The aim of this paper is to analyze the Mexican energy system and its greenhouse gas (gHg) emissions for the year 2012 and to estimate a baseline scenario for 2026 using an input-output analysis. The elasticity of emissions with respect to national demand is calculated in order to identify the total and distributed effects of CO2 equivalent (CO2-eq) emissions. In this framework, the analysis evaluates the effects in the economy related to changes in individual sector demands, and, vice versa, the effect on individual sectors due to global changes in national demands. Results show that passenger and freight transport, power generation, iron and steel industry, chemical industry, air transportation and agriculture concentrate the largest potential for mitigation strategies, and also have important distributive effects on the Mexican economy. Results are evaluated under the mitigation strategies of industrial sector proposed by the Fifth Assessment Report of the IPCC.

Objectives The purpose of this research was to study long term left ventricul ar(LV) adaptations in very high level endurance athletes. Background Knowledge of cardiac changes in athletes, who are at particularly high risk of sudden car diac death, is mandatory to detect hypertrophic cardiomyopathy (HCM) or dilated (DCM) cardiomyopathy. Methods We carried out echocardiographic examinations on 2 86 cyclists(group A) and 52 matched sedentary volunteers (group C); 148 cyclists participated in the 1995 “Tour de France”race (group A1), 138 in the 1998 rac e (group A2), and 37 in both (group B). Results In groups A, A1, A2, and C, resp ectively, diastolic left ventricular diameter (LVID)was 60.1±3.9 mm, 59.2±3.8 mm, 61.0±3.9 mm, and 49.0 ±4.3 mm (A vs. C and A1 vs. A2, pg group A, 147 (51.4%) had LV ID >60 mm; 17 of them had also a below normal (55 mm. In group B, LVID increased (58.3 ±4.8 mm to 60.3 ±4.2 mm, pg with a reduced LVEF in 11.6%(17 of 147), compatible with the diagn osis of DCM. Increased WT was less common (always g the diagnosis of HCM. Serial examinations showed evidence of furth er LV dilation along with wall thinning. These results might have important impl ications for screening in athletes.

AIM:To investigate the feasibility of small bowel polypectomy using double balloon enteroscopy and to evaluate the correlation with capsule endoscopy(CE).METHODS:This is a retrospective review of a single tertiary ***-five patients treated by enteroscopy for small bowel polyps diagnosed by CE or other imaging techniques were *** correlation between CE and enteroscopy(correlation coefficient of Kendall for the number of polyps,intra-class coefficient for the size and coefficient of correlation kappa for the location) was ***:There were 31 polypectomies and 12 endoscopic mucosal resections with limited morbidity and no *** analysis revealed 27 hamartomas,6 adenomas and 3 *** agreement between CE and optical enteroscopy was observed for both location(Kappa value:0.90) and polyp size(Kappa value:0.76),but only moderate agreement was found for the number of polyps(Kendall value:0.47).CONCLUSION:Double balloon enteroscopy is safe for performing *** CE is useful in selecting the endoscopic approach and to predicting the difficulty of the procedure.

目的:评估肺栓塞诊断策略的似然比 (likelihood ratios),并通过验前概率(pretest probability)来决定其临床应用价值。数据来源:通过检索Medline,Embase, Pascal Biomed等网络资源,结合人工检索查找 1990年1月至2003年9月期间发表的文章。研究选择:选择评估确诊或排除肺栓塞的诊断性检查的研究。数据提取:确诊肺栓塞诊断策略的阳性似然比,排除肺栓塞诊断策略的阴性似然比。数据综合:1012篇文章中共有48篇入选。诊断性检查的阳性似然比为:肺通气灌注扫描呈高度可能为18．3(95％置信区间为10．3- 32．5),螺旋CT为24．1(12．4-46．7),下肢静脉超声为16．2(5．6-46．7)。如果患者具有中度或高度的肺栓塞验前概率,那么其验后概率将高于 85％。阴性似然比为:肺通气灌注扫描正常或接近正常为0．05(0．03-0．10);螺旋CT和超声波检查均阴性为0．04(0．03-0．06);定量酶联免疫吸附试验法所测定的D-dimer浓度小于500μg／L 的为0．08(0．04-0．18),如果肺栓塞验前概率为低度或中度,那么其验后概率则低于5％。然而,单纯螺旋CT、肺通气灌注扫描呈低度可能、磁共振血管造影、乳胶凝集法(latex)定量测定D- dimer以及全血凝集法测定D-dimer试验的阴性似然比也是较高的,从而只能排除那些肺栓塞验前概率低的诊断。结论:对于疑似肺栓塞的患者,各种诊断性检查的准确性差异很大。尽管如此,我们还是可以评估每一项确认或排除肺栓塞试验或策略的验前概率的范围。

. The aim of this study was to evaluate the impact of the surgical approach on the management and outcomes of patients with early borderline ovarian tumors (BOTs). Material and methods. We retrospectively reviewed the medical charts of patients with stage Ia to Ic BOT treated surgically between January 1, 1985, and December 31, 2001. We compared patients initially managed by laparoscopy vs laparotomy in terms of potentially harmful procedures and quality of staging. Results. Of the 118 included patients, 48 (41% ) had laparoscopy for initial surgery, 54 (45% ) had laparotomy, and 16 (14% ) had conversion from laparoscopy to laparotomy. Conservative treatment (57% of patients) was more common with laparoscopy (vs laparotomy, P ger than 44 years, P gical approach (P = 0.1). Bag extraction was used in 19 (40% ) of the 48 laparoscopy patients. Staging was incomplete in 73% of patients overall. By univariate analysis, better quality of staging was associated with bilateral adnexectomy, age >44 years, laparotomy, hysterectomy, and treatment after 1995. By multivariate analysis, bilateral adnexectomy or hysterectomy was associated with better staging. Mean follow-up was 40 months, during which recurrence and survival rates were similar in the laparoscopy and laparotomy groups. Conclusion. Staging of macroscopic early stage BOTs was better in patients requiring radical surgery. After adjustment on disease severity, type of surgical access was not related to staging quality.
