Factors Affecting Japanese Pond Smelt (<i>Hypomesus nipponensis</i>) Stock Trajectory in Lake Kasumigaura and Kitaura

The Japanese pond smelt (Hypomesus nipponensis) stock has been observed to fluctuate quite rigorously over the years with sustained periods of low catch in Lake Kasumigaura and Kitaura of the Ibaraki prefecture, Japan which would adversely affect the socioeconomic livelihood of the local fishermen and fisheries industry. This study was aimed at determining the factors affecting the stock fluctuation of the pond smelt through the different years in the two lakes. Through exploratory analysis it was found that the pond smelt had significant relationship with total phosphorus (TP) level in both lakes. The global mean land and ocean temperature index (LOTI) was also found to be indirectly related to the pond smelt stock in lake Kasumigaura and Kitaura at the latitude band of 24°N to 90°N (l). Both TP and LOTI had inverse relationship with pond smelt trajectory in both lakes. For both Lake Kasumigaura and Kitaura, TP for the individual lakes and LOTI (l) were used as independent variables using generalized linear model and response surface methods for modeling the stock dynamics of the pond smelt in the two lakes. Model selection was based on significant parameter estimates (p < 0.05), Akaikes Information Criterion and R2 values. Phosphorus loading is an indication of increasing anthropogenic activities in the surrounding area having negative impact on the pond smelt population. When management decisions are being made regarding pond smelt fishery and sustainability plans in the Ibaraki prefecture, the effects of TP and LOTI should be taken into account. Future research needs to be directed towards deeper understanding the mechanisms by which TP and LOTI affect pond smelt population in Lake Kasumigaura and Kitaura for more effective management.

钢渣与土壤的混合物可以用来改善和修复沿海环境。钢渣中所含亚铁离子对各种海洋植物生长起到重要的作用。利用钢渣修复沿海环境中重要的一点是分析钢渣中所含亚铁离子进入海水后的溶解原理。为研究钢渣各个元素在海水中的溶解行为,在室温下对各种合成渣进行了不同渣/海水质量条件下的振荡实验,结果表明钢渣的碱度对溶解反应的影响远大于渣/水质量比对其的影响。利用溶解度图表对二价铁离子的溶解原理进行了讨论,向亚铁离子溶液中添加葡萄糖酸形成葡萄糖酸铁从而使亚铁离子稳定存在,但当葡萄糖酸铁达到最高浓度后会逐渐分解。

A study on sleep architecture in patients with chronic respiratory failure under long-term oxygen therapy—Focused on the influence of ventilatory failure (high CO<sub>2</sub>) elements on the patient’s sleep architecture

Sleep disturbance related symptoms are common in patients with long-term oxygen therapy (LTOT). Essentially, there were only few previous reports about the sleep architecture in patients with respiratory disease, such as chronic obstructive pulmonary disease (COPD). This study aims to clarify the objective sleep state and the elements that affect sleep architecture in Chronic Respiratory Failure (CRF) patients with focus on clinical cases of chronic hypercapnia. 13 subjects with chronic respiratory failure were enrolled in the study. All the subjects were pre-evaluated by pulmonary function test and Arterial blood gas analysis (ABG) including exercise testing. Polysomnography (PSG) test was performed in each subject with supplemental oxygen. The estimated base line PaCO2 value that reflects overall PaCO2 including sleep period was calculated using equation of PaCO2[2.4×(HCOˉ3)-22]from obtained ABG value just before PSG test. 6 subjects were classified as hypercapnic group (base line PaCO2 ≥ 45 mmHg) and 7 subjects were non-hypercapnic group (base line PaCO2 < 45 mmHg). Latency persistent sleep of PSG data was significant higher in patients with hypercapnic than non-hypercapnic (p < 0.01). Periodic Limb Movement was seen in 23.6% of the subjects, however there was no contribution for arousals. Other PSG data include mean SpO2 were no significant difference. This study suggests that patients with estimated hypercapnia had more disturbed sleep architecture especially significant loss of sleep latency than non-hypercapnic patient with chronic respiratory failure under LTOT. Nocturnal PaCO2 level or ventilatory function may contribute to sleep disturbance in patients with estimated hypercapnia during LTOT.

