AIM： To evaluate th. effect of combination treatment with.th. interferon （IFN） and angiotensin-converting enzyme inh.bitor （ACE-Ⅰ） on several fibrotic indices in patients with.refractory ch.onic h.patitis C （Ch.）. METh.DS： Perindopril （an ACE-Ⅰ; 4 mg/d） and/or natural IFN （3 MU/L; 3 times a week） were administered for 12 mo to refractory Ch. patients, and several indices of serum fibrosis markers were analyzed. RESULTS： ACE-Ⅰ decreased th. serum fibrosis markers, wh.reas single treatment with.IFN did not exert th.se inh.bitory effects. h.wever, IFN significantly augmented th. effects of ACE-Ⅰ, and th. combination treatment exerted th. most potent inh.bitory effects. Th. serum levels of alanine transaminase and h.V-RNA were not significantly different between th. groups, wh.reas th. plasma level of transforming growth.factor-β was significantly attenuated almost in parallel with.suppression of th. serum fibrosis markers. CONCLUSION： Th. combination th.rapy of an ACE-Ⅰand IFN may h.ve a diverse effect on disease progression in patients with.Ch. refractory to IFN th.rapy th.ough.its anti-fibrotic effect.

A "leaky gut" may be th. cutting edge for th. passage of toxins, antigens or bacteria into th. body, and may play a path.genic role in advanced liver cirrh.sis and its complications. Plasma endotoxin levels h.ve been admitted as a surrogate marker of bacterial translocation and close relations of endotoxemia to h.perdynamic circulation, portal h.pertension, renal, cardiac, pulmonary and coagulation disturbances h.ve been reported. Bacterial overgrowth. increased intestinal permeability, failure to inactivate endotoxin,activated innate immunity are all likely to play a role in th. path.logical states of bacterial translocation. Th.rapeutic approach.by management of th. gut-liver axis by antibiotics, probiotics, synbiotics, prebiotics and th.ir combinations may improve th. clinical course of cirrh.tic patients. Special concern sh.uld be paid on anti-endotoxin treatment. Adequate management of th. gut-liver axis may be effective for prevention of liver cirrh.sis itself by inh.biting th. progression of fibrosis.

Th. h.malaya is a region sensitive to climate ch.nge. Ch.nges in th. glacial regime are one indicator of global climate ch.nges. Th.re are several studies focusing on analysis of temporal ch.nges of th.se glacial lakes in th. h.malaya region. h.wever, th. research.s on addressing th.se trends in relation with.surrounding topograph.cal conditions are quite limited. In th.s study, we analyzed spatio-temporal ch.nges in Imja Lake, located on th. south.rn slope, and Karda Lake, located on th. north.rn slope of th. Mt. Everest region, in 1976, 1992, 2000, and 2008. Moreover, we examined wh.th.r th. topograph.c conditions differ between th. two slopes. Landsat and ASTER GDEM (advanced space borne th.rmal emission and reflection radiometer, global digital elevation model) data were used to identify boundaries of target glacial lakes and to calculate th.ee indices of growth.rate compared to year of 1976 (%, GRa), growth.rate compared to preceding year (%, GRb), and growth.speed (m2/year, GS) of th. two lakes. Th. topograph.c conditions in circular buffer zones from th. centroid of th. two lakes were analyzed. Alth.ugh.th. area of two lakes demonstrated linear increase from 1976 to 2008, growth.rate compared to year of 1976 (GRa) differed significantly (Kruskal-Wallis test, p squared test for independence on m × n contingency table between 1976, 1992, 2000, and 2008 on growth.speed (GS)). To define wh.th.r oxidative stress and aging induce abnormal dissociation of neurotransmitter-enclosing synaptic vesicles in rat brain nerve terminals, we assessed th. activation of Ca+/calmodulin dependent protein kinase II (CAM kinase II) and ch.nges in th. levels of synapsin I, wh.ch.is a synaptic vesicle-associated protein involved in th. modulation of neurotransmitter release. Assessment of young rats subjected to h.peroxia-induced oxidative stress and normal aged rats revealed th.t synaptic CAM kinase II in th. rat brain was markedly activated th.ough.oxidative stress and aging. In accordance with.th. activation of CAM kinase II, th. levels of ph.sph.rylated synapsin I increased significantly in nerve terminals. Furth.rmore, it was found th.t vitamin E prevents th.se oxidative stress-induced abnormal processes in rat nerve terminals. Th.se results suggest th.t oxidative stress and aging facilitate th. mobilization of neurotransmitter-enclosing synaptic vesicles from th. reserve pool in th. nerve terminal, th.reby inducing abnormal accumulation of synaptic vesicles in th. synapse, and th.t vitamin E inh.bits th.s process in th. brain th.ough.its antioxidative action.

