Immunovirological and biochemical Changes in Nigerian Patients with Hepatitis b Coinfection on Antiretroviral Therapy

Hepatitis b virus (HbV) co-infection with HIV is high among Nigerians. Some studies have suggested impaired CD4 recovery among coinfected patients compared to the HIV mono infected. This retrospective study of treatment-na?ve HIV infected patients was aimed at determining the trend of changes in CD4+ counts, HIV-RNA and renal and liver function tests in response to combined antiretroviral therapy (CART). A questionnaire was utilised to extract clinical and laboratory data of HbV co infected HIV/AIDS patients before treatment and at six, twelve and eighteen months of therapy with CART. Findings were compared to those of HIV mono infected. Relevant statistical instruments were used to analyse for comparisons of means of Log10 HIV viral load and CD4 count using SPSS package 15.0. All levels of sig-nificance were at 5 %. Two thousand five hundred and sixty two patients were analysed. Of these, 354(13.8%) were HbsAg positive. Majority (63.1%) were females. Most of the recruited patients were on combivir and nevirapine. The median CD4 count for the HbsAg negative was 104 cells/mm3 (IQR 34 – 171) and it was significantly higher than those of the positive (91 cells/mm3) (p gher among HbsAg positives, while urea and creatinine levels were similar. The median change in CD4 count from baseline and during the course of therapy were similar in the two groups. Similarly, virological responses were not different in the two groups at the various time points. In con-clusion no significant difference in the rate of CD4 recovery and HIV-RNA decline in among coinfected and monoin-fected HIV patients at different stages of therapy.

Tecnis Symfony Intraocular Lens with a “Sweet Spot” for Tolerance to Postoperative Residual Refractive Errors

Purpose: To investigate the impact of residual astigmatism on visual acuity (va) after the implantation of a novel extended range of vision (ERV) intraocular lens (IOL) based on the correction of spherical and chromatic aberration. Method: The study enrolled 411 patients bilaterally implanted with the ERV IOL Tecnis Symfony. Visual acuity and subjective refraction were analyzed during the 4- to 6-month follow-up. The sample of eyes was stratified for four groups according to the magnitude of postoperative refractive astigmatism and postoperative spherical equivalent. Results: The astigmatism analysis included 386 eyes of 193 patients with both eyes of each patient within the same cylinder range. Uncorrected vas for distance, intermediate and near were better in the group of eyes with lower level of postoperative astigmatism, but even in eyes with residual cylinders up to 0.75 D, the loss of va lines was clinically not relevant. The orientation of astigmatism did not seem to have an impact on the tolerance to the residual cylinder. The SE evaluation included 810 eyes of 405 patients, with both eyes of each patient in the same SE range. Uncorrected vas for distance, intermediate and near, were very similar in all SE groups. Conclusion: Residual cylinders up to 0.75 D after the implantation of the Tecnis Symfony IOL have a very mild impact on monocular and binocular va. The Tecnis Symfony IOL shows a good tolerance to unexpected refractive surprises and thus a better “sweet spot”.

Dear Editor, ManybacterialproteinsareinvolvedinAgrobacterium- mediated plant transformation. by contrast, relatively little is known about plant proteins that play key roles in transformation. Some of these host proteins interact with Virulence effector proteins, including VirE2, that are transferred from Agrobacterium to plants （gelvin, 2010; Pitzschke and Hirt, 2010）. A recent study indicated that the plant protein SUPPRESSOR OF g2 ALLELE OF SKP1 （SgT1）, a co-chaperone of heat shock protein 90 （HSP90）, is required for Agrobacterium-mediated transformation （Anand et al., 2012）. These studies suggested the involvement of HSPg0 in Agrobacterium-mediated transformation.

The constitutively-expressed cyclooxygenase 1（COX-1） and the inducible COX-2 are both involved in the conversion of arachidonic acid（AA） to prostaglandins（Pgs）.However,the functional roles of COX-1 at the cellular level remain *** hypothesized that by comparing differential gene expression and eicosanoid metabolism in lung fibroblasts from wild-type（WT） mice and COX-2^（-/-） or COX-1^（-/-） mice may help address the functional roles of COX-1 in inflammation and other cellular *** to WT,the number of specifically-induced transcripts were altered descendingly as follows：COX-2^（-/-）〉 COX-1^（-/-）〉 WT ＋ IL-1β.COX-1^（-/-） or COX-2^（-/-） cells shared about 50%of the induced transcripts with WT cells treated with IL-1β,*** interactive ＂anti-inflammatory,proinflammatory,and redox-activated＂ signature in the protein-protein interactome map was observed in COX-2^（-/-） *** augmented COX-1mRNA（in COX-2^（-/-） cells） was associated with the upregulation of mRNAs for glutathione S-transferase（gST）,superoxide dismutase（SOD）,NAD（P）H dehydrogenase quinone 1（NQO1）,aryl hydrocarbon receptor（AhR）,peroxiredoxin,phospholipase,prostacyclin synthase,and prostaglandin E synthase,resulting in a significant increase in the levels of PgE2,PgD2,leukotriene b4（LTb4）,PgF（1α）,thromboxane b2（TXb2）,and PgF（2α）.The COX-1 plays a dominant role in shifting AA toward the LTb4 pathway and anti-inflammatory *** to WT,the upregulated COX-1 mRNA in COX-2^（-/-） cells generated an ＂eicosanoid storm＂.The genomic characteristics of COX-2^（-/-） is similar to that of proinflammatory cells as observed in IL-1β induced WT ***-1^（-/-） and COX-2^（-/-） cells exhibited compensation of various eicosanoids at the genomic and metabolic levels.

