Selective cyclooxygenase-2 (COX-2) inhibitors are widely used due to their efficacy and good safety profile. However, recent case reports have described varying degrees of liver injuries associated with the use of COX-2 inhibitors. We report the case of a patient who developed acute cholestatic hepatitis progressing to hepatic failure requiring liver transplantation, following a 3-d course of celecoxib for treatment of generalized muscle aches and pains. The clinical presentation, the laboratory data, as well as the liver histopathology were supportive of the putative diagnosis of drug induced liver injury.

A randomized controlled trial of imipramine in patients with irritable bowel syndrome

AIM: To study the efficacy of low-dose imipramine in relieving symptoms associated with the irritable bowel syndrome (IBS). METHODS: A randomized, double-blind trial of 25 mg imipramine vs matched placebo for 12 wk was performed. Doubling the dose was allowed once at week 2 in case of an unsatisfactory early response. Primary efficacy variables were subjective global symptom relief and quality of life (QoL) using SF-36 at week 12. RESULTS: One hundred and seven patients were enrolled by advertisement or referral by general practitioners and 56 (31 imipramine: 25 placebo) completed the 16-wk study. Baseline characteristics were comparable. A high overall dropout rate was noted in the imipramine and placebo arms (47.5% vs 47.9%, P > 0.05), a mean of 25.0 and 37.4 d from enrollment, respectively (P gnificant difference in global symptom relief with imipramine over placebo (per-protocol: 80.6% vs48.0%, P = 0.01) and a trend on intent-to-treat (ITT) analysis (42.4% vs 25.0%, P = 0.06). This improvement was evident early and persisted to week 16 (P = 0.024 and 0.053 by per-protocol and ITT analyses, respectively). Mean cumulative and componentspecific SF-36 scores improved in the imipramine group only (per-protocol, P g-related adverse events leading to patient dropout were more common in the imipramine group (25.4% vs 12.5%, P > 0.05). CONCLUSION: Imipramine may be effective in the treatment of IBS patients and is associated with improved QoL. Careful patient selection, initiation of a low dose with gradual escalation and monitoring for side effects may result in an improved therapeutic response.

Eosinophilic gastroenteritis(EgE) is a rare and benign inflammatory disorder that predominantly affects the stomach and the small *** disease is divided into three subtypes(mucosal,muscular and serosal) according to klein's classification,and its manifestations are protean,depending on the involved intestinal seg-ments and ***,accurate diagnosis of EgE poses a significant challenge to clinicians,with evidence of the following three criteria required:Suspicious clinical symptoms,histologic evidence of eosinophilic infiltration in the bowel and exclusion of other pathologies with similar *** this review,we designed and applied an algorithm to clarify the steps to follow for diagnosis of EgE in clinical *** management of EgE represents another area of *** remains the mainstay of treatment;however the disease is recognized as a chronic disorder and one that most frequently follows a relapsing course that requires maintenance *** prolonged steroid treatment carries of risk of serious adverse effects,other options with better safety profiles have been proposed;these include budesonide,dietary restrictions and steroid-sparing agents,such as leukotriene inhibitors,azathioprine,anti-histamines and mast-cell *** cases or small case series have been reported in the literature for all of these options,and we provide in this review a summary of these various therapeutic modalities,placing them within the context of our novel algorithm for EgE management according to disease severity upon presentation.

Phenolic compounds are naturally extracted molecules used nowadays for several industrial applications such as food preservatives and neutraciticals;thus industrial needs are in constant growth. Optimization of phenolic compounds (PC) and monomeric anthocyanins (MA) extraction from grape berries using response surface methodology was here achieved. Cabernet Sauvignon (CS) wine grapes (Vitis vinifera L. cv. Cabernet Sauvignon) are a rich source of anti-radical phe- nols, extractable as dietary supplements. However, efficiency of extraction strongly depends on specific experimental conditions. In this work, the comparison between two extraction mixtures, Acetone/Water (A/W) and Methanol/Water (M/W) and the effects of three critical variables—Extraction Time (between 8 and 88 h), Extraction Temperature (between 1?C and 35?C) and Solvent Content (between 63% and 97%)—on Phenolic Compounds Yield (PCY) and Monomeric Anthocyanins Yield (MAY) were studied using Response Surface Methodology (RSM). Process conditions maximizing the PCY were found to be 87% A/W solvent content, after 88 h incubation at 26?C, and those of the MAY were found to be 97% M/W solvent content, after 8 h incubation at 1?C. In addition, chromatographic profiles of extracts showed very different affinities of the phenolic compounds families to the experimental conditions employed. This process advantages are: low percentage of organic solvent used, high quality of extracted molecules thanks to low temperature treatment, very low energy consumption and low cost of needed machinery. At industrial scale, it may be used as promising technique for the extraction of bioactive compounds from plant or byproduct material such as from wine or grape juice industries.

