Laparoscopic Repair of Perforated Duodenal Ulcer (Series of 50 Cases)

Background:?Perforated peptic ulcer is a common surgical emergency. The classic treatment is the mid-line laparotomy. However, laparoscopic treatment has been shown to be reliable. Few studies have evaluated its overall utility. The aim of this study is to assess the efficacy of laparoscopic repair of perforated duodenal ulcer.?Patients and Methods:?This study included 50 patients presented by perforated peptic ulcer between July 2009 and August 2014. They were submitted to laparoscopic omental patch repair with thorough peritoneal wash. Patients’ demographics, diagnostic techniques, management and outcome were evaluated. Results:?The mean age was 38.6 years with male to female ratio being 1.6:1. The perforation was diagnosed by plain X-ray abdomen in erect position in 43 patients and by abdominal CT scan in 7 patients. The laparoscopic repair of the perforation was successful in 48 patients while in 2 patients mid-line laparotomy was needed for proper control of the severe intra-abdominal sepsis. Post-operatively, all patients tolerated soft diet on the 3rd?post-operative day and full diet on the 4th?post-operative day. The mean duration of hospital stay was 4.5 days. Two patients developed post-operative intra-abdominal collection that was treated by ultrasound guided drainage, three patients developed umbilical port site wound infection while only two patients developed leakage, one of them reoperated after failed conservative surgery. No mortality was encountered in the study. Conclusion:?Laparoscopic repair of perforated peptic ulcer is a safe and reliable technique with accepted morbidity and mortality rates with all the advantages of the minimally invasive surgery.

Seroprevalence of Human Brucellosis in Kuku Dairy Scheme, Khartoum State, Sudan

A seroprevalence investigation of human brucellosis was carried out in Kuku Dairy Scheme, Sudan. A total of 176 serum samples were collected and screened by Rose Bengal Plate Test （RBPT）. The positive sera vqere further examined using Tube Agglutination Test （TAT） and c-Elisa. The seropositivity was 15.9%, 14.8% and 11.4% using RBPT, TAT and c-Elisa respectively. Whereas, the active infection based on seropositivity and clinical signs were 4.6%, 4.6% and 2.3% in case of RBPT, TAT and c-Elisa respectively. Based on c-Elisa result the infected individuals were further subjected to clinical examination and treated with streptomycin and doxocycline for six weeks.

Modification on Synthesis of Mixed Ligand Chelates by Using Di- and Trivalent Transition Metal Ions with Schiff Base as Primary Ligand

New mixed ligand chelates synthesized from di-and trivalent metal ions (Cr, Co, Ni, and Cu ions) and Schiff base (L1) resulted from the condensation of 4-dimethylaminobenzaldehyde with 2-aminophenol as primary ligand, whereas 2-nitroaniline (L2) represents the secondary ligand in a molar ratio of M:L1:L2 [1:1:1]. The synthesized Schiff base and chelates have been characterized by using several tools, such as, elemental analysis, molar conductivity, magnetic moment measurements, infrared and electronic spectra. The mass spectra of the ligands and Ni(II) chelate were used to justify the process of modification, as well as, the electron paramagnetic resonance spectrum which was carried out for Cu(II) chelate all in order to elucidate the chemical and geometrical structure of the chelates. On the basis of the obtained data, the geometry of the products was proposed for all the chelates.

Effects of Wood Ash and Waste glass Powder on Properties of Concrete in Terms of Workability and Compressive Strength in Jaresh City

The potential for using fly ash as a supplementary cementing material in concrete has been known almost since the beginning of the previous century. Fly ash was used as a supplementary cementing material (SCM) in the production of Portland cement concrete. A supplementary cementing material, when used in conjunction with Portland cement, contributes to the properties of the hardened concrete through hydraulic or pozzolanic activity, or both. In this study, the fly ash and waste glass powder were used in concrete blocks to study the improvement of concrete in terms of workability and strength. Therefore, an experimental study will be conducted to measure the engineering properties of cured concrete. In this research, local raw material from Jaresh area was used.

Effects of Stone Cutting Powder (Al-Khamkha) on the Properties of Self-Compacting Concrete

Most of the construction materials research now concerns on investigation of construction materials that is locally produced at a rate and cost compatible with the pace of construction. The present paper is concerned with investigation of fresh and hardened properties of self-compacting concrete (SCC) produced from local available materials in JORDAN. The produced SCC contains the local stone cut waste powder which is called Al-KHAMKHA in JORDAN with different replacement of (0%, 10%, and 25%) of fine silica aggregate;?the study also investigatesthe effects of SP33 super?plasticizer which is used by different doses (1%, 1.5% and 2%) for cement. The slump flow and the compressive strength of SCC were studied and the experimental results indicate the possibility of using Al-KHAMKHA in the production of SCC as the results showed that the compressive strength of the SCC with 10 % replacement by al-khamkha together with 1% SP33?super plasticizer was higher compared to pure SCC without al-khamkha;?the results also showed that as al-khamkha content increased the slump flow decreased.

