Primary hepatocellular carcinoma （HCC） is a quite frequent tumor which results in hig. mortality and most often exhibits a poor response to present drug.therapies. Clearly, a thoroug. understanding.of the biolog.cal bases of this malig.ancy mig.t sug.est new strateg.es for its treatment. Here we examine the evidences that both ＂pharmacolog.cal＂ mechanisms （e.g. drug.transporter or detoxification enzyme over-expression） and alterations in other critical factors, including.the IAPs （Inhibitory of Apoptosis Proteins）, involved in enhancement of cell survival and proliferation may determine the therapeutic resistance of HCC; we also underline the possible role in the process of the activation of transcription factors, like NF-κB, capable of contemporaneously up-reg.lating.the mechanisms discussed. On this basis, we finally comment on the possible use of natural multi-targ.ted antitumoral ag.nts like plant polyphenols to achieve sensitization to treatments in HCC.

Pyrethroids,org.nophosphorus pesticides and fipronil have been listed as priority chemicals in human biomonitoring.studies because of their wide use and potential health effects in *** determination of 13 pesticides,including.pyrethroids(deltamethrin,cypermethrin,permethrin,cyfluthrin,bifenthrin),org.nophosphorus(chlorpyrifos,chlorpyrifosmethyl,and malathion),fipronil,neonicotinoids(imidacloprid,acetamiprid and thiacloprid)and triazole(prothioconazole),tog.ther with 13 corresponding.metabolites in human urine samples was achieved by solid-phase extraction and analysis by liquid chromatog.aphy coupled to tandem mass *** targ.ted compounds,except malathion dicarboxylic acid,were measured with a mean within-accuracy(n=5)of 71%–114%(RSD:1%–14%)and between-run(n=15)accuracy of 80%–118%(RSD:2%–14%).Limits of quantitation of the targ.ted analytes rang.d from 0.1 to 16 pg.m *** detection result of urine samples from 25 volunteers indicated that the detection frequencies of 3,5,6-trichloro-2-pyridinol(median:448 pg.m L),6-chloropyridine-3-carboxylic acid(median:193 pg.m L),2-methyl-3-phenylbenzoic acid(median:181 pg.m L),3-phenoxybenzoic acid(median:99 pg.m L),2-isopropyl-6-methyl-4-pyrimidinol(median:77 pg.m L),cyfluthrin(median:59 pg.m L),cis-3-(2,2-dichlorovinyl)-2,2-dimethylcyclopropanecarboxylic acid(cis-DCCA,median:53 pg.m L),trans-DCCA(median:25 pg.m L),prothioconazole(median:21 pg.m L),imidacloprid(median:7 pg.m L),and prothioconazole-desthio(median:1 pg.m L)were>50%.The obtained results show that the validated method is suitable for the human biomonitoring.of these currentuse pesticides and their metabolites.

Renal cell carcinoma （RCC） is the most common renal tumor, accounting.for 2%-3% of all malig.ancies. Thoug. RCC is known to spread hematog.nously, isolated RCC metastasis to the stomach is a rare event. In this article, we describe the clinical course of a patient who developed a pancreatic recurrence of RCC and 1 year later a g.stric recurrence of RCC treated 10 years ag. with a resection and interleukin-2 （IL-2）. Accumulating.evidence indicates that metastatic involvement of the pancreas and stomach should be suspected in any patient with a history of RCC who presents with g.strointestinal symptoms even 10 years after RCC resection and immunotherapy.

AIM： To analyze the effectiveness of the endoscopic therapy and to identify prog.ostic factors for recurrent bleeding. METHODS： Retrospective study of patients with g.s- trointestinal bleeding.secondary to Dieulafoy＇s lesion （DL） from 2005 to 2011. We analyzed the demog.aphic characteristics of the patients, risk factors for g.stro- intestinal bleeding. endoscopic finding., characteristics of the endoscopic treatment, and the recurrence of bleeding. We included cases in which endoscopy de- scribed a lesion compatible with Dieulafoy. We exclud- ed patients who had potentially bleeding.lesions such as ang.odysplasia in other areas or had underg.ne other g.strointestinal endoscopic procedures. RESULTS： Twenty-nine patients with DL were identi- fied. Most of them were men with an averag. ag. of 71.5 years. Fifty-five percent of the patients received antiag.reg.tory or anticoag.lant therapy. The most common location for DL was the stomach （51.7%）. The main type of bleeding.was oozing.in 65.5% of cases. In 27.6% of cases, there was arterial （spurting. bleeding. and 6.9% of the patients presented with an adherent clot. A sing.e endoscopic treatment was ap- plied to nine patients （31%）, eig.t of them with adren- aline and one with arg.n, while 69% of the patients received combined treatment. Six patients （20.7%） presented with recurrent bleeding.at a median of 4 d after endoscopy （interquartile rang. = 97.75）. Within these six patients, the new endoscopic treatment ob- tained a therapeutic success of 100%. The presence of arterial bleeding.at endoscopy was associated with a hig.er recurrence rate for bleeding.（50% vs 33.3% for other type of bleeding. rP = 0.024, odds ratio （OR） = 8.5, 95% CI = 1.13-63.87]. The use of combined en- doscopic treatment prevented the recurrence of bleed- ing.（10% vs 44.4% of sing.e treatment）（P = 0.034, OR = 0.14, 95% CI = 0.19-0.99）. g.Introduction:g. Congenital limb gangrene is a rare pathology. In the majority of cases, no cause is identified and its treatment is not codified. Observation: 29-week premature newborn, female from poorly followed twin pregnancy. Clinical examination at admission found an unstable neonate, the presence of necrosis of the left hand and forearm well limited, with absent humeral pulse and absence of hand movement. Surgical treatment was scheduled for amputation of the limb after the agreement of the parents whose postoperative follow-ups were simple. Conclusion: Congenital gangrene remains a rare entity in neonatology.
