Anti-Sperm Antibodies: Prevalence of Ig A and Ig g Positive Serology among Infertile Patients in Kisangani, democratic Republic of Congo

Introduction: Infertility in a couple is a stressful situation. Among the causes is the presence of antisperm antibodies that play a harmful role in reducing fertility and negatively influencing in vitro fertilization. The aim of this study is to determine their prevalence among infertile patients in Kisangani. Patients and methods: We conducted an analytical cross-sectional study in health facilities in the city of Kisangani. Of the 1379 patients who consulted for conception, 311 were tested for antisperm antibodies by ELISA. Results: Of the 311 patients, 121 tested positive (38.91%). Women were significantly more likely to be positive (p-value = 0.013). Ig g was positive in 32.48% of patients and Ig A in 10.93%. Age was a significant predictor of the number of positive patients (p-value d to patients of ≤30 years of age, having 31 to 40 years of age or 41 years of age and older increased respectively 2 and 3 times the risk of being positive. In secondary infertility, women are twice as likely to be positive. Conclusion: The prevalence of antisperm antibodies is very high among infertile patients in Kisangani with predominance among women. Older age increases the risk of positivity. This test should therefore be integrated into infertility investigations as often as possible.

Introduction: Antisperm antibodies (ASA) prevalence is high in Kisangani. ASA are one of the male factors of infertility that can reduce spermatozoa motility and cervical penetration or prevent acrosomal reaction and even alter embryo development. This study aims to determine the risk factors of ASA positivity among infertile men in Kisangani. Patient and Method: We conducted a case-control study during a period of 2 years in Kisangani on 111 men who consulted for conception and tested for ASA by ELISA. We did a simple pairing that concerned only the type of infertility. For a better balance between the number of cases and controls per stratum to provide better accuracy in the adjusted OR estimate, we performed a 1:1 match. Results: This study showed that businessmen [ORa = 5.0000(1.2452 - 20.0767)] and jobless [ORa = 5.8125(1.1477 - 29.4367)] were at risk of being positive for ASA. Violent blow to testicles [ORa = 6.7391(1.3455 - 33.7545)], cure of hernia [ORa = 4.3478(1.0722 - 17.6299)], Chlamydia infection [ORa = 4.7125(1.3405 - 16.5665)], leucospermia [ORa = 4.1429(1.0406 - 19.7155)] and presence of Staphylococcus aureus [ORa = 3.4444(0.6410 - 18.5082)] were associated to positive ASA. Conclusion: This study shows that factors that may lead to risky sexual behavior and physical or infectious trauma are important risk factors for ASA positivity. It is therefore necessary to search for them systematically during male infertility in order to guide the search for ASA.

Introduction: Pregnant adolescents are at an elevated risk of premature delivery. In the context of low levels of education and limited resources, several difficulties are encountered in determining gestational age. These include a lack of knowledge of the date of the last menstrual period, failure to perform first-trimester ultrasound, and absence of an electroencephalogram. In such circumstances, the utilisation of a morphological score, analogous to the Finnstrom score, to ascertain gestational age would appear to be a more accessible and straightforward approach. This study aimed to assess the accuracy of the Finnström score in newborns of teenage mothers, where the date of the last menstrual period may be subject to inaccuracy, in order to validate the diagnosis of preterm delivery. Methods: This was an analytical cross-sectional design of 87 newborns of teenage mothers, multicenter, conducted in the city of Kisangani, democratic Republic of the Congo (dRC) with prospective data collection. Results: This study involved 87 newborns born to adolescent mothers. The incidence of premature delivery, as determined by the date of the last menstrual period, was observed to be 17.6% among teenage girls and 5.3% among adults. As indicated by the Finnström morphological score and early ultrasound dating, the incidence of preterm delivery was notably elevated, at 32.2% and 37.7%, respectively. The correlation between gestational age according to the date of the last menstrual period and gestational age according to early ultrasound dating was low (0.338), while there was a satisfactory correlation between gestational age according to the Finnström morphological score and early ultrasound dating (0.828). Conclusion: The Finnström morphological score represents a valuable tool for accurately determining gestational age, thereby validating the diagnosis of preterm delivery in adolescents, who are prone to inaccuracies in determining the date of the last menstrual period. In the democratic Republic of the Congo, the sickle cell trait carriage is estimated at 25%. Routine neonatal screening is not a common practice, leading to a very late diagnosis. In this study, the screening of pregnant women was assessed as well as their attitudes. This is an analytical cross-sectional study conducted in 245 pregnant women, sampled by convenience in four hospitals in Kisangani city (democratic Republic of Congo) and screened using the sickle SCAN® test, from February 1 to July 31, 2019. The sensitivity and specificity of the latter were determined using liquid chromatography coupled with mass spectrometry as the gold standard. The attitudes of 240 pregnant women without previous screening history were assessed upon the announcement of the results. The sensitivity of screening for hemoglobin (Hb) AA and Hb AS was 96.69% and 98.39%, respectively;while the specificities were 99.43% and 96.32%, respectively. The Kappa coefficient (κ) was excellent. Concerning attitudes, Hb SS pregnant women and 55.17% of AS pregnant women worried when the results relating to their hemoglobin status were announced. The sickle SCAN® test was found reliable for sickle cell disease screening in Kisangani. The announcement of the results, mainly positive, raises worry among pregnant woman. Therefore, we recommend the involvement of a clinician psychologist for pre-screening counselling and for results announcement, as well as early newborns and unmarried teenage girls screening.

