BACKGROUND: Though inhaled nitric oxide (iNO) improved oxygenation but had no effect on mortality in randomized clinical trial for acute respiratory distress syndrome (ARDs), its efficacy in modulation of inflammation and antioxidant capability in airway and alveolar epithelial cells in mature lungs remains to be verified. As iNO is usually used in hyperoxic condition, it may alter surfactant lipid de novo synthesis in inflammatory lung. Despite the fact that NO may potentiate damage to surfactant proteins and inhibit surfactant phospholipid synthesis, iNO is still considered an alternative and encouraging therapy in intractable respiratory failure to obviate more invasive and expensive strategies. OBJECTIVE: To investigate whether hyperoxia and/or inhaled nitric oxide (iNO) may affect de novo synthesis of phosphatidylcholine (PC) in mature lungs with inflammatory injury. Regulation of activity of nuclear factor-kB and activator protein-1 by nitric oxide, surfactant and glucocorticoids in alveolar macrophages from piglets with acute lung injury

AIM: To investigate whether acute lung injury (ALl) in ventilated piglets with bacterial infection affects NF-κB and AP-1 expression in alveolar macrophages (AM) and whether nitric oxide (NO), surfactant (surf), glucocorticoids (GC) affect NF-κB and AP-1 activation in AM in vivo and in vitro. METHODs: The animals were intraperitoneally injected Escherichia coli, which caused ALl. Nuclear extracts of AM were analyzed by electrophoretic mobility shift assay (EMsA) for the nuclear factor-kappa B (NF-κB) and activation protein-1 (AP-1) expression. Detection of IκB-α protein was from cytoplasmic extract by Western blotting. Immunocytochemistry staining was used for intracellular location of p65 subunits of NF-κB. REsULTs: In ex vivo experiments, strikingly higher expression of NF-κB and AP-1 by EMsA was found 6 h after bacterial injection in contrast to the Normal group. BACKGROUND: septic lung injury results in intractable respiratory failure and surfactant and nitric oxide (NO) may protect the lungs from injury by suppression of inflammatory process. AIM: To establish a piglet model of acute lung injury (ALI) induced by Escherichia coli peritonitis and investigates anti-inflammation mechanisms of combined surfactant and inhaled nitric oxide (iNO) treatment. METHODs: Thirty piglets (body weight 6-9 kg) were randomly allocated to either a normall group (N, n=6) with only intraperitoneal injection of normal sodium or groups with intraperitoneal injection E coli at 5×10~9 ***-1(n=6 each) followed by treatment with (1) control (C), (2) iNO at 10 ppm (NO), (3) surfactant at 100 mg/kg (surf), (4) both surfactant and iNO (sNO). Background Bronchopulmonary dysplasia(BPD)is a common chronic lung disease in extremely preterm *** outcome and clinical burden vary dramatically according to *** some prediction tools for BPD exist,they seldom pay attention to disease severity and are based on populations in developed *** study aimed to develop machine learning prediction models for BPD severity based on selected clinical factors in a Chinese *** In this retrospective,single-center study,we included patients with a gestational ages who were diagnosed with BPD in our neonatal intensive care unit from 2016 to *** collected their clinical information during the maternal,birth and early postnatal *** factors were selected through univariable and ordinal logistic regression *** models based on logistic regression(LR),gradient boosting decision tree,XGboost(XGB)and random forest(RF)models were implemented and assessed by the area under the receiver operating characteristic curve(AUC).Results We ultimately included 471 patients(279 mild,147 moderate,and 45 severe cases).On ordinal logistic regression,gestational diabetes mellitus,initial fraction of inspiration O_(2) value,invasive ventilation,acidosis,hypochloremia,C-reactive protein level,patent ductus arteriosus and Gram-negative respiratory culture were independent risk factors for BPD *** the XGB,LR and RF models(AUC=0.85,0.86 and 0.84,respectively)all had good *** We found risk factors for BPD severity in our population and developed machine learning models based on *** models have good performance and can be used to aid in predicting BPD severity in the Chinese population.