Water resources in Lebanon are one of the major threatened natural resources, since they spread on different geographic locations creating a network of permanent and temporary watercourses that govern the socio-economy and livelihood. However, lately, these watercourses, and certainly the rivers and streams, are witnessing unfavorable impacts that make them with less benefit and use. In addition, most of the Lebanese river basins have not been characterized due to the newly physical challenges of climate change and the human interference. Hence, characterization of the Lebanese river basins must be obtained in order to monitor the changing of physical and anthropogenic parameters. This can help for better conservation and adaptation approaches, notably for climate change. Arqa River, a coastal Lebanese river in North Lebanon is a typical example. In this study, Arqa River basin and the occupied streams among it have been studied using new advanced geomatic techniques. These techniques are based on the treatment of digital geo-spatial data in systematic approaches. ArcgIS is the software which is used to elaborate and manipulate (data interrelation, measures, overlapping, ordering, etc.) the geo-spatial data. Thus, the geometric and morphometric analysis were obtained for the river catchment and the existing tributaries. This will be a primary tool for further applications including water resources management in the river system. This can be utilized in the execution of dams, mountain lakes, man-made channels and in mapping flood-prone areas. In addition, the obtained characterization of this basin will be helpful in mapping erosion processes and estimation of the infiltration capacity, plus monitoring surface water flow. It is a typical approach of analysis that can be applied for the rest of Lebanese river systems in order to attain a better water resources management. Cholangiocarcinoma is an uncommon malignancy that increases with age. Cholangiocarcinoma has been linked to inflammatory bowel disease, and a relationship between inflammatory bowel disease, anti-tumour necrosis factor alpha antibody (Infliximab) and cholangiocarcinoma has been suggested. Although cholangiocarcinoma is usually associated with ulcerative colitis, recently more cases are being seen in association with Crohn’s disease. The risk of cancer in patients treated with Infliximab has been discussed, and whether Infliximab is associated with an increased risk of cholangiocarcinoma is not clear. Cholangiocarcinoma is an uncommon malignancy and it is difficult to establish a clear correlation with Infliximab treatment. A large number of reports are needed to further address these issues.

In this work, optimization of phenolic compounds (PC) and monomeric anthocyanins (MA) extraction from Syrah (Sy) wine grapes (Vitis vinifera L. cv. Syrah) using response surface methodology was conducted. The comparisons between two extraction mixtures, Acetone/Water (A/W) as well as Methanol/Water (M/W) and the effects of three critical variables, Extraction Time (between 8 and 88 h), Extraction Temperature (between 1℃ and 35℃) and Solvent Content (between 63% and 97%), on Phenolic Compounds Yield (PCY), Monomeric Anthocyanins Yield (MAY) and the DPPH Free Radical Inhibition Potential (DFRIP) were studied. The highest PCY was obtained in 63% A/W solvent content after 88 h incubation at 35℃. The highest MAY was acquired in 97% M/W solvent content after 8 h incubation at 17℃. The highest DFRIP of the extract was attained using 97% A/W solvent content after 16 h incubation at 35℃. The low cost of this process, on economic and environmental levels, could lead to interesting applications on an industrial scale. It could be used to obtain bioactive phytochemicals from direct material or byproducts for either therapeutic or nutritional purposes.

Sexual Perception of Young Lebanese Students

This article presents the sexual perception of young Lebanese students. We hypothesized that, in Lebanon and the Arab countries, premarital sex is forbidden by most cultures because of conservative societies and backgrounds. Sexuality lives as a “myth” in these conservative societies. We examined the influence of demographics and genders among a group of 706 Lebanese university students, from public and private universities, as well as the fluctuation of sexual beliefs and practices, based on their gender, demographics and social pressure. The method used was a questionnaire collected form n = 706 students, 446 females and 260 males. The female population of our sample is 62.9% of the total volunteers while the male population is 36.7%. They come from different religions (Christian, Muslim, Druze and Other), between May 2015 and December 2015. Quantitative significant results: 1) A non-conformity with the social and demographic pressure and an openness of the new generations. Similarly, it shows more permissiveness in flirting. Even though, parts of the negative answers were significantly linked to understanding the rules of prohibited sex (NO 32.4% for belief and NO 34.5% for practice);2) the reality of Lebanese youth, their integrity in engaging in a relationship, and their true daily happening of social and peer pressure;3) An opening to a natural outlook of sexual life, (25.5%) with reluctance on dating (51.1%);4) An important significance is showed in the percentage between beliefs and practice: 18% in belief and 78.3% in practice (Q1) and 34.2% in belief and 60.8% in practice (Q2) emphasizing on the reality of sexual engagement within Lebanese youth;5) However, reluctance in engaging in sexual activity and attachment and fear to social sanctions toward premarital sex and agreeing on abstinence before marriage.
