Prosthetic design of reverse shoulder arthroplasty contributes to scapular notching and instability

AIM To evaluate whether implant design, glenoid positioning, and other factors influenced instability and scapular notching in reverse total shoulder *** We retrospectively reviewed records of patients who had undergone reverse total shoulder arthroplasty by the senior author from July 2004 through October 2011 and who had at least 24 mo of follow-up. The 58 patients who met the criteria had 65 arthroplasties: 18 with a grammont-type prosthesis(grammont group) and 47 with a lateral-based prosthesis(lateral-design group). We compared the groups by rates of scapular notching and instability and by radiographic markers of glenoid position and tilt. We also compared glenoid sphere sizes and the number of subscapularis tendon repairs between the groups. Rates were compared using the Fisher exact test. Notching severity distribution was compared using the χ2 test of association. Significance was set at P grammont group had a higher incidence of scapular notching(13 of 18; 72%) than the lateral-design group(11 of 47; 23%)(P gher incidence of instability(3 of 18; 17%) than the lateral-design group(0 of 47; 0%)(P = 0.019). glenoid position, glenoid sphere size, and subscapularis tendon repair were not predictive of scapular notching or instability, independent of implantdesign. With the lateral-based prosthesis, each degree of inferior tilt of the baseplate was associated with a 7.3% reduction in the odds of developing notching(odds ratio 0.937, 95%CI: 0.894-0.983). CONCLUSION The lateral-based prosthesis was associated with less instability and notching compared with the grammonttype prosthesis. Prosthesis design appears to be more important than glenoid positioning.

RAMARIA SUBgENERA RAMARIA AND LAETICOLORA IN YUNNAN

Three taxa of Ramaria subgenus Ramaria, and 18 of subg. Laeticolora are reported from Yunnan Province. Of these 13 are considered new to science. Only one taxon (R. formosa) is considered of widespread distribution, the others being known olay from the Indo-Asian subcontinent. Wide distribution patterns, based on phenetic similarity, seem to be at the infraspecific to species complex level, but only rarely contaxie. New taxa are as follows: subg. Ramaria: R. eryuanensis; subg. Laeticolora: R. brunneipes, R. distinctissima, R. ephemeroderma, R. hilaris, R. lacviformosoides, R. linearioides, R. linearis, R. nanispora, R. rubri-attenuipcs, R. sino-conjunctipes, R. rubricarnata var. laeta, R. botrytoides var. microspora, and R. neoformosa var. sinensis. Keys to Yunnan taxa of Ramaria subgenera Ramarla and Laeticolora are furnished.

A 32-year-old man recovered completely from hypokalemic hypertension that had been caused by primary reninism after the ablation of an ectopic left testis, epididymis and ductus deferens. For several years, severe hypertension has been resistant to treatment, even the concurrent administration of up to seven antihypertensive agents. In this case, cryptorchidism was associated with an indirect inguinal hernia and an open peritoneo-vaginal process on both sides, aplasia of the posterior wall of the inguinal canal on the right side, an umbilical hernia, and a retroperitoneal tendrillar hemangioma. （Asian J Androl 2006 Mar; 8： 247-250）

Theoretical Analysis of the Fluctuation Theorem Applied to Electric Circuits

We carry out a theoretical analysis of the fluctuations in the Johnson–Nyquist noise in an RC electric circuit being driven by a non-ideal constant current source. Using the appropriate Langevin equation, we derive the fluctuation theorem for the system. This analysis exhibits some interesting features. Firstly, the average of the dissipation function is not necessarily just an instantaneous entropy production, i.e. the work being done on the circuit by the external power source divided by the ambient temperature. Secondly, by appropriate selection of the values for the circuit components,the transient response of the system can be made identical to the nonequilibrium steady state response. In the limit of an ideal current source, the average of the dissipation function reduces to the entropy production. Our analysis accurately reproduces published experimental results.

Instant evaluation of contrast enhanced endoscopic ultrasound helps to differentiate various solid pancreatic lesions in daily routine

BACKgROUND Contrast enhanced harmonic endoscopic ultrasound(CEH-EUS) is a spreading technique; some studies have shown its value in the diagnosis of pancreatic adenocarcinoma using quantitative *** To examine the value of CEH-EUS for differentiating various pancreatic lesions in everyday routine with qualitative and quantitative *** Data of 55 patients with pancreatic lesions who underwent CEH-EUS were analysed retrospectively. Perfusion characteristics were classified by the investigator qualitatively immediately upon investigation, quantitative analysis was performed later on. Samples from fine needle aspiration(EUS-FNA) or surgical specimen served as gold *** CEH-EUS showed 39 hypoenhanced lesions, 3 non-enhanced and 13 hyperenhanced lesions. Concordance of the investigators qualitative classification of peak contrast enhancement with quantitative analysis later on was 100%, while other parameters such as arrival time, time to peak or area under the curve did not show additional value. 34 of 39 hypoenhanced lesions were pancreatic adenocarcinoma; of the hyperenhanced lesions 4 were inflammatory, 3 neuroendocrine carcinomas, 1 lymphoma, 1 insulinoma and 4 metastases(2 of renal cell carcinoma, 2 of lung cancer). Non-enhanced lesions showed up as necroses. Sensitivity for the detection of pancreatic adenocarcinoma was 100%,specificity 87.2% for hypoenhancement alone; in otherwise healthy pancreatic tissue all hypoenhanced lesions were pancreatic adenocarcinoma(sensitivity and specificity 100%, PPV and NPV for adenocarcinoma 100%).CONCLUSION This study again shows the excellent value of CEH-EUS in everyday routine for diagnostics of various focal pancreatic lesions suggesting that qualitatively assessed hypoenhancement is highly predictive for adenocarcinoma. It is likely that the majority of proteins will undergo post-translational modification, be it enzymatic or non-enzymatic. These modified protein(s) regulate activity, localization and interaction with other cellular molecules thereby maintaining cellular hemostasis. Alcohol exposure significantly alters several of these post-translational modifications leading to impairments of many essential physiological processes. Here, we present new insights into novel modifications following ethanol exposure and their role in the initiation and progression of liver injury. This critical review condenses the proceedings of a symposium at the European Society for the Biomedical Research on Alcoholism Meeting held September 12-15, 2015, in Valencia, Spain.
