Musculoskeletal injuries following seizures have a high morbidity and mortality. These injuries are often missed and the diagnosis is delayed due to a lack of clinical suspicion and appropriate investigations. We report a case of 72 years old male with simultaneous bilateral central acetabular fracture dislocation and bilateral posterior shoulder fracture dislocation secondary to an epileptic seizure. Present study high- lights the sig.i.icance of clinical suspicion and clinico-radiological evaluation for diagnosis of a rare injury following episode of seizures. Simultaneous fracture dislocation of all four limbs treated with a holistic approach can lead to a good functional recovery. Surgical management with open reduction and internal fixation is preferred and replacement arthroplasty should be reserved for cases with implant failure and elderly patients.

Purpose： To evaluate the role of immediate and definitive management of gustilo type Ⅲ A/B tibia fractures with intramedullary nailing and fasciocutaneous flap. Methods： From August 2010 to July 2012, 22 patients with gustilo grade Ⅲ A/B tibia fractures were managed with a single stage treatment of ipsilateral fasciocutaneous flap ＆ reamed intramedullary nailing and were included in the study. The severity of the injury was calculated with ganga Hospital injury severity score. Results： The mean age of patients was 41 years and the follow-up time ranged from six months to one year. Among the 22 patients, 73% were type Ⅲ B fractures with upper leg.i.volved in 55% of them. The time interval from injury to completion of surgery was 8 14 h. The incidence of bone infection requiring secondary procedure was 9%; the major and minor soft tissue complication rate was 9% and 14% respectively. The limb salvage rate was 100%. Conclusion： Multidisciplinary management of severe lower limb trauma is important and provides good outcomes, lntramedullary nailing and immediate flap fixation can achieve early bone union and good soft tissue coverage, leading to good outcomes in patient with grade Ⅲ A ＆ B tibia fractures.

The corona virus disease 2019(COViD-19)pandemic has resulted in most nations deciding upon self-isolation and social distancing policies for their citizens to control the pandemic and reduce hospital *** review aimed at evaluating the effect of physical activity on mental well-being during the COViD-19 *** was concluded that the COViD-19 pandemic may lead to augmented levels of angiotensin-converting enzyme(ACE)-2 that led to cardiovascular and neurological disorders associated with highly inflammatory effects of viral infection affecting the brain tissues leading to damage of the nervous system and resulting.i. cog.i.ion dysfunction,insulin sensitivity reduction,and behavioral *** and depression may lead to negative effects on various quality of life domains,such as being physically *** physical activities may reduce inflammatory responses,improve ACE-2 responses,and improve mental well-being during self-isolation and social distancing policies related to the COViD-19 *** studies should be conducted to assess the different intensities of physical activities on cardiovascular function,and mental well-being during the COViD-19 pandemic.

Although the pectoralis major myocutaneous flap is often used in head and neck reconstruction, the extension of the skin paddle beyond the inferior limits of the muscle has not been well described. We aim to clarify the design and application of this extended flap in head and neck reconstruction. in this retrospective study, consecutive cases of extended pectoralis major myocutaneous flap reconstruction of post-ablative head and neck defects at a single tertiary referral center were included for analysis. in 7 cases an extended pectoralis major flap was utilized, in which the skin paddle was extended beyond the inferior border of the pectoralis major to include the rectus sheath. Skin and soft tissue as well as composite defects of the oral cavity, parotid/temporal region and neck were reconstructed. All flaps healed satisfactorily with no loss of skin viability. The extended pectoralis major myocutaneous flap is robust and has versatile applications for reconstruction of large, high and three dimensionally complex defects in the head and neck region.

Descending branch of the lateral circumflex femoral artery （LCFA） is commonly used pedicle for anterolateral thigh （ALT） flap. Oblique branch of LCFA is an alternative pedicle that can be used in microvascular surgery. According to review of literature and to the best of our knowledge we could not find the use of oblique branch of LCFA as a pedicle of the ALT flap in regional soft tissue reconstruction. Here we presented a case of a 55-year-old man sustaining soft tissue injury and wound over the left trochanteric and gluteal region following a road traffic accident, who was treated by the use of extended ALT pedicle flap with oblique branch of LCFA as the pedicle for reconstruction of soft tissue defect in trochanteric and gluteal regions with successful outcome.

Feed scarcity is a major challenge in smallholder production systems especially during the dry season. Sweet potato vines (SPV) contribute over 40% of the crop residues fed to dairy cattle but they are highly perishable resulting.i.to losses of about 24% per season. Ensiling SPV mitigates seasonal feed shortages and assists in coping with seasonal feed price fluctuations. This study was conducted to evaluate the effect of sweet potato vines silage (SPVS) supplementation on the performance of lactating Ankole × Friesian crossbred dairy cattle. Four primiparous cows in early lactation were randomly allotted a basal diet of Rhodes grass (Chloris gayana) hay supplemented with dairy pellets and four graded levels of SPVS (0, 100, 200 and 300 g/kg of the daily ration) in a 4 × 4 Latin square design. The animals were given a one-week adaptation period followed by a two weeks data collection period for each diet. Dry matter intake (DMi), live weight changes, milk yield and composition were determined. Dry matter intake improved with addition of SPVS (P ≤ 0.05), with intake being highest (11.9 kg) in cows that were fed the highest SPVS level and lowest in cows that were not given SPVS (6.8 kg). Although animals on diets with 0, 100, 200 g of SPVS/kg of daily ration lost weight, there were no sig.i.icant differences (P ≥ 0.05) in body weight changes across all treatments. Milk yield increased with SPVS supplementation by 1.5 - 1.7 kg per cow per day but there were no sig.i.icant differences between the different SPVS supplementation levels. Milk fat and solids-non-fat differed (P ≤ 0.05) across the four diets but there was no consistent trend observed. Considering profit margins, supplementing dairy cows with SPVS at a level of 100 g/kg of daily ration was the most profitable at Uganda shillings 1290 (0.344 USD) per cow per day. in conclusion, SPVS supplementation improved DMi and milk yield but supplementation beyond 100 g/kg of the daily feed intake was not cost-effective.
