Diagnostic value of ABCD2 and ABCD3-Ⅰ risk scoring systems in determining one-month risk of stroke in patients with transient ischemic attack: An observational study

Objective:To compare the diagnostic value of the ABCD2 and ABCD3-栺scoring systems in predicting the development of a 30-day neurological event in adult patients with transient ischemic attack(TIA)presenting to the emergency ***:The study was observational and prospective and was conducted in a single *** diagnostic values of the ABCD2 and ABCD3-栺scores in predicting a neurological event within one month were compared in patients diagnosed with TIA in the emergency ***:A statistically significant difference was observed between groups with or without stroke within one month in terms of both the ABCD2 and ABCD3-栺scores(P=0.044 and P=0.029,respectively).There was no statistically significant difference between the patients with and without a recurrent TIA within one month in relation to the ABCD2 score(P=0.934),but a statistically significant difference was found in the ABCD3-栺scores of these groups(Pnclusions:Both the ABCD2 and ABCD3-栺scoring systems could predict ischemic stroke within 30 days of TIA,the ABCD3-栺score is more effective than the ABCD2 score in the prediction of TIA recurrence.

犯罪—刑罚的发生次序类似于个体本能反应中的刺激—反射,但又与之存在根本差异。符合群体伦理信念的刑罚能得到强化,反之则会被削弱。刑罚的设定应遵循该当性原则,符合社会公众的一般观念。犯罪与刑罚应跟随群体伦理信念调整。现代社会刑罚结构的制定权常由专门机构、社会精英行使。虽不应倒退回刑罚制定的原初阶段,但亦不能忽视刑罚的社会文化基础,否则难以取得预想的实施效果,还可能适得其反。

Background: globally, much has changed in the imaging modalities available to investigate maxillofacial trauma. nevertheless, the regional practice patterns are highly influenced by availability, affordability and accessibility of imaging services as well as competence in interpreting radiological images and the practice of defensive medicine. The aim was to assess the use of maxillofacial trauma imaging modalities as well as to document patterns of skeletal injuries at selected government-sponsored health facilities in Kenya. Material and Methods: A cross sectional study, done in three health facilities. Results: There were 137 (81.5%) males and 31 (18.5%) females with a mean of 27.73 ± 9.86 years. Commonest aetiology of trauma was interpersonal violence (80, 47.62%) and road traffic crashes (63, 37.50%). The total number of radiological examinations was 250 with a mean of 1.49 ± 0.99 examinations per patient. Plain film radiographs were more (210, 84%) than computed tomographic scans (39, 15.6%). Dental panoramic technique was the most (104, 61.9%) popular. Most participants experienced isolated fractures to the mandible (99, 58.9%) and midface (28, 16.7%). Conclusion: Dental panoramic radiography remained the imaging of choice for mandibular fractures which had the commonest occurrence. There is a shift from the traditional conventional skull radiography to computed tomography in the diagnosis of midfacial fractures. Judicious utilization and prompt provision of affordable services in dental panoramic and computed tomographic scanning at the study centers and other upcoming health facilities are paramount in ensuring timely diagnosis and management of maxillofacial injuries in Kenya.

The Relationship between giant goiter and Operative Complications: A Retrospective Study

Background: Thyroidectomy for giant goiter is a surgical challenge due to distorted and displaced anatomy. The aim of this study is to evaluate the relationship between giant goiter and its operative complications. Material and Methods: A retrospective multicenter study of consecutive patients who had thyroid surgery was conducted, including 639 patients who undergone thyroidectomy in State hospital at Van and Corlu city—Turkey. Seven cases had giant goiter in the patients. Total thyroidectomy was performed all patients. Results: All patients were women. The mean weight of glands removed was 689 gr in giant goiter’s patients. Two operative complications had happened;right site injury of the external branch of the superior laryngeal nerve had happened to one patient;hypocalcemia was happened to another one patient. In those two patients previously were operated partial thyroidectomy. Conclusions: Thyroidectomy for a massively enlarged goiter is technically challenging. The predominant operative complications were related to previously operate and the thyroid gland due to distorted and displaced anatomy. The surgical approach to such cases requires carefully preoperative evaluation and planning. Especially, using of intraoperative nerve monitoring is to be useful in these difficult cases which previously had undergone surgery.