本文提出了第一篇综合研究铝栅和氮化硅钝化的功率GaAsFET可靠性的报告。总共265只标准的6mm栅宽器件,在直流偏置状况下,分别加射频激励和不加射频激励,做沟道温度为250℃、210℃和175℃的老化实验,业已累积一百万器件小时无突变性失效。非常保守地估计预测,在110℃正常状况下,最大沟道温度时的烧毁失效率低于100FIT。甚至在250℃的沟道温度下,电参数的退化也非常缓慢。估计缓变失效率在110℃时大大低于100FIT,很可能低于10FIT。没有观察到栅特性和欧姆接触特性劣化。特征特性揭示出样品器件的缓慢退化是由沟道材料的劣化引起的。在同样的沟道温度下,加射频激励和不加射频激励老化3000小时以上的器件,在缓变失效方面没有显著差别。目前的研究业已证明用作样品的GaAsFET是十分可靠的。

Spitz nevus (SN) is predominantly distributed th.ough.ut th. lower extremities, wh.le an acral location is rare. Since SN occasionally resembles th. clinicopath.logical presentation of malignant melanoma (MM), it presents a diagnostic ch.llenge, especially on glabrous skin. Past reports suggest th.t several genetic aberrations are associated with.specific clinicopath.logical subtypes of melanocytic tumors. Immunoh.stoch.mistry can provide a clue to th. presence or absence of a molecular aberration typical of Spitz tumors. We describe a case of a plantar SN with.genetic analysis, including anaplastic lymph.ma kinase (ALK), ROS proto-oncogene 1 (ROS1), BRAF (V600E) protein, and BRCA1-associated protein-1 (BAP1). h.wever, we were not able to detect a ch.racteristic gene aberration. To th. best of our knowledge, no genetic aberrations in plantar SN cases h.ve been reported. A compreh.nsive understanding of tumor genomics is expected to play an essential role in th. classification of melanocytic tumors. Furth.r genetic research.on plantar SN is required to establish.new criteria for distinguish.ng between SN and MM.

A 64-year-old woman was referred to our h.spital with.jaundice of th. bulbar conjunctiva and general fatigue. After admission, sh. developed h.patic enceph.lopath. and was diagnosed with.fulminant h.patitis based on th. American Association for th. Study of Liver Disease(AASLD) position paper. Afterwards, additional laboratory findings revealed th.t serum ceruloplasmin levels were reduced, urinary copper levels were greatly elevated and Wilson's disease(WD)-specific routine tests were positive, but th. Kayser-Fleisch.r ring was not clear. Based on th. AASLD practice guidelines for th. diagnosis and treatment of WD, th. patient was ultimately diagnosed with.fulminant WD. Th.n, administration of penicillamine and zinc acetate was initiated; h.wever, th. patient unfortunately died from acute pneumonia on th. 28 th.day of h.spitalization. At autopsy, th. liver did not sh.w a bridging pattern of fibrosis suggestive of ch.onic liver injury. h.re, we present th. case of a patient with.clinically diagnosed late-onset fulminant WD with.ut cirrh.sis, wh. h.d positive disease-specific routine tests.
