The search for new biomarkers predictive of type 2 diabetes currently constitutes a research avenue in Bioclinical. Total homocysteine remains a preferred targ.t due to its involvement in the occurrence of deg.nerative complications in type 2 diabetics. The aim of this work was to study hyperhomocysteinemia and other biochemical markers associated with T2D in the Cong.lese population. This was an analytical case-control study carried out between October 2022 and October 2023. The study population consisted of 150 subjects including.100 T2D patients and 50 control subjects. The main clinical data were collected on a pre-established form. Homocysteine determination was carried out by the sandwich ELISA method. The other biochemical markers were measured by colorimetric enzymatic methods. Hyperhomocysteinemia was present in 27.3% (41/150) of the entire study population. Type 2 diabetics had a frequency of hyperhomocysteinemia of 36% (36/100) and control 10% (5/50) (p = 0.001). The mean hyperhomocysteinemia concentration was 31.9 μmol/l with extremes rang.ng.from 18 to 103 μmol/l. Means of biolog.cal markers between diabetics and controls showed a statistically sig.ificant difference (p = 0.01). The risk factors associated with this HHcy were: sex (OR = 3.5), ag. (OR = 9.4), sedentary lifestyle (OR = 3.4) and g.ycosylated hemog.obin (OR = 12) with a p-value g.est that hyperhomocysteinemia can be considered as a predictive biomarker in the bioclinic of Cong.lese type 2 diabetic patients.

Acute pancreatitis(AP)is a leading.cause of g.strointestinal-related hospitalizations in the United States,resulting.in 300000 admissions per year with an estimated cost of over$2.6 billion *** severity of AP is determined by the presence of pancreatic complications and end-org.n *** moderate/severe pancreatitis can be associated with sig.ificant morbidity and mortality,the majority of patients have a mild presentation with an uncomplicated course and mortality rate of less than 2%.Despite favorable outcomes,the majority of mild AP patients are admitted,contributing.to healthcare cost and *** this Editorial we review the performance of an emerg.ncy department(ED)pathway for patients with mild AP at a tertiary care center with the g.al of reducing.hospitalizations,resource utilization,and costs after several years of implementation of the *** discuss the clinical course and outcomes of mild AP patients enrolled in the pathway who were successfully discharg.d from the ED compared to those who were admitted to the hospital,and identify predictors of successful ED discharg. to select patients who can potentially be triag.d to the *** conclude that by implementing.innovative clinical pathways which are established and reproducible,selected AP patients can be safely discharg.d from the ED,reducing.hospitalizations and healthcare costs,without compromising.clinical *** also identify a subset of patients most likely to succeed in this pathway.

Urban ag.lomeration is a serious concern due to its hig. energ. usag. and impact on the local *** countries strive to determine the development path to optimize energ. *** present study aims to examine the local climatic zones(LCZs)performance in warm and humid climate throug. a multi-objective approach for the residential *** performance is assessed by evaluating.the urban microclimate and cooling.load consumption for both summer and winter months using.binomial log.stic *** study concludes that LCZ 2_(3)(compact mid-rise with open low-rise)and LCZ 6_(B)(open low-rise with scattered trees)perform better for 80%and 50%of total hours in warm and humid *** also proves the presence of sig.ificant performance differences between mid-rise and low-rise *** intra-zonal differences between the climatic variables are hig.er than the inter-zonal differences due to the impact of land surface temperature(LST).The hig. aspect ratio and low sky view factor of LCZ 2_(3) help the residents in that morpholog. in enhancing.better thermal comfort and reducing.cooling.load *** present study contributes to building.reg.lation policymakers by providing.information on the suitable morpholog. for warm and humid climate.

Compartmentalization of the inflammatory response during.bacterial sepsis and severe COVID-19

Acute infections cause local and systemic disorders which can lead in the most severe forms to multi-org.n failure and eventually to death. The host response to infection encompasses a larg. spectrum of reactions with a concomitant activation of the so-called inflammatory response aimed at fig.ting.the infectious ag.nt and removing.damag.d tissues or cells, and the anti-inflammatory response aimed at controlling.inflammation and initiating.the healing.process. Fine-tuning.at the local and systemic levels is key to preventing.local and remote injury due to immune system activation. Thus, during.bacterial sepsis and Coronavirus disease 2019 (COVID-19), concomitant systemic and compartmentalized pro-inflammatory and compensatory anti-inflammatory responses are occurring. Immune cells (e.g., macrophag.s, neutrophils, natural killer cells, and T-lymphocytes), as well as endothelial cells, differ from one compartment to another and contribute to specific org.n responses to sterile and microbial insult. Furthermore, tissue-specific microbiota influences the local and systemic response. A better understanding.of the tissue-specific immune status, the org.n immunity crosstalk, and the role of specific mediators during.sepsis and COVID-19 can foster the development of more accurate biomarkers for better diag.osis and prog.osis and help to define appropriate host-targ.ted treatments and vaccines in the context of precision medicine.
