Surgical outcome after docetaxel-based neoadjuvant chemotherapy in locally-advanced gastric cancer

AIM:To investigate feasibility,morbidity and surgical mortality of a docetaxel-based chemotherapy regimen randomly administered before or after gastrectomy in patients suffering from locally-advanced resectable gastric ***:Patients suffering from locally-advanced(T3-4 any N M0 or any T N1-3 M0)gastric carcinoma,staged with endoscopic ultrasound,bone scan,computed tomography,and laparoscopy,were assigned to receive four 21 d/cycles of TCF(docetaxel 75 mg/m 2 day 1,cisplatin 75 mg/m 2 day 1,and fluorouracil 300 mg/m 2 per day for days 1-14),either before(Arm A)or after(Arm B)*** morbidity,overall mortality,and severe adverse events were compared by intention-to-treat ***:From November 1999 to November 2005,70 patients were *** preoperative TCF(Arm A),thirty-two(94%)resections were performed,85% of which were *** response was complete in 4 patients(11.7%),and partial in 18(55%).No surgical mortality and 28.5%morbidity rate were observed,similar to those of immediate surgery arm(P= 0.86).Serious chemotherapy adverse events tended to be more frequent in arm B(23%vs 11%,P=0.07),with a single death per ***:Surgery following docetaxel-based chemotherapy was safe and with similar morbidity to immediate surgery in patients with locally-advanced resectable gastric carcinoma.

Hydrophilic and hydrophobic competition in water-methanol solutions

We study the thermal behavior of the longitudinal spin-lattice,T1,and the transverse spin-spin,T2,relaxation times of the macroscopic magnetization in water/methanol *** aim is to investigate the reciprocal influence of hydrophobic effects on water properties and of hydrophilicity(via hydrogen bond,HB,interactions)on the *** classical Nuclear Magnetic Resonance spectroscopy,we find a single characteristic correlation time r c that reflects all local structural configurations and characterizes the thermal motion effects of the magnetic nuclei on the spin-spin *** find that in the supercooled regime the correlations are stronger,with respect to ambient temperature,because the HB interactions have a lifetime long enough to sustain a stable water ***,increasing the temperature,progressively decreases the HB interaction lifetime and destroys the water clusters with a consequent decoupling in the dynamic modes of the *** addition,at temperatures higher than about 265 K,the hydrophobicity becomes gradually stronger and governs the physical properties of the solutions.

The goals of global vaccination are to control,eliminate,or eradicate infectious diseases in a sustainable way that strengthens public health *** the use of vaccines is essential for the control of epidemics,the vaccines against coronavirus disease 2019(COVID-19)proved to be inadequate to end the pandemic and thus are considered *** vaccines failed to prevent infection,so their primary purpose has been shifted to prevent severe disease and reduce hospitalizations and ***,we believe that all the strategies available to reduce transmission,hospitalizations and deaths due to COVID-19 will be put in *** is reported that uncontrolled inflammation and thrombosis are the principal mechanisms for aggravation and death in patients with *** corticosteroids that should not be administered at the beginning of the symptoms for their immunosuppressive action,which could worsen the evolution of the disease,the usefulness of non-steroidal anti-inflammatory drugs in the early at-home treatment of the disease is becoming evident.

Mediation for medical malpractice actions: an efficient approach to the law and veterinary care

Veterinarians today face an increased risk for malpractice claims in view of increased client awareness of the benefits of the human-animal bond and advances in preventive and specialized veterinary *** important problem with medical malpractice litigation is the manner in which malpractice claims are resolved. In several countries, thousands of claims have been successfully resolved via Alternative Dispute Resolution processes such as mediation, conciliation and *** medical media-tion, health care providers, patients/animal owners, families, clergy and attorneys partici-pate directly in an informal, usually voluntary, dispute resolution process that can lead to for unique and promising approaches to resolving *** in Italy framework legislation addressing mediation has been introduced and it is also used to resolve medical malpractice *** that in Italy medical mediation is formally a new concept and is still evolving, an examination is made. The importance of “mediation” for veterinarians’ civil liability will be evaluated, underlining how it could become an important instrument in speeding up the resolution of court actions concerning medical mal-practice.