Aluminum Metal Matrix Composites (MMCs) sought over other conventional materials in the field of aerospace, automotive and marine applications owing to their excellent improved properties. These materials are of much interest to the researchers from few decades. These composites initially replaced Cast Iron and Bronze alloys but owing to their poor wear and seizure resistance, they were subjected to many experiments and the wear behavior of these composites were explored to a maximum extent and were reported by number of research scholars for the past 25 years. In this paper an attempt has been made to consolidate some of the aspects of mechanical and wear behavior of Al-MMCs and the prediction of the Mechanical and Tribological properties of Aluminum MMCs.

Stress enhances expression of calcium-binding proteins and nMDAR subunit genes in the rat hippocampus

Backgrounds:Oxidative stress impairs the function of calcium-binding proteins and deregulates calcium signaling in living *** have previously explored the overexpression of calcium-binding protein genes in a reactive oxygen and nitrogen species-induced in vitro cell model of stress that leads to ***,in in vivo models,low levels of stress leads to depressive-like ***,we aimed to analyze gene expression of major calcium-binding proteins(calcineurin,calmodulin,calsyntenin,synaptotagmin,and calreticulin)and n-methyl-D-aspartic acid(nMDA)receptor subunits(glutamate receptor ionotropic[glun]glun1,glun2A,and glun2B)in the hippocampus of stress-induced ***:Six-week-old male Wistar rats were assigned to two stress induction groups and a control group without stress(n=6).Stress was induced by using H_(2)O_(2)(3%in water)or by immobilization(using a sticky mat)over a period of 30 *** of calcium-binding protein genes in the hippocampus,antioxidant assays,structural alterations in hippocampal neurons,and depressive-like behavior were ***:Expression of genes encoding calcium-binding proteins calcineurin,calsyntenin,synaptotagmin and nMDA receptor subunit glun1 was enhanced in both chemical and physical stress-induced rats compared with control rats(4.25±0.05 vs.1.03±0.02,pnduced rats,neurons in the CA2 region of the hippocampus were fewer and appeared disorganized compared with control ***,stressinduced rats showed decreased mobility and lower sucrose preference in behavioral studies compared with control ***:Lower levels of reactive oxygen and nitrogen species(ROnS)can also lead to stress in rats by affecting their calcium signaling,buffering capacity in the neurons leading to depressive symptoms.

Statistical Characteristics of Scattered Radiation in Medium With Spatial-temporal Fluctuations of Electron Density and External Magnetic Field

Influence of temporal fluctuations of both electron density and external magnetic field fluctuations on scattered ordinary and extraordinary waves in magnetized plasma is investigated using the ray-(optics) method. Transport equation for frequency fluctuations of scattered radiation has been derived. Broadening of the spatial power spectrum and amplification of the intensity of frequency fluctuation taking into account geometry of the task and the features of turbulent magnetized plasma is analyzed for the anisotropic gaussian correlation function using the remote sensing data. It is shown that spatial-temporal fluctuations of electron density and external magnetic field, anisotropy and angle of inclination of prolate irregularities relative to the external magnetic field may lead to the exponential amplification of the intensity of frequency fluctuations of scattered electromagnetic waves in the collisional magnetized plasma.

Conventional or traditional methods for the use of food processing have been outperformed by novel approaches recently. One such promising technology is ultra-sonication and recent studies have shown better effects in physical characteristics of food which are induced by ultrasound. The previous researches further emphasize that the introduction of ultra-sonication on dairy proteins has resulted in the improvement of their structural, physical and chemical properties while it is widely used in hydrocolloid systems to accelerate the gelation process. Therefore this theory could be useful for the gelation of milk proteins in the presence of a polysaccharide. After discussing briefly on hydrocolloids and the theory of ultrasound, the possibility and the impact of ultrasound on gelation of milk protein mixtures with Carrageenan is highlighted in this study. The inherent characteristics of both milk proteins and carrageenan and properties of the ultrasound treatment have cumulatively effect on gelation. Therefore, there is a necessity to develop specific variables depending on the type of dairy product.
