using reverse transcription-polymerase chain reaction(RT-PCR).Patients with coronavirus disease 2019(COVID-19)were followed up on days 7 and *** swab,collected in viral transport media(VTM),and plasma samples were investigated at each time *** were isolated from the nasal swabs(collected in VTM)and plasma using differential ultracentrifugation and estimated at each time *** transmission or replication by the EVs was assessed in Vero E6 ***:In patients with baseline RT-PCR positive,SARS-CoV-2 RNAs inside the EVs were found in 68/80(85%)patients with higher viral load in the nasal swabs than in the EVs(cycle threshold(Ct)value,23.4±5.7 vs.30.3±5.0,P<0.001).On follow-up at day 7,of the 32 patients negative for COVID-19,15(46.9%)had virus persistence in the EVs(Ct value,30.7±2.7),and on day 14,of the 56 patients with negative SARS-CoV-2,16 patients(28.6%)had positive SARS-CoV-2 RNAs in the EVs(Ct value,31.4±3.0).The mean viral load decreased on days 7 and 14 compared to baseline in the nasal swabs(P<0.001)but not in the ***,SARS-CoV-2 RNAs were undetectable in the plasma,but 12.5% of patients were positive in the plasma *** prolonged and high viral load was found in the EVs on day 14 in COVID-19 patients combined with CLD compared with COVID-19 patients(P?0.0004).We found significant higher levels of EV-associated with endothelial cells and hepatocytes

Batch, Fed Batch Production and Characterization of Glutaminase Free L-Asparaginase II of <i>Pectobacterium carotovorum</i>MTCC 1428 in <i>Escherichia coli</i>

The present study describes the production optimization of recombinant L-asparaginase II of Pectobacterium carotovorum MTCC 1428 in Escherichia coli BL21 (DE3) at batch and fed batch bioreactor level. Production of recombinant L-asparaginase II in batch and fed batch mode was found to be 1.34 and 5.38 folds higher, respectively as compared to shake flask culture. SDS-PAGE and native PAGE of the purified enzyme revealed that molecular mass of the subunits and native enzyme are ~37.5 kDa and ~150 kDa, respectively. Optimum range of pH and temperature for hydrolysis of L-asparagine were found to be 7.5 - 8.5 and 47°C - 52°C, respectively. The recombinant enzyme is very specific for its natural substrate, L-asparagine. The activity of recombinant L-asparaginase II is improved by mono cations and diverse effectors including Na+, K+, L-cystine, L-histidine, glutathione and 2-mercaptoethanol whereas, it is moderately inhibited by different divalent cations and thiol group blocking reagent. The kinetic parameters Km, Vmax, kcat and Km/Kcat of purified recombinant L-asparaginase II were determined. The purified L-asparaginase II possesses no partial glutaminase activity, which is prerequisite to reduce the possibility of side effects during the course of anti-cancer therapy.

A Novel Approach in Identification of Urban Hot Spot Using Geospatial Technology: A Case Study in Kamrup Metro District of Assam

Urbanization in recent years plays an important role in increase in impervious areas with reducing in vegetation cover and pervious areas of natural landscape. This leads to a rise in temperature of urban areas, by several degrees particularly at night [1,2]. A novel geospatial approach has been adopted to determine the maximum temperature areas (hot spots) over Kamrup Metro District of Assam, which is a gateway for seven neighboring north eastern states of India. The G statistics have been calculated for detecting the presence of hot spot or cold spot over the entire study area which is a new approach in urban heat island studies. The resultant z-scores and p-values show the pixels with either high or low values cluster spatially. For statistically significant positive z-scores, the larger the z-score is, the more intense the clustering of high values (hot spot) and vice versa. Land Surface Temperature (LST) anomaly values and percentage of Impervious Surface Area (ISA) along with climatic data are used to conform the hot spot location. It is one of the densely populated areas with more commercial pockets thereby giving rise to anthropogenic heat discharge which accelerates the heat island phenomenon. Incorporation of socio-economic survey data as well as certain biophysical parameters can be used to know about the cause and future impact of urbanization.

Spent silk worm pupae ofAntheraea assama were analyzed for its amino acid constituents and compared with the amino acid composition of fiber from A. assama, A. pyrni, and Bombyx mori. Amino acid percent was determined by UV Spectrophotometer at 570 nm. Infra Red spectra recorded the presence of different amino acids in the spent silk worm pupae powder which corroborate with the amino acids present in the fiber of different silk species.

Root Extract of Polygonum Hydropiper Alters the Expression of Rat Uterine Protein Profile in Presence and Absence of Ovary in-situ during Periimplantation Period:An Evidence on SDS-PAGE

Objective To study its effects on reproductive performance in albino rats.Methods The chromatographic fraction （CF） of crude methanolic extract of the herb has been subcutaneously administered to female albino rats. Experiments were carried out in adult cyclic females and oavriectomised （OVX） females during early gestation period. Uterine horns were collected following the respective treatment regimen to stud）, the protein profile in 15% gel SDS-PAGE.Results The CF induced changes in the expression of protein in rat uterus. New proteins have been expressed in uterus of adult cyclic females having ovary in-situ. The OVX females treated with CF showed altered uterine protein profile compared with that of OVX control and OVX estradiol-17β （E2） treated rats. The CF exerted its effect on expression of uterine protein during early gestation period in rats. While uterine proteins of CF treated females were similar to that of controls during preimplantation period; many of the proteins on day 6 of gestation have been found either missing or expressed in lesser intensity.Conclusion The root of Polygonum hydropiper contains potential compound（s） which can alter the reproductive performance of female rats modulating uterine protein expression.

Stereoselective synthesis of vic-halohydrins and an unusual Knoevenagel product from an organocatalyzed aldol reaction:A non-enamine mode

Stereoselective synthesis by an aldol reaction between chloroacetone and aldehyde was studied using a synthesized chiral organocatalyst and triethylamine. The reaction gave α-chloro-β-hydroxy ketones in excellent yield with high anti selectivity and enantioselectivity. The chiral organocatalyst was also used in the Knoevenagel reaction, which gave α-cyano-β-hydroxy ketones at a low temperature and the usual Knoevenagel product at a high temperature. Both products were obtained in good to moderate yield with good anti selectivity in the case of α-cyano-β-hydroxy ketone derivatives.
