Prevalence and genotype distribution of hepatitis B virus among migrant workers in Lombok Island, Indonesia

Objective:To examine the potential risk of hepatitis B virus(HBV)spread in Indonesia by migrant workers,based on the molecular characteristics of HBV ***:Sera collected from migrant workers traveling to their destination countries(pre-migrant workers)and those returning to Indonesia(post-migrant workers)were screened for HBsAg by ELISA,followed by HBV DNA detection by PCR and(sub)genotype/subtype determination according to surface region and whole genome ***:Of 87 pre-migrant workers,15(17.24%)were HBsAgpositive,whereas 15(12.10%)of 124 post-migrant workers were HBs Ag *** genotype analysis based on the S region showed that HBV-B3/adw2 was predominant(96.15%,25/26)whereas 3.85%(1/26)of isolates were HBV-C3/adrq+.Whole genome sequencing of selected strains and phylogenetic tree analysis identified subgenotype B7 in three samples previously categorized as subgenotype B3 based on S region analysis,supporting a recent argument that subgenotypes B5/B7/B8/B9 could be considered as a quasi-subgenotype of ***:A high prevalence of HBsAg carriers was detected among migrant workers from Lombok Island,with no significant difference in prevalence between before and after returning to *** strains were classified into genotypes common in Indonesia,and the results suggested that migrant workers are not a risk factor for HBV transmission into Indonesia.

Progress in the R&D Experiments About a Novel Method of Electr Scattering off Short-lived Nuclei

A novel method has been established to realize the experiment of electron scattering off short-lived nuclei. The method was based on the well known ＂ion trapping＂ phenomenon in electron storage rings. In the R^D experiments at Kyoto University, stable nucleus, 133^Cs, was employed as the target nucleus. The luminosity of scattering experiment was nearly 1026 cm^-2s^-1 at electron beam current around 75 mA. The angular distribution of elastically scattered electrons from trapped Cs ions was measured and the result was well fitted by theoretical calculation. It was indicated that higher luminosity can be reached with larger electron beam current.

The Success of Healthcare Providers Who Fought the Epidemic of the Meiji Era in Mizunami City, Gifu Prefecture, Japan

Introduction: Knowing an epidemic existed in Mizunami city and knowing the healthcare provided to overcome the disease may give hints on how the epidemic was handled. Aim: To examine the literature on the epidemic that occurred in the Meiji era and the contributions of healthcare providers who were active in the history of Mizunami city. Method: This is a historical study, mainly using literature. In collaboration with the Mizunami City Board of Education, we searched for materials stored by Mizunami city. In addition, with the cooperation of the local residents of the healthcare providers who were active in Mizunami city, we collected materials in reference to our purpose. Result: The outbreak of the epidemic near Mizunami city was seen as a fight against smallpox. In addition, cholera, dysentery, Spanish flu, and typhoid fever also occurred. Vaccinations were carried out in the Tono region. Healthcare providers who were active in the Meiji era include six doctors and two midwives. Discussion: In the future, it will be necessary to provide more in-depth evidence through surveys, such as interviews with the cooperation of relatives of the active healthcare providers. Conclusion: In the Meiji era, in the vicinity of Mizunami city, doctors and midwives actively fought against epidemics, such as smallpox and other infectious diseases.

Basket pattern blood flow signals discovered in a case of splenic hamartoma by power Doppler ultrasonography

We present the gray-scale ultrasonography （GSUS）, power Doppler ultrasonography （PDUS）, abdominal computed tomography （CT）, and magnetic resonance imaging （MRI） findings for a case of splenic hamartoma in a 27-year-old man, showing a φ 50 mm homogeneous, iso- and hypo-echoic splenic mass with evidence of a small plural cystic lesion. This splenic hamartoma showed increased vascularity on power Doppler sonograms. PDUS showed multiple circular blood flow signals inside the mass （i.e. a basket pattern）, which was consistent with the small plural cystic lesion shown by GSUS. Spectral analysis also confirmed arterial and venous flow. CT scans showed that the mass had lowdensity relative to the normal spleen and MRI showed that the mass was isodense, relative to the normal spleen. Therefore, CT and MRI are not useful for the diagnosis of splenic hamartoma. Ultrasonography can be used to diagnose splenic hamartoma without administration of a contrast matedal and therefore is an indispensable method for the diagnosis of splenic hamartoma.

