Successful Management of Takayasu’s Arteritis with Pulmonary Hypertension in Pregnancy: A Rare Case

Introduction: Takayasu’s arteritis (TA) is a rare systemic, chronic inflam-matory, progressive, idiopathic disease of aorta and its main branches. Taka-yasu’s arteritis causes narrowing, occlusion and aneurysm of arteries. The eti-ology of TA is still unknown. It may be autoimmune or genetic in origin or infective diseases. It is commonly found in Asia and oriental countries. Worldwide annual incidence is 1.2 - 2.6 cases/million. Women are affected in 80% - 90% of cases with age of onset below 40 years. TA does not worsen or improve with pregnancy but has adverse effect on pregnancy in the form of abortion, Intrauterine fetal death, superimposed preeclampsia, Intrauterine growth restriction, abruption and congestive heart failure. Because the disease is common in women of childbearing age, management of pregnancy in these patients becomes important;however optimal management for pregnant pa-tients with this disease has not yet been established. Due to the manifold car-diovascular complications that can occur in the course of the disease, man-agement of pregnancies in TA patients is a challenge for the clinician. An in-terdisciplinary collaboration of obstetrician, rheumatologist, cardiologist and anaesthetist is necessary to improve maternal and fetal prognosis. This was the first reported case of TA in pregnancy in hasan Sadikin Hospital with superimposed preeclampsia and pulmonary hypertension. Materials and Methods: The patient, G1P0A0 was diagnosed as TA since one year ago, and when the patient was pregnant, the patient routinely came to obstetric and rheumatology clinics. Angiographic examination showed dilatation of aorta ascendant, narrowing of aorta descendent at 8 thoracal vertebrae, and arcus aorta, abdominal aorta and the branches were without abnormalities that lead to type IIb TA. On physical examination blood pressure was 200/100 mmHg on the right arm, and 140/100 mm Hg on the left arm, right hand pulse 120 x/m, left hand pulse 113 x/m, respiration rat

Background Acute kidney injury(AKI)is common in critically ill children with significant mortality and *** creatinine is an insensitive and late biomarker compared to newly proposed AKI *** Prospective study in pediatric intensive care unit(PICU)over three months to compare between serum cystatin-C(s-Cys-C)and urinary neutrophil gelatinase-associated lipocalin(uNGAL)as AKI biomarkers at multiple time points with pediatric risk,injury,failure,loss,end-stage renal disease(pRIFLE)classification in diagnosing *** Forty children were *** these 40 children,22 developed AKI according to pRIFLE *** was no significant difference between AKI and non-AKI in age(P=0.29).Post cardiac surgery,renal insult was the main cause of AKI(27.3%).There was a twofold increased risk of incident AKI in those patients with high baseline uNGAL at PICU admission and almost a fourfold increased risk in patients with high baseline s-Cys-C at PICU *** levels were highly predictive of AKI during the follow-up period[area under the curve(AUC)=0.76,95%confidence interval(CI)0.61-0.92].The cutoff point with the highest correctly classified proportion was 223 ng/mL(≥12 centiles)which correctly predict 80.0%patients with AKI,with a corresponding sensitivity of 72.7%and a specificity of 89.9%.AUC for s-Cys-C was 0.86(95%CI 0.75-0.97),and the highest correctly classified proportion was 1009μg/L(≥13 centiles);75%of patients with AKI,with a corresponding sensitivity of 63.6%and a specificity of 88.9%.Conclusion uNGAL and s-Cys-C predicts AKI early in critically ill children.

This study investigates the ethical orientation of students of business schools in the Middle East, with special reference to the state of Kuwait. It examines whether differences in students’ demographics and professional characteristics, including gender, academic level, participation in business ethics training programs, and working status, have any influence on their attitudes towards business ethics. Further, this study compares and contrasts the ethical orientations of business students in Kuwait and their counterparts in other countries. The pre-validated scale of Attitudes towards Business Ethics Questionnaire （ATBEQ） is adopted as a measurement instrument. For comparison purposes, the T-test for equality of means is used to test for significant differences between our results and results of research in other countries, while the ANOVA F-test is conducted to test for significant differences among countries, on a group basis. The overall mean for business ethics is 2.963. This is slightly lower than the mid-point （3）. Students responded differently by providing higher/lower levels of agreements toward the reported ethics statements. Results support the view that there is a significant difference between male and female respondents in terms of their attitudes towards ethics. The results indicate that higher academic levels （i.e., senior and graduate students） have more positive views towards business ethics compared to lower academic levels （i.e., freshman, sophomore and junior students）. Participation in business ethics training programs and working status do not have an influence on the ethical orientation of the respondents. A comparison of the overall mean of the present study with means of previous studies shows that the ethical orientations of business students in Kuwait cluster with those of the Iranian and Malaysian students. On the contrary, significant differences seem to exist between the Kuwaiti respondents and those in the USA, Australia and Israel