High Supracostal Percutaneous Nephrolithotomy Access: Assessing Safety in Access above the Eleventh Rib after Performing Preoperative Planning with Computed Tomography

Objective: To determine if supracostal renal access above the 11th rib during percutaneous nephrolithotomy (PCNL) is a safe option in carefully selected patients determined by preoperative computed tomography (CT) imaging. Patients and Methods: We retrospectively isolated 142 patients who underwent access above the eleventh rib during PCNL, which we term “high supracostal renal access.” We then compared these patients to 113 individuals who underwent access below the twelfth rib. Renal access was achieved by the operative surgeon with fluoroscopic guidance in conjunction with pre-operative computed tomography (CT) scan. Outcomes were compared. Results: Overall surgical outcomes were equivalent when comparing high supracostal versus subcostal access sites. As expected due to proximity, pleural complications occurred in 4% of the high supracostal group (n = 6) compared with 0% of the control (subcostal) group (p = 0.035). Of these six complications, three were managed conservatively with observation and two required cardio-thoracic intervention with video-assisted thoracoscopic pleural repair (1%). In the remaining case, the patient was preoperatively consented for placement of a thoracostomy tube, which was placed during the procedure, due to the difficult location of her upper pole stone and closely adjacent low-lying pleura, and the planned transpleural approach. Hospital stay was not significantly pro-longed between the high supracostal access and subcostal access groups, with an average length of stay of 2.2 ± 2.1 days and 2.0 ± 1.9 days (p = 0.59) respectively. Conclusions: Careful, systematic preoperative planning based on CT and fluoroscopic imaging allows for a confident understanding of a “safety zone” in placement and dilatation of renal access points during PCNL. We have shown that planned upper pole renal access above the 11th rib is achievable with acceptable morbidity and excellent success rates.

Wearable devices and IoT applications for symptom detection, infection tracking, and diffusion containment of the COVID-19 pandemic: a survey

Until a safe and effective vaccine to fight the SARS-CoV-2 virus is developed and available for the global population, preventive measures, such as wearable tracking and monitoring systems supported by Internet of Things(IoT) infrastructures, are valuable tools for containing the pandemic. In this review paper we analyze innovative wearable systems for limiting the virus spread, early detection of the first symptoms of the coronavirus disease COVID-19 infection, and remote monitoring of the health conditions of infected patients during the quarantine. The attention is focused on systems allowing quick user screening through ready-to-use hardware and software components. Such sensor-based systems monitor the principal vital signs, detect symptoms related to COVID-19 early, and alert patients and medical staff. Novel wearable devices for complying with social distancing rules and limiting interpersonal contagion(such as smart masks) are investigated and analyzed. In addition, an overview of implantable devices for monitoring the effects of COVID-19 on the cardiovascular system is presented. Then we report an overview of tracing strategies and technologies for containing the COVID-19 pandemic based on IoT technologies, wearable devices, and cloud computing. In detail, we demonstrate the potential of radio frequency based signal technology, including Bluetooth Low Energy(BLE), Wi-Fi, and radio frequency identification(RFID), often combined with Apps and cloud technology. Finally, critical analysis and comparisons of the different discussed solutions are presented, highlighting their potential and providing new insights for developing innovative tools for facing future pandemics.

Transcriptional regulation of ethylene and jasmonate mediated defense response in apple(Malus domestica)root during Pythium ultimum infection

Apple replant disease(ARD)is a significant economic restraint to the successful re-establishment of new apple orchards on sites previously planted to the same *** ultimum,an oomycete,is a significant component of the ARD pathogen *** ethylene(ET)-and jasmonic acid(JA)-mediated defense responses are intensively studied in the foliar pathosystem,the transferability of this knowledge to the interaction between a perennial root system and soilborne pathogens is *** aim of this study was to test the hypothesis that the ET/JA-mediated defense response is conserved in roots of tree crops in response to infection by *** genes with the annotated function of ET/JA biosynthesis,MdERF(ethylene response factor)for signaling transduction and a gene encoding a pathogenesis-related(PR)protein(b-chitinase,the target of ERF)were identified from the apple genome *** transcriptional profiles of these genes during *** infection and after exogenous ET and/or JA treatment were characterized using *** genes showed a 10-to 60-fold upregulation in apple root tissue 24-48 h post inoculation(hpi).Exogenous ET and JA treatment exhibited either a positive or negative influence on expression of ET or JA biosynthesis genes,depending upon gene isoforms and the tissue types,while the expression of MdERF and the PR protein encoding gene was upregulated by both ET and JA *** data are consistent with the hypothesis that ET/JA-mediated defense pathways are functional in the root system of perennial tree species defending soilborne pathogens.