已知两位置交换分子运动速率对于静态13CNMR谱的影响是由一简单代数关系决定，它已成为分析固体中分子运动的常规方法．慢速的魔角旋转可以保留边带，所产生的边带型原则上也可用来分析分子运动，只是缺乏象静态谱那样的简单的关系，以至于在实际应用上是不可行的．本文给出作者所发现的存在分子运动时魔角旋转核磁共振自由诱导衰减信号的解析表达式的理论推导，提供分析固体分子运动的理论谱计算公式．我们选择二甲矾（dimethylsulfone）作为模型化合物，记录其不同温度下13CNMR谱，又计算了两个甲基做两位置交换运动的理论谱．理论谱与实验谱定性一致．定量上，边带强度存在一些差别，但边带宽度符合很好．通过理论谱与实验谱的比较，得到交换速率与温度的对应关系．虽然本文限于化学位移各向异性相互作用两位置交换问题，但容易推广到偶极作用多位置交换问题．

Laparoscopic cholecystectomy(LC) does not require advanced techniques, and its performance has therefore rapidly spread worldwide. However, the rate of biliary injuries has not decreased. The concept of the critical view of safety(CVS) was first documented two decades ago. Unexpected injuries are principally due to misidentification of human factors. The surgeon's assumption is a major cause of misidentification, and a high level of experience alone is not sufficient for successful LC. We herein describe tips and pitfalls of LC in detail and discuss various technical ***, based on a review of important papers and our own experience, we summarize the following mandatory protocol for safe LC:(1) consideration that a high level of experience alone is not enough;(2) recognition of the plateau involving the common hepatic duct and hepatic hilum;(3) blunt dissection until CVS exposure;(4) Calot's triangle clearance in the overhead view;(5) Calot's triangle clearance in the view from underneath;(6) dissection of the posterior right side of Calot's triangle;(7) removal of the gallbladder body; and(8) positive CVS exposure. We believe that adherence to this protocol will ensure successful and beneficial LC worldwide, even in patients with inflammatory changes and rare anatomies.

开发了数学模型,以研究电磁制动(EMBR)对薄板坯连铸结晶器涡流的影响。结晶器内熔融钢水由结晶器浸入式水口(SEN)的两个侧孔流出。采用低雷诺数k-ε紊流模型计算有效粘度。数学模拟表明,即使在水口外形对称和注流稳定的条件下也能产生非对称的涡流。模拟中的非对称流动由数据冗余引起,数据冗余代替了实际生产中的水口堵塞、水口不对中和随机湍流等。涡流强度取决于由水口出流角和拉速决定的结晶器表面流速。施加静态磁场可以显著抑制涡流和改变涡流形态,但不能将其彻底消除。表面流速随线圈电流的减小而显著降低,同时结晶器内的波高明显变小。此外,浸入式水口附近由涡流引起的失稳区逐渐变得稳定。结晶器下部的下降流流速受到抑制并形成活塞流。

AIM:To investigate thrombotic microangiopathy (TMA)in liver transplantion,because TMA is an infrequent but life-threatening complication in the transplantation field. METHODS:A total of 206 patients who underwent living-donor liver transplantation (LDLT) were evaluated,and the TMA-like disorder (TMALD) occurred in seven recipients. RESULTS:These TMALD recipients showed poor outcomes in comparison with other 199 recipients. Although two TMALD recipients successfully recovered,the other five recipients finally died despite intensive treatments including repeated plasma exchange (PE) and re-transplantation. Histopathological analysis of liver biopsies after LDLT revealed obvious differences according to the outcomes. Qualitative analysis of antibodies against a disintegrin-like domain and metalloproteinase with thrombospondin type 1 motifs (ADAMTS-13) were negative in all patients. The fragmentation of red cells,the microhemorrhagic macules and the platelet counts were early markers for the suspicion of TMALD after LDLT. Although the absolute values of von Willebrand factor (vWF) and ADAMTS-13 did not necessarily reflect TMALD,the vWF/ADAMTS-13 ratio had a clear diagnostic value in all cases. The establishment of adequate treatments for TMALD,such as PE for ADAMTS-13 replenishment or treatments against inhibitory antibodies,must be decided according to each case. CONCLUSION:The optimal induction of adequate therapies based on early recognition of TMALD by the reliable markers may confer a large advantage for TMALD after LDLT.