g>Introduction:g> Induction of labor has become an increasingly utilized obstetric intervention in developed countries. It contributes to reduce the maternal and perinatal morbidity and mortality. dinoprostone derivatives are often used under cardiotocography. In poor countries, health structures have neither fetal monitoring, nor means of preserving dinoprostone derivatives. Misoprostol therefore constitutes an alternative. This study seeks to establish the efficacy and safety of oral Misoprostol and to assess maternal and perinatal prognosis in an area with limited resource. g>Methods: g>This is a multi-center clinical trial, conducted in 3 hospitals centers in Kisangani, Tshopo Province/democratic Republic of Congo from december 1, 2020 to May 31, 2021. Our sample was constituted with 68 pregnant women with term pregnancy. Fifty micrograms of Misoprostol was administered orally. The data were collected prospectively, their encoding was carried out on an Excel sheet 2013 and their analysis carried out using the EPI INFO software. g>Results:g> The average age of the pregnant women was 23.17 ± 5.76 years. The average parity was 1.02 ± 1.2. The average body mass index (BMI) was 24.98 ± 2.55 Kg/m2. 66 (97.1%) patients had received a maximum of 3 doses of Misoprostol. Out of the 16 parturients who received 3 doses or more, 11 (68.8%) had a BMI ≥ 25. The average duration of labor was 16.03 ± 7.99 hours. 66 (97.1%) pregnant women had delivered vaginally. 57 (83.8%) patients delivered within 24 hours. Out of the 11 patients who delivered after 24 hours, 7 (63.6%) had a BMI ≥ 25. The induction failure rate was 2.9% (2 patients). 2 (2.9%) fetuses had presented pathological modification of fetal heart rate (FHR). 2 (2.9%) patients had undergone cesarean section. g>Conclusion:g> Labor induction with oral Misoprostol is effective and associated with low maternal and perinatal morbidity. We show that when A is a separable C^(*)-algebra,every countably generated Hilbert A-module is projective(with bounded module maps as morphisms).We also study the approximate extensions of bounded module *** the case where A is aσ-unital simple C^(*)-algebra with strict comparison,and every strictly positive lower semicontinuous affine function on quasitraces can be realized as the rank of an element in the Cuntz semigroup,we show that the Cuntz semigroup is equivalent to unitarily equivalent classes of countably generated Hilbert A-modules if and only if A has stable rank one.

On Sawada-Kotera and Kaup-Kuperschmidt integrable systems

To obtain new integrable nonlinear differential equations there are some well-known methods such as Lax equations with different Lax representations. There are also some other methods that are based on integrable scalar nonlinear partial differential equations. We show that some systems of integrable equations published recently are the M2-extension of integrable such scalar equations. For illustration, we give Korteweg–de Vries, Kaup-Kupershmidt, and SawadaKotera equations as examples. By the use of such an extension of integrable scalar equations, we obtain some new integrable systems with recursion operators. We also give the soliton solutions of the systems and integrable standard nonlocal and shifted nonlocal reductions of these systems.