Literature Review on the Significance of Death Conferences for Nurses in Japan

Aim: To identify evidence in the literature on the significance of death conferences for nurses. Method: This is an integrative literature review, consisting of scientific articles published on “Ichushi-Web”, an Internet-based retrieval service by the Japan Medical Abstracts Society (JAMAS), PubMed, Medline, Cochrane library, and Google Scholar for ‘the whole year,’ combining the keywords “death conference”, “nurse”, and “KJ method”. The significance of death conferences was reviewed by comparison of prior qualitative studies analyzed by the KJ method. Result: Seven studies, all from Japanese, were identified. The significance of death conferences showed “refraction”, “cooperation”, “directionality of nursing”, “caring for patient”, “caring for family”, “specific methods of nursing”, and “feelings of nurses”. Conclusion: The significance of death conferences is considered to be the effect on nurses for amelioration in the quality of patient and family care, development of human relations, and growth as a nurse. The quality of death conferences should be enhanced based on the guidelines for its clear definition.

本文结合安吉拉·卡特的个体经历与社会文化的变迁,以《英雄与恶徒》、《染血之室与其他故事》、《马戏团之夜》作为卡特60年代、70年代、80年代的代表作品来探讨卡特不同年代动物叙事的特点及变迁过程。六十年代的卡特将自己对于神话与专制、核战与生命的思考融入了《英雄与恶徒》的动物叙事中,此时卡特笔下的动物尚处于被动状态,以动物符号及人类之他者的身份登场。及至70年代,基于夏尔·佩罗《鹅妈妈故事集》的启发,她对于女性情欲、野性与文明、父权制与资本主义的思考与政治诉求更集中地体现在短篇小说集《染血之室》的动物叙事中。此时卡特笔下的动物在与人的关系中已逐渐获取了可自主性,动物逐渐成为与人并行的平等物种,甚至成为了人类命运的改变者,在动物与人交互易位、彼此变通的世界形态中暗喻着逃逸出人类文明的隐秘通途。进入二十世纪八十年代的英国发生了巨变,英国社会的整体调性由二战后两党共识下的福利制社会向新自由主义式社会转型,极力反对帝国主义-民族主义神话的卡特将其对于帝国谎言、文明建构与人类理性神话的反思与批判注入《马戏团之夜》的动物叙事中,此时的卡特将动物叙事的复杂性推进到了极致,人与动物作为平等的生命存在共同构成叙事的中心,人类、兽类与半人半兽间的关系也被重新演绎。最终,卡特也得以通过动物叙事展现其作为社会主义者的政治诉求,只有当人类意识到自身也是动物之一时,才能实现生命与社会的平等。

Fatigue and Depression from Early Postpartum to 1 Month among Postpartum Women with Mental Disorders

Objective: This study aims to ascertain the prevalence of fatigue and depression from early postpartum to 1 month after delivery among postpartum women with mental disorders. Methods: The participants of this study were postpartum women who had delivered a child after a full-term pregnancy and were suffering from a mental disorder. We administered the Postpartum Fatigue Scale (PFS) on days 1, 3, and 4 (to multiparas), and 1, 3, and 5 (to primiparas), and 1 month after delivery to all participants. The Edinburgh Postnatal Depression Scale (EPDS), Japanese version, was also administered at the same time intervals as PFS except on day 3 after delivery. Results: A total of 7 primiparas and 9 multiparas participated in this study. The average age was 29.0 ± 7.0 years in primiparas and 32.0 ± 4.0 years in multiparas. The PFS scores were higher among the primiparas and peaked at day 1, and more than half the primiparas scored more than 9 points in EPDS. In addition, the EPDS score at 1 month had a strong correlation with the PFS subscale “mental stress situation” (r = 0.818/p = 0.047). In multiparas, the percentage of women who scored more than 9 points decreased. However, their EPDS scores at 1 month were related to the total scores of the PFS, “physical stress situation”, “mental stress situation”, and “sleep deprivation situation”. Especially, the score of “sleep deprivation situation” was higher than those during hospitalization. Conclusion: There was more fatigue 1 month after the discharge among postpartum women with mental disorders. The percentage of women whose EPDS scores were more than 9 points was high in both primiparas and multiparas. This study suggests assisting primiparas and multiparas by controlling the former’s disorder during childcare, and using family support for the latter, so that they get time to rest.

A laterally-spreading tumor in a colonic interposition treated by endoscopic submucosal dissection

Herein we describe an early colonic carcinoma which developed in a colonic interposition 14 years after surgery for esophageal cancer, which was successfully treated by endoscopic submucosal dissection (ESD). An 80-year-old man underwent colonic interposition between the upper esophagus and stomach after surgery for an early esophageal squamous cell carcinoma in 1994. He received a surveillance endoscopy, and a laterally-spreading tumor of granular type, approximately 20 mm in size, was identified in the colonic interposition. An endoscopic biopsy revealed moderately differentiated adenocarcinoma histologically, however, we diagnosed the lesion as an intramucosal carcinoma based on the endoscopic findings. The lesion was safely and completely removed en bloc by ESD using a bipolar knife. Histologically, the lesion was an intramucosal moderately differentiated adenocarcinoma in a tubular adenoma.