Effect of Pre-Existing Chronic Medical Conditions on Clinical Features and Disease Outcomes of COVID-19 Infection in Jordanian Children

Background: The COVID-19 pandemic emerged unexpectedly, impacting millions worldwide and, though waning in many regions, remains a significant concern. This study focused on understanding COVID-19’s clinical presentation, disease progression, and outcomes among hospitalized pediatric patients. Additionally, it sought to investigate the impact of concurrent chronic medical conditions on disease severity. Methods: Conducted retrospectively between September 2020 and December 2021, this study observed pediatric patients (below 18 years) hospitalized with confirmed COVID-19 at an Irbid, Jordan tertiary healthcare center. Patients meeting inclusion criteria, including a positive nasal swab PCR test and hospitalization need, were analyzed. Epidemiological and clinical data were collected and segregated into two groups (A and B) based on the presence of chronic conditions. Comparative analysis encompassed presentation, disease trajectory, and outcomes between these groups. Results: Among 451 tested positive patients, 112 were included in the study. Significant differences emerged between the two pediatric groups hospitalized for COVID-19. Those with concurrent chronic illnesses exhibited heightened symptom severity—fever, dyspnea, fatigue—and a higher incidence of abnormal chest radiographs, often requiring intensive care and experiencing elevated mortality rates. Conclusion: This study underscores the criticality of prioritizing COVID-19 management strategies for pediatric patients with pre-existing chronic medical conditions. The findings highlight the increased vulnerability and poorer outcomes experienced by this subgroup, emphasizing the necessity for tailored interventions and focused care approaches.

Esophageal Cancer in Sudan: Demographic and Histopathlogical Variations

Background: Sudan is experiencing a growing cancer problem. Esophageal cancer (EC) is a devastating disease with high mortality. Our objective was to study the demographic factors and histopathological pattern of EC cancer in Khartoum, Sudan. Methods: In a retrospective cross-sectional study, the records and histopathology reports of all patients diagnosed endoscopically as EC and who were referred to Zain center, a reference histopathology center in Khartoum Sudan, were studied over a 6-month period. Descriptive and simple statistics were used for analysis. Results: Out of 74 histopathologically studied patients, 66 (89.2%) had squamous cell (SCC) cancer (average age 59 y, mainly females) and 8 (10.8%) had adenocarcinoma (AC-average age 69 y). The female to male ratio is 1.5:1. In females with SCC, the tumors mainly located in the upper and middle thirds of the oesophagus. On the other hand, 6 out of the 8 patients with AC were men, with the tumors located mainly in the lower third. Conclusions: In Khartoum, Sudan, esophageal cancer affects females (predominantlySCC) more than males (predominantly AC). It occurs at a relatively young age, and is predominantly squamous cell in type. No specific risk factors were identified and this needs further studies.

Objective:This study aimed to examine the psychometric properties of the Conditions for Workplace Effectiveness Questionnaire-Ⅱ-Arabic Version(CWEQ-Ⅱ-AV),which measures structural empowerment among *** enhance the usability and credibility of the CWEQ-Ⅱ among researchers within Arabicspeaking countries,a valid and reliable Arabic version of the measure is ***:A cross-sectional research design was *** December 2018 to June 2019,275 nurses working in 4 hospitals participated in the *** was assessed by examining internal consistency and split-half reliability.A confirmatory factor analysis was performed to evaluate the factor structure of the CWEQ-Ⅱ-***:The psychometric properties of the CWEQ-Ⅱ-AV were excellent regarding the six-factor model(opportunity,information,resources,support,formal power,and informal power).The results showed the following fit indices meet the criteria set a priori:comparative fit index(CFI)=0.96,root mean square of error approximation(RMSEA)=0.06,andХ^(2)/df=***'s a coefficient was 0.95 for the total questionnaire and ranged between 0.83 and 0.89 for the individual *** split-half reliability was 0.91 for the total questionnaire and ranged from 0.83 to 0.87 for individual ***:This study provides evidence that CWEQ-Ⅱ-AV is both a reliable and valid measure of structural empowerment among Arab nurses.

Establishment of Human Neuroblastoma Cell-Line (SK-N-SH) as an In Vitro Model of Morphine Addiction

Morphine is a schedule II-controlled substance that used to allow the diminution of intra-operative, post-operative orchronic pain. However, its usage is limited due to addiction and overdose liabilities. Morphine was observed to cause tolerance,dependence and withdrawal in human. Justification: to date lack of scientific evidence of morphine addiction was carried out byusing specific single human neuroblastoma cell-line (SK-N-SH). Therefore, this study was performed to establish the morphineaddiction model in this cell line. The cells were exposed to morphine for 24 hrs before treatment with methadone, as ananti-withdrawal drug for subsequence 24 hours. The cytosolic fraction of the cell was used in different objectives including receptoraffinity, withdrawal properties, endocytic machinery, desensitisation or internalisation and cellular adaptation. The result shows thatmorphine and methadone bind to the μ-opioid receptor. The morphine-treated cells were observed to increase the expression ofaddiction markers, have a low rate of the endocytic machinery, cause desensitisation of receptor and reduce cellular *** changes by morphine were normalised by the treatment of methadone. As a whole, it is postulated that neuroblastoma cell line,SK-N-SH, can be used as an in-vitro model to demonstrate morphine addiction before animal and human testing.