Hepatitis C virus enhances incidence of idiopathic pulmonary fibrosis

AIM: To investigate the cumulative development incidence and predictive factors for idiopathic pulmonary fibrosis in hepatitis C virus (HCV) positive patients. METHODS: We studied 6150 HCV infected patients who were between 40-70 years old (HCV-group). Another 2050 patients with hepatitis B virus (HBV) were selected as control (HBV-group). The mean observation period was 8.0 ± 5.9 years in HCV-group and 6.3 ± 5.5 years in HBV-group. The primary goal is the development of idiopathic pulmonary fibrosis (IPF) in both groups. The cumulative appearance rate of IPF and independent factors associated with the incidence rate of IPF were calculated using the Kaplan- Meier method and the Cox proportional hazard model. All of the studies were performed retrospectively by collecting and analyzing data from the patient records in our hospital. RESULTS: Fifteen patients in HCV-group developedIPF. On the other hand, none of the patients developed IPF in HBV-group. In HCV-group, the cumulative rates of IPF development were 0.3% at 10th year and 0.9% at 20th year. The IPF development rate in HCV-group was higher than that in HBV-group (P = 0.021). The IPF development rate in patients with HCV or HBV was high with statistical significance in the following cases: (1) patients ≥ 55 years (P < 0.001); (2) patients who had smoking index (package per day × year) of ≥ 20 (P = 0.002); (3) patients with liver cirrhosis (P = 0.042). CONCLUSION: Our results indicate that age, smoking and liver cirrhosis enhance the development of IPF in HCV positive patients.

A Phase II Study of Erlotinib in Patients with Previously Treated Non-Small Cell Lung Cancer

Background: Erlotinib has been reported to be effective for the treatment of non-small cell lung cancer (NSCLC). To evaluate the efficacy and safety of erlotinib under conditions similar to daily clinical practice, a phase II trial was conducted in Japanese patients with previously treated NSCLC. Methods: The eligibility criteria were stage IIIB/IV NSCLC, a performance status (PS) of 0 - 2, and previous treatment with 1 - 2 non-EGFR-TKI regimens. Patients received erlotinib (150 mg/day) orally until disease progression or intolerable toxicity occurred. The primary endpoint was the objective response rate (ORR). In addition, the disease control rate (DCR), progression-free survival (PFS), overall survival (OS), safety, and EGFR gene mutation status were evaluated. Results: Thirty-eight patients were enrolled, and 37 patients were evaluated. The median age was 69 years (range, 50 - 80 years). Patient characteristics were as follows: 26 were male and 11 were female;12 had a PS of 0, 20 had a PS of 1, and 5 had a PS of 2;and 26 had adenocarcinoma, and 11 had non-adenocarcinoma histology. The ORR and DCR were 21.6% (95% confidence interval [CI], 11.4% - 37.2%) and 54.1% (95% CI, 35.9% - 66.6%), respectively. Twenty-seven patients could be evaluated for EGFR gene status (12, mutated;15, wild-type). The ORR for EGFR-mutated patients was 41.7%, while that for patients with wild-type EGFR was 13.3%. The median PFS was evaluated as 4.4 months (95% CI, 2.2 - 10.7 months). The median OS was 14.9 months (95% CI, 9.2 months - not reached). Common adverse events were tolerable skin toxicities, diarrhea, and stomatitis. In addition, interstitial lung disease occurred in 8.1% of patients. Conclusion: As efficacy and safety were similar to previous studies, erlotinib was found to be effective for Japanese patients with previously treated NSCLC in clinical practice.