PURPOSE. Potential vision testing attempts to predict the visual outcome that might be expected as a result of a cataract operation. This report details the clinical utility of critical flicker frequency (CFF) as a potential vision test (PVT).METHODS. CFF thresholds were determined in 31 subjects with age-related idiopathic cataract and no other eye disease, 19 subjects with macular disease (MD) and clear ocular media, and 24 age-matched control subjects. In addition, the CFF technique was administered before cataract surgery in 52 patients and compared with the information provided by presurgical case history and ocular examination alone (ophthalmological judgment [OJ]) and results from two commonly used PVTs (the retroilluminated pinhole and the potential acuity meter). RESULTS. CFF thresholds obtained in the nonsurgical cataract group were unrelated to cataract severity and were similar to those in the control group. In contrast, CFF scores were significantly related to visual acuity (va) in the MD group. In the pre-and postsurgical studies, OJ predicted postoperative va very well in patients with moderate cataract and normal fundi and better than all the PVTs. OJ performed less well in patients with comorbid eye disease and dense cataracts, when information from the PVTs would probably have been useful. CFF provided the most accurate predictions of postoperative va in the small sample of patients with dense cataracts. CONCLUSIONS. CFF was unaffected by cataract, yet sensitive to MD, and provided useful information about the postoperative visual outcome beyond that obtained through history and ocular examination in patients with dense cataracts.

LOD score exclusion analyses for candidate disease susceptibility genes using case-parents design

The focus of almost all the association studies of candidate genes is to test for their importance. We recently developed a LOD score approach that can be used to test against the importance of candidate genes for complex diseases and quantitative traits in random samples. As a complementary method to regular association analyses, our LOD score approach is powerful but still affected by the population admixture, though it is more conservative. To control the confounding effect of population heterogeneity, we develop here a LOD score exclusion analysis using case-parents design, the basic design of the transmission disequilibrium test (TDT) approach that is immune to population admixture. In the analysis, specific genetic effects and inheritance models at candidate genes can be analyzed and if a LOD score is ≤ - 2. 0, the locus can be excluded from having an effect larger than that specified. Simulations show that this approach has reasonable power to exclude a candidate gene having small genetic effects if it is not a disease susceptibility locus (DSL) with sample size often employed in TDT studies. Similar to association analyses with the TDT in nuclear families, our exclusion analyses are generally not affected by population admixture. The exclusion analyses may be implemented to rule out candidate genes with no or minor genetic effects as supplemental analyses for the TDT. The utility of the approach is illustrated with an application to test the importance of vitamin D receptor (VDR) gene underlying the differential risk to osteoporosis.

Medically tailored meals for the management of symptomatic ascites:the SALTYFOOD pilot randomized clinical trial

background:Ascites is a costly,morbid complication of *** a low-sodium diet is central to the clinical management of ascites,its efficacy is limited by poor *** aimed to determine the feasibility and impact of lowsodium medically tailored meals(MTM)***:We enrolled 40 persons with cirrhosis and ascites at the time of a paracentesis in a 12-week,1:1 randomized trial of standard of care(SOC)(low-sodiumdiet educational handout)or MTM withg of sodium,>2,100 kcal,and>80 g of protein including a nocturnal protein *** determined the proportion of eligible candidates recruited and adherence to *** primary outcome was the number of paracenteses performed during weeks *** also collected ascites-specific quality-of-life(ASI-7)***:The median age of the enrolled subjects was 54(IQR,47-63)years,46%were female,with median MELD-Na 18(IQR,11-23)and albumin 2.7(IQR,2.5-3.3)g/*** baseline,subjects reported a median of two(IQR,1-3)paracenteses in the prior 4 *** to the meal schedule was excellent save for when hospitalizations *** 12 weeks,patients in the MTM arm required fewer paracenteses per week than those in the SOC group[median(IQR):0.34(0.14-0.54)vs 0.45(0.25-0.64)].During the trial,four(20%)SOC patients died,whereas two(10%)died and one(5%)was transplanted in the MTM ***-specific quality of life improved to a greater degree in the MTM arm compared to the SOC arm,by 25%(IQR,-11%to 61%)vs 13%(IQR,-28%to 54%).Conclusion:A trial of MTM for persons with ascites is feasible and potentially *** arms experienced benefits,highlighting the role for improved education and closer monitoring in this challenging condition.
