Antisperm antibodies （ASA） occur in men and women and may significantly impair fertility. The identification of functionally relevant antigens of ASA is important for establishing causal treatment options on the one hand and may, on the other hand, enhance the development of tools for immunocontraception. One candidate protein is addressed in an article of Wang et al. [1] published in Asian Journal of Andrology.

Male genital tract inflammation/infection and its impact on male fertility are still widely neglected in the workup of male fertility *** from inconsistent definitions of such inflammatory conditions which allow no adequate diagnosis and,consequently,no rational therapeutic approach,the role of bacterial infection,in particular by Chlamydia trachomatis,is also controversially *** reason for that may be insufficient tools of sperm analysis in many laboratories that only rely on basic semen parameters as sperm number,motility,and morphology,in best cases accompanied by determination of the concentration of leukocytes.

Drug treatment remains an active domain in the therapy of male fertility disorders. Although there are only a fewconditions that allow causal treatment, rational approaches are possible in many cases. Best results are obtained in casesrequiring an anti-inflammatory treatment and in patients with an impaired sperm transport. High-dosage administrationof FSH is a promising new development, aimed particularly at improving the disturbed sperm structures. A careful di-agnostic work-up with elucidation of the underlying disease is essential to achieve a successful therapy.

Since release of the latest WHO manual with the new lower reference values of semen parameters, a lot of discussion has been raised about their usefulness and appropriateness for assessment of male fertility. As with the previous reference values the new limits do neither allow an andrological diagnosis based on nosological criteria nor clear-cut differentiation between fertility and sub-/infertility. Therefore, considering the fact that fertility is a continuum, the new lower reference limits should not be overestimated. Most probably, more sperm function tests, such as determination of DNA integrity, and--in the future--assessment of biomarkers, such as sperm proteomics will be included into andrological work-up, thus resulting in a more personalized approach of infertility management. On the other hand, the detailed instructions for standard and advanced semen analysis provided in the new manual are very much appreciated and should be adopted by each seriously workin~ laboratory.

Involuntary childlessness occurs in about 6%-9% of all couples in central Europe,1 the incidence in the Unites States is comparable.2 At least 50% of all couples affected do not seek medical help？ This may have different reasons. In the present article, the authors point out that awareness and familiarity with infertility is greater in high socioeconomic status men but did not differ by race. In contrast, non-Caucasian men indicated that the infertility would be a serious condition for them; moreover, they are concerned about infertility and believe that it impairs a man＇s quality-of-life. Thus, lack of awareness of the condition of infertility and of its treatment options may contribute to previously described disparities in the treatment of infertility.

Amongst the reasons for male infertility the factor ＇stress＇ is frequently cited, however, without providing more detailed explanations. In the article ＇Decline of semen quality during IVF is not associated with subjective male stress＇ published on Asian Journal ofAndrology, Nouri et al1 describe a decline in progressive sperm motility in a retrospective cohort of 155 male in vitro fertilization （IVF） patients during a time period of 4-6 weeks prior to the first IVF cycle and the time of oocyte retrieval. In a subsequent prospective cohort study on men undergoing their first IVF semen quality and subjective male chronic stress were investigated at the same time intervals.

Aim： To investigate age-related inflammatory events in the male genital tract. Methods： In a total of 4 265 randomly collected patients attending the andrological outpatient clinic of the Center for Dermatology and Andrology, University of giessen, germany, ejaculate volume, pH-value, sperm concentration, total and progressive sperm motility, concentration of polymorphonuclear （PMN） elastase, number of peroxidase-positive cells and fructose were measured and correlated with patient＇s age. Results： While ejaculate volume, motility and fructose all correlated negatively with age, sperm concentration, PMN elastase and the pH-value showed a positive correlation. The prevalence of male genital tract inflammation （as defined by PMN elastase 〉 250 ng/mL） and its severity increased significantly. PMN elastase did not correlate with sperm motility. Fructose as a marker of seminal vesicle function showed a significant negative relationship with the PMN elastase levels, the number of peroxidase-positive cells and sperm motility. Conclusion： The significant increases of PMN elastase levels as marker of male genital tract inflammation in older men appear to be indicative of age-related changes in local immunoregulatory mechanisms. Because there is no association of PMN elastase with sperm motility, a direct inhibitory effect of this enzyme can be excluded.