Optimization of DNA Staining Technology for Development of Autonomous Microbe Sensor for Injection Seawater Systems

Microbial activity in the water injection system in oil and gas industry leads to an array of challenges, including biofouling, injectivity loss, reservoir plugging, and microbiologically influenced corrosion (MIC). An effective mitigation strategy requires online and real-time monitoring of microbial activity and growth in the system so that the operators can apply and adjust counter-measures quickly and properly. The previous study [1] identified DNA staining technology-with PicoGreen and SYBR Green dyes—as a very promising method for automated, online determination of microbial cell abundance in the vast Saudi Aramco injection seawater systems. This study evaluated DNA staining technology on detection limit, automation potential, and temperature stability for the construction of automated sensor prototype. DNA staining with SYBR Green dye was determined to be better suited for online and real-time monitoring of microbial activity in the Saudi Aramco seawater systems. SYBR Green staining does not require sample pre-treatment, and the fluorescence signal intensity is more stable at elevated temperatures up to 30℃. The lower detection limit of 2 × 103/ml was achieved under the optimized conditions, which is sufficient to detect microbial numbers in Saudi Aramco injection seawater. Finally, the requirements for design and construction of SYBR-based automated sensor prototype were determined.

A stable mode-locked laser was demonstrated using a newly developed zinc phthalocyanine(ZnPc)thin film as passive saturable absorber(SA)in ytterbium-doped fiber laser(YDFL).The ZnPc thin film was obtained using a casting method and then inserted between the two fiber ferrules of a YDFL ring cavity to generate mode-locked *** resulting pulsed laser operated at a wavelength of 1034.5 nm having a repetition rate of 3.3 *** pump power of 277 mW,the maximum output power and pulse energy are achieved at 4.92 mW and 1.36 nJ,*** has a high chemical and photochemical stability,and its significance for use as a potential SA in a mode-locked laser is reported in this work.

Effect of restorative techniques on fracture resistance of endodontically treated premolars

Purpose: This study was aimed to evaluate the effect of restorative materials and glass fiber post on the fracture resistance of endodontically treated premolars. Material and Methods: Fifty extracted single rooted lower premolars were used in this in vitro study and divided into 5 groups: sound teeth (control group);mesial-occlusal-distal (MOD) preparation + endodontic treatment + composite restoration;MOD preparation + endodontic treatment + glass fiber post + composite restoration;MOD preparation + endodontic treatment + amalgam restoration;MOD preparation + endodontic treatment + glass fiber post + amalgam restoration. The specimens were loaded on a universal testing machine at the crosshead speed of1 mm/min until fracture. The data were analyzed using two-way ANOVA test. Results: The mean loads at fracture were 860.11 N, 801.79 N, 761.39 N, 737.14 N and 707.85 N respectively. There were no statistically significant differences (P > 0.05) in the fracture resistance of teeth restored with composite and amalgam, as well as restored with or without glass fiber post. Conclusions: The type of restorative material and the usage of glass fiber post do not have visible influence on the fracture resistance of the endodontically treated mandibular premolars.

In many cases of wave structure interactions,three-dimensional models are used to demonstrate real-life complex environ-ments in large domain *** the seakeeping context,predicting the motion responses in the interaction of a long body resembling a ship structure with regular waves is crucial and can be *** this work,regular waves interacting with a rigid foating structure were simulated using the open-source code based on the weakly compressible smoothed par-ticle hydrodynamics(WCSPH)method,and optimal parameters were suggested for diferent wave *** displacements were computed,and their response amplitude operators(RAOs)were found to be in good agreement with experimental,numerical,and analytical *** of numerical and experimental RAOs tended to increase at low wave frequencies,particularly at amidships and near the *** addition,the instantaneous wave contours of the sur-rounding model were examined to reveal the efects of localized waves along the structure and wave *** results indicated that the motion response from the WCSPH responds well at the highest frequency range(ω>5.235 rad/s).
